Description
STATEMENT OF WORK/SERVICES The Department of Veterans Affairs (VA), Medical Center, Fayetteville, Arkansas has a need for Internal Medicine Hospitalist/Inpatient Physician to begin AS SOON AS POSSIBLE, on or around July 1, 2011, pending satisfactory Credentialing and Privileging. The assignment will be for approximately one (1) year {12 full calendar months} and will be located at the Department of Veterans Affairs, Veterans Health Care Systems of the Ozark (VHSO), VA Medical Center, 1100 N. College Avenue, Fayetteville, Arkansas in accordance with the terms and conditions of this contract. The physician(s) will provide 100% outpatient care as a primary care physician, working on a team that consists of one (1) physician, one (1) nurse, and one (1) clerk. The position is for a Board Certified or Board Eligible Internal Medicine Hospitalist/Inpatient Physician certified by the American Board of Medical Specialties (ABMS) or the Bureau of Osteopathic Specialist (BOS). BLS and ACLS certification is required. Average fifteen (15) patients per day, mostly male with an average age of 65 and some female veterans, only minor procedures may be required. Hours shall be 8:00 a.m. through 4:30 p.m., with a ½ hour lunch, Monday thru Friday, excluding Saturday, Sundays or Holidays. There shall be no overtime (OT) without prior written approval. There are NO night call or ON call expectations for this position. The physician must be currently ACLS certified and maintain that certification during the duration of the contract. Candidates with prior VA experience and knowledge of the VA's computerized patient record system are preferred. Vetpro is also required, if not already in the program. Candidates must provide no less than three (3) current references of work for the past year; references must be forwarded with the CV. Candidates must have a recent physical, within one year of selection, and have had a TB Test read with test results when arriving for duty. NOTE::: HEALTH CARE PROVIDERS ARE REQUIRED TO HAVE A NPI NUMBER WHEN ARRIVING FOR DUTY. Two (2) forms of identification, such as current pictured driver's license and original Social Security card must be presented when candidate reports for duty. Additional Security requirements require physicians to be electronically fingerprinted. Physicians will work contingent on satisfactory fingerprinting results. This requirement is in additional to VHA Directive 0710, dated August 19, 2005, which outlines the VA's Personnel Suitability and Security Program. Further information on electronic fingerprinting requirements will be given upon selection of physician. SCHEDULE OF SERVICES 1. SERVICES: The Contractor shall provide to and at the Department of Veterans Affairs, Veterans Health Care Systems of the Ozark (VHSO), VA Medical Center, 1100 N. College Avenue, Fayetteville, Arkansas the services of a Board Certified Internal Medicine Hospitalist/Inpatient Physician to perform services as per the requirements of this contract. The Contractor personnel shall be responsible for providing the specified services in accordance with the terms and conditions of the contract. The Statement of Work and any applicable terms and conditions specified herein shall be included in any subcontracts. a. The selected candidate(s) shall be responsible for all activities of the medical center during the assigned tour of duty. During the tour of duty, the physician is responsible for the care of all patients in the medical center including the Intensive Care Unit, providing consultations to nursing staff regarding medical care of patients, responding in person or by telephone, as appropriate, to patients whose condition becomes acutely urgent or emergent, and in person to patients who experience cardiopulmonary arrest. The physician shall provide evaluation and treatment for patients presenting to Urgent Care. b. Evaluation of patients: Evaluate patients who are presented for care or whose condition requires evaluation by a physician which include the following: (1) A detailed and complete history and physical will be accomplished. (2) Orders and interprets appropriate diagnostic studies. (3) Development of a plan of care. (4) Writes appropriate orders and/or prescriptions. (5) Establishes plans for follow-up care including appropriate consultations and appointments. (6) Effective communication of plan of care with patient and/or family. (7) Assures appropriate disposition of patient: admission, transfer to another facility or discharge to home. (8) Establish communication in the medical record for the patient's primary care physician or to the physician to which the patient is referred or transferred. c. Admission of patients which includes the following: (1) Complete and detailed history and physical examination. (2) Initial diagnosis(ses) of patient's condition. (3) Write initial admission orders consistent with the initial care plan until Attending Staff are available. (4) Write outpatient orders and/or prescriptions and order appointments. (5) Order medications for patients not requiring hospitalization. (6) Provide direct physician care to patients during tour of duty hours. (7) Write medical orders needed for treatment, as well as consultation and patient transfers. d. Serve as Code Blue Team Leader at all cardiopulmonary arrests occurring on the inpatient service and Urgent Care (A code blue pager will be provided). Also, provide medical care and/or consultation to assure patient stabilization until a staff physician is available to assume responsibility for the patient's care. e. In the absence of the primary care physician, serve as attending physician to those inpatients whose condition becomes acutely urgent or emergent. f. Dictation of death or transfer summary at the time of the event in the Urgent Care Area. g. Charting requirements: Documentation of all care provided and planned or recommended including as appropriate, medical prescriptions. This information will be entered in each patient's medical record with a frequency that allows for timely implementation of medical plans and reflection of care provided. This includes, at a minimum, one timed note for each episode of care. Documentation of employee visits must be completed in the employee health records. h. Consultation: Consultation will be provided, as needed, to nursing staff, ancillary staff, and medical staff to assure timely, safe and appropriate implementation of all plans of care. i. Physician shall participate in activities of the medical center that are designed to meet the requirements of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) pertaining to Quality Improvement (QI) for which medical staff are responsible, i.e., maintain medical records and participate in peer review. The physician shall be responsible for assuring that medical records are completed in a timely fashion satisfying VHA and Medical Center Policies and JCAHO requirements within times documented on the chart. j. The ordering of diagnostic laboratory procedures appropriate for the patient. k. Evaluate and report any incident or adverse event in accordance with current VA or Medical Center policy and procedures. l. Referral, when indicated, of patients with multiple health problems to other providers within the hospital; consultations with physicians and others, when requested, to augment the care of their patients. m. Emergency or Urgent Care: The physician shall report any patient's need of immediate medical attention (including surgery). 