- PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number VA-258-11-RP-0129 is issued as a Request for Proposal (RFP). This solicitation and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-49. This solicitation will be 100% Set Aside for SDVOSB and the associated North American Industry Classification System (NAICS) code for this requirement is 541350 with a size standard of $7.0 Million. See Attachment 1: Pricing Doc, Offeror's are to use this attachment to submit pricing information. Scope of Contract: This solicitation will result in a firm-fixed price contract for Joint Commission's Statement of Conditions (SOC) Survey for Patient Care Buildings at the Albuquerque VA Medical Center. Prices awarded will remain in effect for the contract period without an increase in price. Project start date shall be within 14 days of award. See attachment 6 for Statement of Work. Solicitation questions shall be submitted by 03-18-2011 2:00 PM EST to Carlos Jaquez, Contract Specialist at carlos.jaquez@va.gov. Any questions submitted after this period, will not be answered. 52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (JUN 2008) 1.Your response will also include two (2) separate files/attachment named a)Pricing Doc. b)Technical/Experience Proposal Doc. NOTE: Your proposed pricing should be in the format outlined in Attachment 1. 2.The Technical/Experience Proposal should be in a format which clearly addresses the evaluation factors found in Attachment 3. Each response shall address each factor and clearly identify which factor is being addressed. This is not intended to restrict answering in as much detail as the Offeror deems necessary to adequately present and address each factor in the technical proposal. 3.You must also provide the Past Performance Information sheets (Attachment 2) to three (3) business references who have first-hand knowledge of performance relative to the same type of service being requested, dates of contract performance, and total contract amount awarded. Your business references should email this form to Carlos Jaquez, Contract Specialist, at carlos.jaquez@va.gov no later than March 25, 2011, 2:00 PM EST. The Offeror shall ensure that the documentation required in support of Past Performance is supplied by the evaluating agency to the Contracting Officer. It is the Offeror's responsibility to follow up with the evaluating customer to ascertain if the required information has been forwarded prior to the established closing date. The Government will not seek out corrections to incomplete or insufficient information on behalf of Offerors. Unverifiable information will not be considered in the past performance assessment. NOTES: No Past Performance Survey submittals will be accepted directly from the Offeror being evaluated. The Offeror should verify completion and submittal of the forms with the agencies/companies to which they provided the questionnaire. Electronic Mail Transmittals: The completed survey must be emailed to: Carlos.jaquez@va.gov 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: BEST VALUE PROCUREMENT: INSTRUCTIONS TO OFFERORS - COMMERCIAL ITEMS ADDENDUM A contract award will be made based on the overall Best Value to the Government. Technical/Experience and Past Performance (all of equal value) when combined are more important than Price. Price/Cost will be considered in the overall evaluation of the offered services to ensure award is made to the Offeror presenting the best value to the Government. Award may be made to an Offer that is not the lowest price. All quotes received must meet the requirements found in the Statement of Work to be considered. Qualifications: Offers will be considered only from Offerors who are regularly established in the business called for and who in the judgment of the Contracting Officer are financially responsible and able to show evidence of their reliability, ability, experience, equipment, facilities and personnel directly employed or supervised by them to render prompt and satisfactory service in the volume required for all items under this contract. Evaluation Criteria: a. The technical portion of the proposal will be very important in the award of a possible contract and should, therefore, be as complete and specific as possible, as the evaluation team will rate technical proposals only on detailed information as required by the solicitation and as submitted by the Offeror. b. The merits of each proposal will be carefully evaluated in terms of the requirements and in relation to the criteria established below. The evaluation will take into consideration the Offeror's technical and administrative capabilities of the proposal in relation to the needs of the El Paso VAHCS and the work to be accomplished. 1.Technical/Experience Proposal (The subfactors are of equal value): The technical proposal is to include the following. Failure to address any one of the items listed will result in the rejection of the offer. Proposals will be evaluated on the basis of technical/Experience/Evidence factors / information found in Attachment 3 and Section B.2.K. NO PRICE INFORMATION SHALL BE INCLUDED IN THE TECHNICAL/EXPERIENCE PROPOSAL. "Draft work plan describing the Offeror's plan for completing the tasks under Statement of Work. "Evidence of Surveyors degree in Fire Protection and Safety Engineer and have working knowledge and onsite experience with NFPA codes and regulations in addition to JC SOC. "Evidence of Surveyors experience with and have conducted Life Safety and Statement of Condition inspections in facilities that are built in the VA Design ® style. "Evidence of Surveyors having experience and having conducted Joint Commission (JC) inspections, Statement of Conditions reports and filed request for equivalencies and/or Plan(s) for Improvement (PFI) within the last four (4) years. "The contractor shall identify, by name, and submit a resume for the key management and technical personnel who will work on this contract. 2.Past Performance: Contractor shall demonstrate past performance by identifying a minimum of three (3) references for whom contracts of similar scope, size and complexity are either ongoing or were completed within the past three years and having references submit a past performance survey directly to Contracting. A past performance survey is attached for use. Based on the quotes received, the Government may contact some, all, or none of the references provided. A vendor's level of past performance may be evaluated solely on the information provided by that vendor. 3.Price: Price will not be rated however it will be evaluated to ensure it is fair and reasonable. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful Offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
