SOURCES SOUGHT NOTICE: This is not a request for quote. Please do not send pricing information. Seeking sources that can provide all necessary labor, tools, equipment and materials for load testing, loaner lifts and factory repairs, at the Birmingham VAMC AL. See Statement of Work. This Source Sought is a 100% set-aside for Service Disable Veteran Owned Business only. This is a market survey for informational and planning purposes only. THIS IS NOT A SOLICITATION AND DOES NOT OBLIGATE THE GOVERNMENT TO ISSUE A SOLICITATION. Please be advised that the Government will not pay for any information submitted under this announcement. Your response should identify your company name, point of contact with contact information, indicate all areas you are capable of providing service, capabilities statement and identify your business size, and if you are (by the definitions in the FAR) any of the following: Small Business, 8(a) disadvantaged, disadvantaged, woman-owned, veteran-owned, service disabled veteran owned or HUB Zone certified. A Request for Quote shall be issued at a later date. The NAICS code for the procurement is339113 and the small business size standard is 500. Responses are requested no later than 1:00 PM EST 29 March 2011 to iridious.ruise@va.gov. STATEMENT OF WORK PATIENT LIFT SYSTEMS This service agreement shall provide preventive maintenance, load testing, loaner lifts, and factory repairs. Not included in this agreement is on site labor. All inspections and preventive maintenance will be performed by a local service representative. Description of Work As a result of the service contract, veterans at Birmingham VA Medical Center will receive care using the safest lifting equipment possible. Additionally, this contract will ensure that staff are performing their work assignments in a safe and injury free environment. To ensure that this has occurred to the satisfaction of the facility, the inspector will be required to provide a checklist outlining the actual service completed. This checklist will be provided to the Safe Patient Handling Coordinator for the facility. This contract will include the following: General Inspection and Preventive Maintenance for track lift products Below is a non-inclusive list of services contractor will provide during their visit(s): Visually inspection 2 times yearly (track products): "The lift lifting tape shows NO signs of fraying or breaking along its entire length. "The stitching on the lift lifting tape where it connects to the carry bar shows NO signs of fraying, or breaking. "The sling(s) that will be used shows NO signs of unusual wear and tear. The straps of the sling that connect to the carry bar of the lift show NO signs of fraying or breaking. Refer to specific sling instructions. "The airline tube that connects the hand control to the lift is not kinked, twisted, knotted, cut or damaged. "All the functions on the hand control work correctly (e.g. UP/DOWN/LEFT/RIGHT, etc.) "The brackets that hold the track in place on the ceiling are secure and do not move or appear loose. "There are not cuts, dents or sharp edges on the carry bar that may damage the straps of the sling. "The lift has no unusual sounds when the carry bar is moved UP/DOWN or the lift is moved LEFT/RIGHT. "Ensure that there are end stops installed at each end of the track. "The lift moves freely along the entire length of the track. "Certify and record the lift inspection for inspector records Preventative maintenance 1x yearly (track lifts): Diagnosis inspection; preventative maintenance if it hasn't had any preventative maintenance for: 1,000 lifts (four or five lifts a day 180 days); or, Five (5) hours. When recommend preventative maintenance due, the lift will: "Beep one (1) time every thirty (30) minutes "Flash "Maintenance" in the first line of the display (regardless of which default display mode the user has selected). "Record then reset the PM lifts counter: "Inspect complete lift operation as per Operators manual "Check and load test Batteries "Lubrication of moving parts as needed "Wipe clean inside track "Check strap limit switches "Certify and record of lift inspection for inspector records Load Testing to EN10535 standards 1x yearly (track lifts): "Tracks will be load tested to 1.25 times the safe working load; each track attachment will be point load tested. The deflection of the track will be measured and determined if it is acceptable to (EN10535) standards. All beds must be available within 3 continuous days. "Maximum load of the track lift per manufactures specifications "Inspect above ceiling attachments "Certify and record of lift inspection for inspector records Visually inspection 2 times yearly (floor based products): "The sling(s) that will be used shows NO signs of unusual wear and tear. "The lift has no unusual sounds when the carry bar is moved UP/DOWN "Inspection and cleaning of the casters and breaks of floor based products "Inspection of the structural integrity of the mast, lift arm and base "Inspection of the actuator for structural integrity wear and looseness at lift arm "Certify and record of lift inspection for inspector records Preventative maintenance 1x yearly (floor based products): "Inspect complete lift operation as per Operators manual "Check and load test Batteries "Lubrication of moving parts as needed "The lift has no unusual sounds when the carry bar is moved UP/DOWN "Inspection and cleaning of the casters and breaks of floor based products "Inspection of the structural integrity of the mast, lift arm and base "Inspection of the actuator for structural integrity wear and looseness at lift arm "Certify and record of lift inspection for inspector records "Maximum load of the lift per manufacturer's specifications The quantities are definite for this contract and will service 122 ceiling lifts and 21 floor lifts. "6 Waverly Glen STEDY Standing Aids "6 Waverly Glen F550 Floor Lifts "9 Waverly Glen S440 Standing Aids "1 Arjo 60191 Standing Aid "10 Arjo Maxisky600 Ceiling Lifts "108 Waverly Glen C625 Ceiling Lifts "4 Waverly Glen TA800 Ceiling Lifts All ceiling lifts will be installed using the following to ensure that they met the VHA Directive 2005-019 Seismic Safety of VHA Buildings and VA Master Design Specification 3.05.041 Seismic Restraint Requirements for Non-Structural Components. All