- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. BEYOND THE HORIZONS 2011 EL SALVADOR PERFORMANCE WORK STATEMENT Trash Removal Service C.1.0. GENERAL: C.1.1. SCOPE: The Contractor shall furnish all management supervision personnel, training, equipment, material, supplies, and any other equipment and personnel to accomplish the tasks of trash removal service specified herein except as specified as U.S. Government (USA) furnished. Services shall be provided during the hours and days specified herein. C.2.0. PERIOD OF PERFORMANCE: 21 February - 19 June 2011 C.3.0. GOVERNMENT-FURNISHED PROPERTY AND SERVICES: NONE C. 4.0. CONTRACTOR-FURNISHED PROPERTY AND SERVICES: The Contractor shall furnish all management supervision personnel, training, equipment, material, supplies and any other equipment and personnel to accomplish the task of providing Trash Removal Services and provisions specified herein. Services shall be provided during the hours and days specified herein. C.5. SPECIFICATIONS: The Contractor shall pick up trash from the dumpsters between the hours of 0800 and 1000 twice a week from the FOB and Eng sites each Monday and Thursday beginning on 21 February 2011. Also, the vendor shall collect trash from the up to 12 trash collection containers of up to 50 gal capacity during the same time period. C.5.1. The contractor shall provide eight (8) x eight (8) cubic yard dumpsters; as per below listed table: LOCATION DATES QTY FOB San Vicente 21 Feb-19 June 2011 3 San Antonio Caminos 27 Mar-19 June 2011 1 San Francisco Aguas Agrias 27 Mar - 19 June 20111 Nuevo Tepetitan 27 Mar - 19 June 20111 San Francisco Chamoco 27 Mar - 19 June 20111 C.5.2. All trash collected from sites specified herein shall be disposed of properly at Government of El Salvador approved disposal sites. The trash shall be disposed of at such sites in accordance with the laws of the country of El Salvador. C.6. MAKING CHANGES TO CONTRACT: The only person authorized to make changes to the contract is the Contracting Officer (KO). Failure to clear changes in contract with the KO in advance of providing service may result in the USG not being responsible for the charges, and non-payment to the Contractor. Accordingly, the KO shall be notified prior to any additional services/performances necessitating contract modification. The contractor shall only take change orders from the KO. C.7. CONTRACT MANAGEMENT REPORTING The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor shall report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor shall completely fill in all the information in the format using the following web address: https:\\cmr.army.mil. The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officer's Technical Representative; (2) Contract number, including task or delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Estimated direct labor hours (including sub-contractors); (6) Estimated direct labor dollars paid this reporting period (including sub-contractors); (7) Total payments (including sub-contractors); (8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if different); (9) Estimated data collection cost; (10) Organizational title associated with Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information); (11) Locations where contractor and sub-contractors perform the work (specified by zip code in the United States and nearest city, country, when in an overseas location, using standardized nomenclature provided on website); (12) Presence of deployment or contingency contract language; and (13) Number of contractor and sub-contractor employees deployed in theater this reporting period (by country). As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending 30 September of each government fiscal year and must be reported by 31 October of each calendar year. Contractors may use a direct XML data transfer to the database server or fill in the fields on the website. The XML direct transfer is a format for transferring files from a contractor's systems to the secure web site without the need for separate data entries for each required data element at the web site. The specific formats for the XML direct transfer may be downloaded from the website. Also, contractor shall provide the required number of dumpters as stated on paragraph C.5.1 of the PWS. Price the total cost per site for period of performance for trash removal, makimg sure to include all related costs, i.e. Insurance, Taxes, Wages, Visa Card Fee, etc. ITEM NOSUPPLIES/SERVICESQUANTITYUNITUNIT PRICE 0002 Trash Removal-Caminos 1 Lump Sum Contractor is to provide for this service as stated on the Performance Work Statement (PWS). Also, contractor shall provide the required number of dumpters as stated on paragraph C.5.1 of the PWS. Price the total cost per site for period of performance for trash removal, makimg sure to include all related costs, i.e. Insurance, Taxes, Wages, Visa Card Fee, etc. ITEM NO SUPPLIES/SERVICES QUANTITY UNITUNIT PRICE 0003 Trash Removal-Aguas Agrias1Lump Sum Contractor is to provide for this service as stated on the Performance Work Statement (PWS). Also, contractor shall provide the required number of dumpters as stated on paragraph C.5.1 of the PWS. Price the total cost per site for period of performance for trash removal, makimg sure to include all related costs, i.e. Insurance, Taxes, Wages, Visa Card Fee, etc. ITEM NO SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE 0004 Trash Removal-Tepetitan1Lump Sum Contractor is to provide for this service as stated on the Performance Work Statement (PWS). The following factors shall be used to evaluate offers: Specifications on the PWS (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
