Description: PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. RFP DUE DATE: Responses to this solicitation shall be via electronic submission to Frank Gamboa III at frank.gamboa@va.gov on or before March 24, 2011 at 2:00 PM EST. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number VA-258-11-RP-0164 is issued as a Request for Proposal (RFP). This solicitation and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-49. This solicitation will be 100% Set Aside for Small Business concerns and the associated North American Industry Classification System (NAICS) code for this requirement is 485991 with a size standard of $7.0 million. Scope of Contract: This solicitation will result in a short-term three (3) month firm-fixed price contract for non-emergent transportation services for beneficiaries of the Phoenix VA Health Care System. Prices awarded will remain in effect for the contract period without an increase in price. Period of Performance is April 1, 2011 through June 30, 2011.