tools used in service will be supplied by the technician. The following industry and VHA standards must be followed: VHA Directive 2005-019 Seismic Safety of VHA Buildings and VA Master Design Specification 3.05.041 Seismic Restraint Requirements for Non-Structural Components. The cost of the contract has been determined in advance and any additional cost to the facility will include but not limited to the replacement of batteries and shipping incurred if motors require repair at the Prism Medical Plant. The work will be performed in the building, specific floors and rooms where ceiling lifts are located. The typical response time for the service technical will be 48 hours. There will be loaner lifts on site to decrease down time and to ensure continuity of care. Annual Preventative Maintenance Checklist for Ceiling Mounted Patient Lifts 1.The annual preventative maintenance for patient ceiling lift system(s) shall include, but not be limited to, the following points as components of the inspection/maintenance procedures as outlined in the annual preventative maintenance checklist. 2.Proper personal protection equipment (PPE) shall be worn by staff during these commissioning procedures. 3.The manufacturer's representative responsible for annual PM and Load Testing: a.______________________________________________________ 4.The following personnel shall be notified when the annual preventative maintenance is completed: a.Clinical Representative: (SPH Coordinator): __________________________________ b.Safety Representative: _________________________________________________ Annual Preventative Maintenance Checklist for Ceiling Mounted Patient Lifts Date Completed: Facilities Engineering Maintenance RepLocation of lift: Manufacturer information, including model and serial number(s). EE Number: Model: Serial Number: Confirm operator and maintenance manuals available. Verification of proper connections of structural system to the building's structure. Verification of proper interface of lift unit at ceiling (hard deck or soft tile) and proper installation of all protective features around the support rods and rails/tracks. Annually inspect and tighten all track brackets, tracks and stops. Inspection of lift motor casing for cracks and alignment. Full extension and inspection of lift strap for loose threads or frays. Inspection of spreader bar and clips for cracks and for loose or missing rings or cotter pins. Verification that all rail end stops are in place and tightened. Inspection and activation of hand control for full operation (e.g., up, down, left, right) and "return to charge" function if applicable. Inspection and activation of emergency up/down motor case control buttons if applicable. Confirm any and all motor case indicator lights are functioning (e.g., red service warning light, charging state light). If included in installation, verify rail turntable function, exchanger function, and gate alignment. Confirm track is clean and clear of all debris (suggest wiping entire length of interior track channel with a soft cloth). Note: Use manufacturer's recommended cleaning materials to avoid damage to the motor case and other components. Phenol or chlorine solutions may damage some motor case surfaces. Verification of any "soft start" or "soft stop" features and that lifting speed does not exceed 2.5 inches per second with "zero" load. Verification of load testing and deflection testing at lift listed maximum for each lift unit at its maximum rated lift capacity. Conduct this test in three progressive stages starting with a 100 lbs load, then 50% of maximum rated lift capacity, then 100% of maximum rated lift capacity. Verification of any "soft start" and "soft stop" features and that lifting speed does not exceed 1.5 inches per second under maximum rated lift capacity. Verification of function of emergency brake at maximum rated lift capacity. Verification of emergency lowering feature at maximum rated lift capacity. Verification that charging functions are working Inspect trolley wheels, clean as needed Identify replacement date for battery (every two years) Date Required: Identify replacement date for main lifting strap Date Required: QUALITY ASSURANCE PERFORMANCE STANDARDS SUMMARY REQUIRED SERVICEPERFORMANCE STANDARDMONITORING METHODRESULTS FOR MEETING OR NOT MEETING THE PERFORMANCE STANDARDS Servicing and Preventative Maintenance of the following: "77 track lift systems "4 F-550 floor lifts The Contractor will utilize the Annual Preventative Maintenance Checklist for Ceiling Mounted Patient Lifts criteria(COTR will provide prior to work) for reliability of operation which includes but is not limited to: "Inspecting "Cleaning "Adjusting "Appraising "Testing100% Direct Observation Contract Officers Technical Representative (COTR). 5% of the total invoice amount shall be deducted when the performance standard is missed. Contractor shall submit a complete written report to the COTR within seven calendar days of the last site visit; however, deficiencies that are deemed catastrophic shall be immediately reported to the COTR for immediate action. Contractor QualificationsContractor shall submit proof of the following referenced certifications: "Occupational Safety and Health Administration (OSHA) "CE or equivalent100% Direct Observation COTR shall verify proof of documentation validity 5% of the total invoice amount shall be deducted when the performance standard is missed. Contractor's representatives shall or shall not be allowed to conduct maintenance, inspection or testing of the electrical power distribution system and accompanying apparatus. Performance of WorkThe Contractor shall be equipped with all the necessary tools, equipment and personal protective equipment to perform work safely, effectively and timely. In addition, perform work in accordance with the manufactures instructions not limited to, adjustment, calibrations, cleaning, and lubrication and testing as required to keep equipment operating efficiently. Periodic Inspections. 100% VA qualified personnel will inspect equipment on a monthly schedule to verify proper equipment operations.5% of the total invoice amount shall be deducted when the performance standard is missed. Documentation shall be made indicating that services rendered are effective or that services are substandard.
