- VA Connecticut Healthcare System (VACHS) Pharmaceutical Inventory Analysis and Waste Management Services Scope of Work Background: 1.Pharmacy Department in VA Connecticut Healthcare System (VACHS) consists of In-patient, Out-patient and Research Pharmacy in West Haven, and Outpatient Pharmacy Newington Campus to serve veteran patients in Connecticut. The Pharmacy Department also provides medication supplies and technical support to medication rooms in wards and Community Based Out-patient Clinics (CBOCs). 2.The purpose of this project is to conduct a hazardous waste determination to evaluate pharmaceuticals based on current inventory, and a gap analysis on current plan and to improve waste stream identification and the pharmaceutical waste management in VACHS. The following services are appropriate for this project: Task 1: - Inventory Analysis/Waste Determination, Patented PharmE ® Waste WizardSM online Access, On-Site Risk Assessment and Gap Analysis and Evaluation Services a.Conduct inventory analysis and waste determination based on the inventory provided by VACHS including In-patient, research, methadone clinic, outpatient pharmacy and any other dispensing locations, inventory in order to properly manage pharmaceutical waste throughout our organization. Provide VACHS with a detailed analysis identifying federally hazardous waste, State of CT hazardous and regulated waste along with complete waste stream management recommendations in accordance with Resource Conservation and Recovery Act (RCRA) 40 CFR 261and State of Connecticut Department of Environmental Protection (CT-DEP) regulations. b.Provide online access of PharmE ® Waste WizardSM to unlimited number of users from VACHS to search for pharmaceutical waste disposition information and identify RCRA hazardous waste, State hazardous and regulated waste, and other hazardous drugs, such as those listed on the OSHA or NIOSH Hazardous Drug Alert. c.Conduct on-site risk assessment and gap analysis: A review of VACHS's current hazardous pharmaceutical waste management plan, policies shall be conducted. A walk-through of areas involved in pharmaceutical waste management practices shall be conducted to evaluate current pharmaceutical management practices and procedures. Develop a gap analysis to determine the level of risk relative to the Federal EPA RCRA regulations and the regulations of CT-DEP. a.At the conclusion of the on-site assessment and gap analysis, the vendor will provide a closing conference/seminar to key organizational leaders regarding their findings, comparison of pertinent statutes, regulations and delivery of the findings. The closing conference/seminar will include a discussion of the consultants suggested action plan. Task 2: - Implementation Support and Training Services: Provide a training program for Supervisors and organizational leaders regarding organizational compliance with the pharmaceutical waste management program. This training must include at a minimum, 1.Overview of regulations both State and Federal that impact on waste management 2.Definition of RCRA pharmaceutical wastes 3.Review of our formulary and the associated waste determinations 4.Explanation of VACHS pharmaceutical waste management program 5.Explanation of expectations of supervisors in support of this compliance program. Training sessions for all staff that handle pharmaceuticals will be provided. This level of training will focus on the responsibilities of the employee that will be handing the drug and generating the waste as well as inventory management, storage, and handling of pharmaceuticals. The vendor shall provide a written report, documenting the findings, recommendations, and a customized action plan. The report shall include a cross-walk between environmental regulations and extensive regulatory as well as interpretive references to justify and support their recommendations. Three (3) hard copies and one electronic copy of the report shall be provided to VA Safety Office. All data tables may be provided in Microsoft Excel format. Final payment for services will not be made until all services and project documents are received.
