- DESCRIPTION/REQUIREMENTS/WORK STATEMENT 1. SCOPE: The contractor shall provide Preview CT scan reconstruction and case planning services for the veterans eligible for care at the North Florida/South Georgia Veterans Health System (hereinafter referred to as NF/SGVHS), Department of Veterans Affairs (VA) in accordance with the terms and conditions stated herein. Services shall be performed at the location of the contractor. 2. TERM OF AGREEMENT: This agreement is effective on or about March 1, 2011 - February 28, 2012 with four option year renewals and is subject to availability of funding after September 30 of each fiscal year. Initial year:March 1, 2011 - February 28, 2012 First option year:March 1, 2012 - February 28, 2013 Second option year: March 1, 2013 - February 28, 2014 Third option year: March 1, 2014 - February 28, 2015 Forth option year: March 1, 2015 - February 28, 2016 Contractor shall have no obligation to provide services to the VA until it has received written notice from the VA that funding is available. The contractor will insure that the product received is protected with encryption to assure patient privacy. 3. PERFORMANCE: The contractor shall perform 3D reconstruction and case planning service based on their Preview software application for the treatment and care for eligible veteran beneficiaries as referred and authorized by NF/SGVHS. A CD is made (as discussed in #4) and then sent via UPS/Federal Express to the requestor. In addition, the contractor will guarantee secure electronic access for review of the product to avoid potential delay in patient care. The services will be performed within the VA policies and procedures and the regulations of the medical staff by laws of the NF/SGVHS. 4. REQUIREMENTS OF THE PREVIEW SOFTWARE GENERATED PRODUCT: The specific services to be performed include the acquisition, upload, and assessment of quality of digital CT scan images, multiplanar reformatting of digital data, and production of a 3D reconstruction tool for use in case planning for endovascular procedures as related to: "CT Based Image Reconstruction - includes acquisition and assessment of digital data quality, transferring and reformatting the data to create a 3D reconstruction image suite for endovascular case planning. "Provision of Hard-Copy Preview Product - return by next-day services a CD containing all of the original data, the reformatted, data, the 3D reconstruction images, and the Preview Software self-contained for viewing, analysis, and case planning. "Provision of Secure Electronic Access to Preview Product - electronic access via a secure, password-protected account to the original images data, reformatted image data, 3D reconstructed images, and Preview Software for remote or urgent access for viewing, analysis, and case planning. "Provision of Software Updates and Technical Support - regular announcement/delivery of software updates either contained within CD packaging or via online services; availability of technical support during regular business hours either online or via telephone. The above identified services will be available to the Vascular Section of Surgical Service.
