- This is an amended copy of RFQ #N00183-11-T-0130-0001 for the repairs of seven (7) Maquet, Inc. Operating Tables. Also, before you quote, you must show proof of your certification to service Maquet, Inc. Operating Tables. Please do not quote, if you are not certified by Maquet, Inc. If there are any questions, please contact Carolyn Johnson via phone (757-953-5740) or e-mail (adline.johnson@med.navy.mil). This is a conform copy of RFQ #N00183-11-T-0130 for the repairs of seven (7) Maquet, Inc. Operating Tables. Also, before you quote, you must show proof of your certification to service Maquet, Inc. Requirement to be awarded on an all or none basis. Technical review of vendor's quote will consist of SOW (your intent to conform to what is asked of you in the SOW), warranty, & price. Also, certification from Maquet, Inc. is needed to do work on products that are manufactured by them. Items on this RFQ are for Repairs of Operating Tables manufactured by Maquet, Inc. for Naval Medical Center-Portsmouth, VA. Quotes should reflect pricing on all items for consideration. Please provide government & destination pricing. Also, state warranty information for each CLIN number (Line Item). If freight charges apply, indicate if charges will be "per shipment" or "single one-time charge". Contractor shall supply a current published price list or GSA/VA Federal Supply Schedule contract for supplies &/or services being procured. Refrigerated, frozen, or hazardous material must be indicated as such & a copy of the MSDS (Material Safety Data Sheet) must be supplied for any hazardous material. Specify any special handling instructions. Solicitation closes on Friday, 25 March 2011. STATEMENT OF WORKS 1132.01BO 01693 Left Side and Right Side cover plates ("Pinch Plates") damaged from collision; need to be replaced Upper and Middle Column Casings (metal) damaged, need 2 of each Mechanical play in Leg and Trendelendburg axes; possibly just air in system after table out of use for so long; might be check valves. Need double check valve with pressure limiter for leg plates; regular double check valve for trendelenburg. Back plate not operating correctly during Level, Flex, and Reflex functions; need new potentiometer.
