- Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is FA4855-11-Q-0018 and is a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-49, Defense Federal Acquisition Regulation Supplement Change Notice (DPN) 20110202, and Air Force Acquisition Circular (AFAC) 2010-1027. The North American Industry Classification System (NAICS) is 325998 All Other Miscellaneous Chemical Product and Preparation Manufacturing. The Offeror must be an authorized dealer and must quote on an all or none basis. Award will be based on Best Value to the Government. Written quotes are required (oral offers will not be accepted). Only one contract will be awarded. All items shall be delivered FOB DESTINATION. This solicitation is for the procurement of: Line items: 0001- REPLACE 500 GAL. FOAM BLADDER TANK, PERFORM PARTIAL SYSTEM DUMP TEST FOR APPROXIMATELY 500 GAL. OF AFFF 3% FOAM. It is the firm's or individuals' responsibility to be familiar with applicable provisions and clauses. The following Provisions and Clauses apply to this solicitation and are incorporated by full text: FAR 52.212-1 Instructions to Offerors - Commercial Items The following addendum is provided to this provision: Paragraph (b): entitled "Submission of Offers", Request for Quotation should be submitted and shall contain the following information: Solicitation Number; Time Specified for Receipt of Offers; Name; Address; Telephone Number of Offeror; Terms of the Expressed Warranty; Price; Any Discount Terms and Acknowledgement of all Solicitation Amendments. Quote shall also contain all other documentation specified herein. Signed quotes must indicate quantity, unit price and total amount. Offers may be submitted via facsimile, e-mail or mail. Paragraph (b)(4): A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation Statement of Work (SOW) shall be submitted. This may include product literature, or other documents, as necessary to ensure what is being quoted meets the requirements of the SOW. Paragraph (c): entitled "Period of Acceptance for Offers", the offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers. Paragraph (f)(1): entitled "Late submissions, modifications, revisions, and withdrawals of offers", Late offers: Request for Quotations or modification of quotes received at the address specified for the receipt of offers after the exact time specified for receipt of offers will not be considered. FAR 52.212-2 Evaluation - Commercial Item Evaluation -- Commercial Items (Jan 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Lowest price technically acceptable. Technical will be rated on a pass/fail basis as outlined in 52.212-1 Paragraph (b)(4). (b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. FAR 52.212-3 Offeror Representations and Certifications - Commercial items. Complete only paragraph (j) of this provision if the annual representations and certificates have been completed electronically at http://orca.bpn.gov. If the annual representations and certifications have not been completed electronically at the ORCA website, then only paragraphs (b) through (i) of this provision need to be completed. FAR 52.252-1 Solicitation Provisions Incorporated by Reference. This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The offeror is cautioned that the listed provisions may include blocks that must be completed by the offeror and submitted with its quotation or offer.