2. STATISTICS: a. Monday through Friday - 8:00 p.m. to 8:00 a.m. - a daily average number of ten (10) patients are seen in Urgent Care. Approximately three (3) are admitted. Approximately two (2) inpatients require attention. b. Saturday and Sunday - A daily average number of forty (40) patients are seen in Urgent Care. Approximately four (4) are admitted. Approximately ten (10) inpatients require attention. 3. SUBSTITUTION OF PHYSICIANS: Contractor shall assign to this contract only those persons whose resumes, personnel data forms, or personnel qualification statements were submitted and approved as required. No substitution of personnel shall be made except as follows: a. All proposed substitutions must be submitted in writing at least 60 days in advance of proposed substitution to the Contracting Officer. Each request shall provide a detailed explanation of the circumstances necessitating the proposed substitutions, a complete resume for the proposed substitute and any other information required by the Contracting Officer to approve or disapprove the proposed substitution. All proposed substitutes shall have qualifications that are equal to or higher than the qualifications of the person being replaced. b. The Contracting Officer shall evaluate requests for substitution of personnel and promptly notify the Contractor, in writing, of whether the request is approved or disapproved. c. The physician must sign all verbal and telephonic orders prior to leaving the facility. d. Medications will be prescribed in the Urgent Care Area in accordance with Medical Center policy. Routine narcotic refills will not be given. 4. HOURS: Monday through Friday, 8:00 p.m. - 8:00 a.m. Contractor will be required to have personnel available to perform the services from 8:00 p.m. to 8:00 am, Monday through Friday. Contractor shall provide coverage 24 hours a day (8:00 a.m. - 8:00 a.m.) on weekends and federal holidays. The physician will not be relieved of his/her responsibilities until a replacement has personally reported to the Admitting Office of the Administrative Officer of the Day (AOD) for duty. All physicians who are to work under the terms of this contract are to be scheduled by the Contractor. 5. ABSENCES: The nature of this contract does not provide for any absence (i.e., sick leave, holiday or vacation time). Should an unavoidable delay occur, the contractor shall immediately notify the COTR, or designee, at 479-444-5016, Monday through Friday, 8:00 a.m. to 4:30 p.m. 6. CONTRACT MONITORING PROCEDURES: a. Record-Keeping Monitoring System: A record will be established by the Contracting Officer's Technical Representative (COTR) for individual services for verification of actual procedures performed by the Contractor. The COTR to monitor contract compliance is the Chief, Medical Service, or designee, and is the VA official responsible for verification of actual procedures performed by the Contractor. Failure of contractor personnel to follow procedure may result in nonpayment or delay of payment. b. Physician shall check in with the Administrative Officer of the Day (AOD) or designee upon arrival. A record will be established for verification of actual performance. c. COTR or designee will randomly check medical records to insure contractor makes documentation and verbal orders are signed, dated and timed. d. COTR or designee will periodically interview patients. e. The COTR will review prescribing practices of the physician. f. The physician could be subject to random drug screening at the discretion of the facility. g. As requested by the COTR, the contractor shall meet (conference calls are acceptable) with the COTR to discuss contract performance, process improvement ideas, and the status of any contract issues. h. The COTR will furnish a statement in writing to the Contracting Officer at the close out of the contract to include summary of contractor actions and a statement that all requirements of the contract were fulfilled as agreed. i. A summary evaluation of contractor performance, based upon the compliance or noncompliance of contract requirements as evidenced under the monitoring procedures shall be forwarded by the monitoring official to the Contracting Officer prior to exercising any option. j. The Government may evaluate the quality of professional and administrative services provided, but retain no control over the medical, professional aspects of services rendered (e.g., professional judgments/diagnosis for specific medical treatment). k. A physician must be present at the VA facility and must be actually performing the required services for the period specified in the contract, or the contract cost will be decreased accordingly during each billing cycle. l The VA will not pay for "on-call" or standby time. m. Contractor and VA will establish a record keeping system to verify contract performance. All contracted physicians will sign in at the time of arrival and sign out at the end of their shift. This will be verified by on-site VA personnel. These records will demonstrate that the VA has received services called for under the contract and payment will be made in accordance with these records. Prices shown herein are "net" ceiling prices-not to exceed rates. The net ceiling rates include malpractice insurance for non-personal services task-orders and exclude travel and lodging. Personal Services task-orders will also be accepted. Minimum Qualifications of Professionals: Medical Liability Insurance Limits: $1,000,000.00 per occurrence; $3,000,000.00 aggregate. Weekend On-Call Rate: One-half of the daily rate. The weekend on-call rate is the rate of pay for a 24-hour period on Saturday and Sunday when a physician is required to be available by phone or pager for consultation or to return to the facility. Must remain within a 30-minute radius of the facility. Includes 2-hour patient contact. Not applicable to Emergency Medicine and Hospitalist. Weeknight On-Call/Call Back Rate: lx the normal hourly rate for a 15 hour period when a physician is required to be available by phone or pager for consultation or return to the facility. The weeknight period is generally 4:00p-7:00 am. Physician must be within 30 mile or 30 minute radius from the facility. Physician receives straight pay when physically onsite for all hours worked. Not applicable to Emergency medicine and Hospitalist. Workweek: Workweek is Sunday through Saturday
