Description
1 GENERAL 1.1 BACKGROUND INFORMATION: Vendor shall provide Eisenhower Army Medical Center (EAMC), Department of Pathology, Fort Gordon GA with maintenance services for the existing apheresis equipment and shall provide a qualified service staff, necessary test equipment and parts inven tory, as required. Vendor shall schedule maintenance visits at mutually agreed time during normal duty hours (7:00 A.M.-7:00 P.M. EST, Monday - Friday, excluding Holidays). (a)Performance assurance inspection shall include a complete inspection and calibration of each unit described in the Performance Based Work Statement List of Equipment Covered Section in accordance with manufacturer service manual. Following the performance assurance inspection, a copy of the completed checklist and calibration sticker shall be provided to the EAMC Biomedical Maintenance/Clinical Engineer Manager. (b)Emergency and repair services shall be provided within 24 hours, Monday through Friday, 7 A.M. to 7 P.M., except holidays. Service shall include labor, travel, expenses and all replacement parts (except disposables) for the equipment as required. Service provided for all maintenance shall be performed in accordance with established procedures, as outlined in the current revision of the Original Equipment Manual (OEM). 1.2 Vendor shall provide at a cost per kit rate, consumables required to perform 60 to 80 apheresis collections a month. Consumables must meet or exceed regulatory standard as set forth by Food and Drug Administration, College of American Pathologist, and American Association of Blood Banks. 1.3 PERSONNEL. Services under this contract shall be performed by individuals who have demonstrated knowledge and competence in the servicing and maintaining specific instrumentation under the contract. The Contractor shall ensure repair technicians are trained on the specific equipment under contract and have two (2) years of experience in instrumentation chosen to meet the requirement of the EAMC Blood Services. 1.3.1. TIME LIMITS. The service coverage for instruments is 5 days per week, Monday - Friday from 08:00 A.M.-5:00 P.M. Phone technical service shall be available 7 days a weeks, 24 hours a day. 1.4 PHYSICAL SECURITY. Contractor personnel or any representative of the contractor entering Fort Gordon shall abide by all security regulations and shall be subject to security checks. 1.5 PERSONAL APPEARANCE. Contractor personnel working under this contract shall practice high standards of personal hygiene and maintain a clean, neat appearance while on duty. 1.6 QUALITY CONTROL. The contractor shall ensure that the quality of service and materials provided under this contract meet or exceed the performance work statement. 1.6.1 QUALITY ASSURANCE. Upon completion of work, an acceptance inspection shall be conducted by the Government personnel from Biomedical Maintenance Branch and equipment user to ensure services are complete. The Government will evaluate the contractor's performance under this contract using the method of surveillance specified in Quality Assurance Schedule attached. All surveillance observations will be recorded by the Government. When an observation indicates defective performance, the Contracting Officer's Representative (COR) will obtain the contractor's representative's initials on the record of the observation. All Material Safety Data Sheets associated with the consumables shall be provided to EAMC Laboratory upon receipt of consumables or products. 1.6.2 COMPLETION OF SERVICES. Upon completion of services, a written service report shall be provided to the Biomedical Maintenance/ Clinical Engineering Manager. The service report shall be provided within 24 hours from completion of service. The report shall include detailed information regarding the cause of equipment malfunction and corrective action taken, to include the time required to complete the work, price of labor hourly rate, and a list of parts replaced with a price for each part. In the event all information is not available to contractor's representative when services are performed, the initial service report shall include all information stated above except price. Contractor shall provide the balance of the required information in writing to the Chief, Biomedical Maintenance Branch, not later than ten (10) days after services are completed. 1.6.3 Upon completion of PMCS, the contractor shall sign off on or affix a DD Form 2163 Calibration Verification sticker in accordance with instruction provided in TB 38-750-2. 1.7 SOFTWARE COVERAGE: Customer shall receive software upgrades at no additional charge to the government. All upgrades shall be installed as soon as the upgrades are available. Any training needed after the upgrades shall be the contractors responsibility coordinating with the Department of Pathology. After any software installation or upgrades are completed the operators and service manuals must be updated by the contractor. 