- Cover letter, 1449, and Buy List This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 under Simplified Acquisition procedures in Subpart 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ) Number 11-233-SOL-00080. This acquisition is being conducted as a Total Small Business Set Aside. The associated North American Industry Classification System (NAICS) Code is 315299. This requirement is being issued as Brand Name or Equal. Please see attached in PDF format. Manufacture's name, part number, and NSN number are provided to assist in further defining the Agency requirements only. New Equipment ONLY. All items must be covered by the manufacturer's warranty. In responding to this solicitation/request for quote, you must provide the manufacturer's name and part numbers. If you are quoting on an "or equal" basis, your specification should indicate: Quoting on Manufacturer (Insert Name) and Part Number (Insert Number), containing the following salient characteristics (insert salient characteristics). If your quote response does not contain all of the required information called out in this notice, your quote will be determined nonresponsive and will not be considered for award. Salient characteristics are defined as "pronounced features of an item that identify it describe its size and composition, functional intent, and/or operational capabilities or limits." When researching product capabilities, you must, as a minimum, respond with feature for feature comparisons in order for your product to be technically evaluated for sufficiency. All "equals" will be evaluated to ensure that they meet the same specifications and characterizes of the requirement. Quote must be good for 30 calendar days after close of the RFQ. Delivery must be made within 30 days or less after receipt of order (ARO). The offeror must provide, within its offer, the number of days - not to exceed 30 - required to make delivery after it receives a purchase order from the buyer. Offeror must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the Offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov. It is also required any contractor doing business with the government under contract be registered in ORCA, website at https://orca.bpn.gov/ Quotes are due Wednesday, February 23, 2011 by 0700 Eastern Standard Time, submit written offers (electronically)(oral offers will not be accepted). The quote must reference the RFQ Number 11-233-SOL-00080. All responsible sources may submit a proposal, which if timely received, shall be considered by the agency. Proposals must be submitted electronically (via email) to Micah Kauzlarich, Contract Specialist, at micah.kauzlarich@psc.hhs.gov The award will be made based upon the requirements specified in this combined/synopsis, and to the Offeror providing the best value in meeting the Government's requirements. The Government intends to evaluate offerors and may award a contract without discussions with Offerors. Therefore, the initial offer should contain the offerors best terms from a technical and price standpoint. However the Government reserves the right to conduct discussions if later it is determined by the Contracting Office to be necessary. The Government may reject any or all offers, waive informalities and minor irregularities in offers received. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.211-6 Brand Name or Equal; FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR52.212-2, (a), The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be advantageous to the Government, price and other factors considered; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items.
