NA (15) Contractor. NA (16) Description. The James E. VanZandt VA Medical Center intends to award a requirements type contract to provide all supplies, materials, labor, supervision, transportation, patient education, safety management and infection control for home oxygen services to eligible beneficiaries as described in this performance work statement (PWS). The successful offeror shall provide service for all beneficiaries covered by the facility at the contract rates, regardless of the physical location of the beneficiary. The James E. Van Zandt VA Medical Center currently serves an estimated 300 oxygen using patients and 535 C-Pap/Bi-Pap patients. This volume or amount shown in the solicitation are estimates only for pricing purposes and impose no obligation on the VA either minimum or maximum. The contract shall be for the actual requirements of the VA as ordered by the VA during the life of the contract. (17) Place of Contract Performance. Area of coverage for the James E VanZandt VA Medical Center: The contractor shall provide services to the area of coverage as specified below for the VA Medical Center, 2907 Pleasant Valley Blvd, Altoona, PA 16602. The following is a list of the counties in the State of Pennsylvania that shall be provided service under the terms and conditions of this contract: Bedford Blair Cambria Cameron Centre Clearfield Clinton Elk Huntingdon Indiana Jefferson Juniata Mifflin Somerset (18) Set-aside Status. This procurement is set-aside for HUBZone Small Businesses (b) Transmittal. This solicitation will be available for 30 days via the Internet at http://www.fedbizopps.gov on March 15, 2011. (c) "Description." (1) National Stock Number (NSN) if assigned. NA (2) Specification and whether an offeror, its product, or service must meet a qualification requirement in order to be eligible for award, and identification of the office from which additional information about the qualification requirement may be obtained (see Subpart 9.2). The contractor must be Joint Commission (JC) or equivalent accredited and must maintain accreditation for the duration of the contract. NA (13) (i) If the solicitation will include the FAR clause at 52.225-3, Buy American Act-Free Trade Agreements-Israeli Trade Act, or an equivalent agency clause, insert the following notice in the synopsis; "One or more of the items under this acquisition is subject to Free Trade Agreements." (ii) If the solicitation will include the FAR clause at 52.225-5, Trade Agreements, or an equivalent agency clause, insert the following notice in the synopsis: "One or more of the items under this acquisition is subject to the World Trade Organization Government Procurement Agreement and Free Trade Agreements."
