Description
The Department of Veterans Affairs will issue a Request for Proposal (RFP) for two models of Power Wheelchairs, (Compact and Standard). The RFP will be posted on March 15,, 2011 and will close June 15, 2011 at 1200 Noon Central Time. The RFP will be for a Base Year with four Option Years. Estimated Yearly Use is: Compact Power Wheelchair2405/year Standard Power Wheelchair4109/year This will be a Full and Open RFP. The Trade Agreements Act will apply to all items on the Schedule of Supplies. Approvals & Test Reports "Food & Drug Administration (FDA): All submitted offers must be FDA (510(k)) approved with supporting documentation included. Failure to provide evidence of FDA approval will result in the offer being rejected and withdrawn from further consideration. "Rehabilitation Engineering and Assistive Technology Society of North America (RESNA) Standards: A RESNA test report must be included for each sample power wheelchair submitted. Failure to provide a comprehensive RESNA test report or submitting a report which is incomplete or not legible will result in the submitted sample being rejected and withdrawn from further consideration. RESNA test reports will be used to confirm compliance with all required specifications. Lack of compliance with specifications as listed will result in the submitted sample being rejected and withdrawn from further consideration. The offeror is REQUIRED to provide an index for the RESNA Test Report which provides the page and item number which confirms that the PWC complies with each and every required specification. Reference to product literature to confirm compliance with specifications will not suffice. RESNA test reports will be used to verify and support parameters identified as Evaluation Factors (performance, ease of use). The RESNA test report will also serve as a heavily weighted Evaluation Factor with increased credit assigned to reports that reflect testing performed by independent facilities.
