- Ethics Policies 52.212-5 52-212-3 Business Associate Agreement Clauses Statement of Work Price Schedule This is a combined synopsis/solicitation for General Dentist Services for the Indian Health Service Hospital, Pine Ridge, SD prepared in accordance with the format in the Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This procurement is being conducted under Simplified Acquisition procedures pursuant to the authority in FAR Part 13, and FAR Part 12, Acquisition of Commercial Items (Title VII of the Federal Acquisition Act of 1994 (Public Law 103-355)). This announcement constitutes the only solicitation; therefore a written solicitation will not be issued. The Aberdeen Area Indian Health Service is seeking the General Dentist Services for the Pine Ridge IHS Hospital, Pine Ridge, SD. This is a small business set aside. The period of performance will be March 1, 2011 - December 31, 2011. The NACIS code is 621210 the small business size standard is $10 million. The solicitation documents and incorporated provisions are those in effect through 2005-48. Please fill out and sign the SF-1449 along with any applicable documents necessary to complete the quote.
