Statement of Work Schedule of Items Table DFAS Clauses SHREDDING SERVICES REQUEST FOR QUOTES (RFQ) NO: HQ0423-11-T-0005 This is a combined synopsis/solicitation (RFQ) to obtain services for collection, shredding/destruction and disposal of Government paper/material on an as needed basis at Defense Finance and Account Service (DFAS) Cleveland, OH in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This will be a Firm Fixed Price Contract type. NAICS code for this announcement is 561990 with small business size standard $7.0 million. This acquisition is a total small business set-aside. SOLICITATION CLOSING DATE : All quotes must be received by Close of Business. EST, Friday, February 25, 2011. Preferred delivery method is via email : carrie.fairbanks@dfas.mil. Quotes shall include in addition pricing: CCR Cage Code; DUN & Bradstreet number; Tax Identification Number; and Company Point of Contact Name, Phone Number and Email Address. Note: A Schedule of Items Table has been included as an attachment to this announcement to assist with submission of pricing for your convenience. If you elect to not use the table, please furnish prices using in a format similar in nature by providing unit prices per Bin for the Line Items specified below. Price Schedule: Base Year: CLIN 0001 - Collection, shredding/destruction and disposal of Government paper/material on an as needed basis as follows: (Note: a Bin is defined as a 65 gallon tote.) Site 1: Anthony J. Celebrezze Federal Building. Estimated quantity for pickup on an as needed basis (estimated weekly) for paper and related products is 20 – 35 bins. Estimated quantity for pickup on an as needed basis (estimated semi-annually) for diskettes and related products is 2 bins (estimated 2,000 diskettes/bin). Estimated quantity for pickup on an as needed basis (estimated annually) for hard drives is 1 bin (estimated 300 hard drives). Unit price shall be provided per bin. Site 2: North Point Building. Estimated quantity for pickup on an as needed basis (estimated every other week) for paper and related products is 8 – 10 bins. Unit price shall be provided per bin. Site 3: Isaac I. Campbell (Bratenahl ). Estimated quantity for pickup on an as needed basis (estimated every other week) for paper and related products is 4 – 8 bins. Unit price shall be provided per bin. Campbell (Bratenahl), 555 East 88 th, Cleveland, OH 44108 Period of performance will be one (1) twelve (12) month base period and three (3) twelve (12) month option periods. Note: For calculation purposes, the maximum estimated quantity for number of Bins per site will be used to calculate total price based on unit price provided for the line items specified above for evaluation.
