- statement of work 52 212-2 CLAUSE 52 212-3 CLAUSE 52 212-5 CLAUSE RFQ 11 130 - SF 18 52 252-1 CLAUSE This is a combined synposis/solicitation for commerical items prepared in accordance with the format in Subpart 12.6, as supplements with additional information included in this notice. This announcements constitutes the only solicitation; proposals are being requested and written solications will not be issued. The Aberdeen Area Indian Health Service (IHS) intends to award a firm fixed price non-personal services type purchase order in response to Request of Quotes (RFQ) 11-130. This for a base year plus four(4) one-year option years. The solication documnets and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-38. The associated North American Industry Classification Sstem Code is 811219. PURPOSE OF CONTRACT: SEE ATTACHED STATEMENT OF WORK: Location is at the Sioux San IHS Hospital, 3200 Canyon Lake Drive, Rapid City, SD 57702. PRICE SCHEDULE: MAINTENANCE AGREEMENT FOR GE PROTEUS X-RAY UNIT- The quoted unit pricing must be an all inclusive cost( to included by not be limited to transportation, food, lodging, per diem and fringe benefits). The contractor is responsible for all associated costs for the maintenance agreement. BASE YEAR (MARCH - SEPTEMBER, 2011) approximately 7 months of service to be provided $_________________________ OPTION YEAR ONE (10/01/2011 - 09/30/2012) approximately 12 months of service to be provided $________________________ OPTION YEAR TWO (10/01/2012 - 09/30/2013) approximately 12 months of service to be provided $________________________ OPTION YEAR THREE(10/01/2013 - 09/30/2014) approximately 12 months of service to be provided $________________________ OPTION YEAR FOUR(10/01/2014 - 09/30/2015) approximately 12 months of service to be provided $________________________ ATTACHED ARE THE FEDERAL ACQUISITION REGULATIONS (FAR) AND THE HEALTH AND HUMAN SERVICES ACQUISITION REGULATION (HHSAR) CLAUSES THAT ARE APPLICABLE INCLUDING THE FOLLOWING: FAR 52.212.-1 INSTRUCTION TO OFFERS - COMMERCIAL ITEMS (JUNE 2008): 1. Technical description of the service being offered in sufficient detail to evaluate compliance with the requirements in the solication. 5. Terms of any express warranty 6. Price and any discount 7. A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. FAR 52.212-2 EVALUATION - COMMERICAL ITEMS (JANUARY 1999) Please respond to the below technical evaluation questions and submit with other needed information. A. METHODOLOGY AND APPROACH (50 POSSIBLE POINTS) Contractor will provide services as defined below and adhere to all time frames. Included all parts, labor and travel to keep Protues x-ray unit operating to manufacturer's specifications including the x-ray tube. FE Onsite Cov. Weekdays: Mon - Fri, 8am-5pm FE Onsite Response time: 6 hours Insite/Tech. Phone support Insite Response Time: Immediate Parts Delivery: Priority QA Cov. Weekdays: Within Contract Hours TiP Answer Line Uptime Committment: 97% QA Frequency: 1-QA/EQC(including1-IQC) per year INDICATE STRENGTHS: INDICATE WEAKNESSES: B. PAST PERFORMANCE (50 POSSIBLE POINTS) The contractor has all documentation required by servicing company and issue's an invoice within 72 hours of service/PM Past history of the contractor's company will be reviewed and evaluated. Provide listing and contract information of contract(government and others) of simliar nature to this contract.
