- Description
- The RFQ worksheet has been changed to add CLIN's for testing for asbestos and an option for asbestos abatement. The statement of work was amended to incluse testing for asbestos and asbestos abatement, if required. The purpose of this amendment is to revise the Statement of Work and RFQ worksheet. (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is F1M3E31053A001 and is being issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-49. (iv) This acquisition is 100% set-aside for small businesses. The associated NAICS code is 562910 with a size standard of 500 employees. (v) Contractors shall submit a lump sum quote to perform the mold abatement services and to replace ceiling tiles according to the statement of work. A site visit will be conducted on 10 March 2011 at 9:00 am EST. All contractors shall meet on the second floor of bldg 503, Charleston AFB. You must have a valid driver's license, proof of insurance, and vehicle registration to get on base. (vi) F1M3E31053A001: The Contractor shall provide labor, materials, and equipment necessary to accomplish complete mold abatement and ceiling tiles in room Bldg 113 on Charleston Air Force Base, South Carolina. (See attached statement of work). (vii) Repairs must begin within five (5) calendar days of award and be completed within thirty (30) calendar days of award. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers in the following relative order of importance: 1. Price. The Government will evaluate the total price of the offer for the award purposes. 2. Past Performance. Past Performance is significantly more important than Price. If the lowest priced evaluated responsible offer is judged to have an exceptional performance risk rating, that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers. Offerors shall provide 3-5 references of your most relevant and recent projects (within three years).