1.8 PAYMENTS: Payments shall be initiated through Wide Area Workflow (WAWF) by the contractor. The Laboratory Manager or the responsible designee will provide acceptance of invoice in WAWF for completed services and supplies. 1.9 RESERVED. 1.10 HOURS OF OPERATION. 1.10.1 Normal Hours. EAMC, Department of Pathology Section hours of operation are 7 days a week, 24 hours a day. 1.10.2 Emergency Services. The contractor shall perform unlimited emergency calls during normal duty hours, five days a week, excluding Federal Holidays. Response time for emergency repairs shall be within two hours after verbal notification to the contractor. Completion time shall be within 24 hours after arriving on site. 2 DEFINITIONS: 2.1 STANDARD DEFINITIONS. 2.1.1 Contracting Officer means a person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings. 2.1.2 Contracting Officer Representative (COR). An individual designated by the Contracting Officer to act as his/her representative to assist in administering contract. The source and authority for a COR are contained in the written letter of designation. 2.1.3 Customer Complaint. A means of documenting certain kinds of contract service problems. A government program that is explained to every organization that receive service under this contract which is used to evaluate contractor's performance. 2.1.4 Defective Performance. A service output that does not meet the standard of performance associated within the Performance Requirements Summary (PRS). 2.1.5 Government Property. All property owned or leased to the government or acquired by the Government under the terms of the contract. Government property includes both government furnished property and contractor acquired property as defined in FAR 45.101. 2.1.6 Material Safety Data Sheet (MSDS). A document that contains information on the potential health effects of exposure to chemicals, or other potentially dangerous substances, and on safe working procedures when handling chemical products. 2.1.7 Performance Requirement. The point that divides acceptable and unacceptable performance. When the method of surveillance is other than random sampling, the performance requirement is the number of defectives or maximum percent defective in the lot before the government will affect the price computation system in accordance with the Performance Requirements Summary and the Inspection of Services Clauses. 2.1.8 Performance Requirements Summary (PRS). Identifies the key service outputs of the contract that will be evaluated by the government to assure contract performance standards are met by the contractor. 2.1.9 Quality Assurance (QA). Those actions taken by the government to assure services meet the requirements of the Performance Work Statement (PWS) and all other service outputs. 2.1.10 Quality Assurance Surveillance Plan (QASP). An organized written document used for the quality assurance surveillance. The document contains specific methods to perform surveillance of the contractor. 2.1.11 Quality Control. Those actions taken by a contractor to control the performance of services so that they meet the requirements of the PWS. 2.1.12 Sample. A sample consists of one or more service outputs drawn from a lot. The number of outputs in the sample is the sample rate. 2.1.13 Defective Performance. A service output that does not meet the standard of performance associated with it in the Performance Requirements Summary (PRS). 2.1.14 RESERVED. 2.1.15 Food and Drug Administration (FDA) Federal regulatory body that has purview of the manufacture and distribution of blood and blood products 2.1.16 College of American Pathologist (CAP). Peer review body of the laboratory industry that ensures compliance with CLIA 88 and CLIP for DoD clinical laboratories 2.1.17 American Association of Blood Banks (AABB). Peer review body of the Blood industry that ensures develops standard for blood collecting center in compliance with federal statutes and CFR. 3 CONTRACTOR FURNISHED ITEMS AND SERVICES. 3.1 The contractor shall provide all necessary tools and equipment to provide maintenance and repairs on the instrument. Any cost associated with travel, lodging or meals to conduct services or preventive maintenance shall be at the cost of the vendor with no cost to the government. 4 SPECIFIC TASKS. 4.1 The contractor shall ensure proper packaging and delivery of consumables to the Government. Delivery Schedule is to be determined by Department of Pathology listed. 4.2 The contractor shall notify the Government within 24 hours of a recalls on consumables. All consumables found to be unsafe or unreliable shall be replaced at no cost to the government. 4.3 The contractor is responsible for submission of all invoices in Wide Area Work flow in order for payment to be processed in a timely manner. 4.4 Any consumables sent outside or above or beyond the contract listed consumables are at the cost of the contractor. 4.5 Upon completion of PMCS, the contractor shall sign off on or affix a DD Form 2163 Calibration verification sticker in accordance with instruction provided in TB 38-750-2. 4.6 Customer shall receive software upgrades at no additional charge to the government.
