- HIV, Hepatitis B & Hepatitis C Information Sheet Animal Exposure Surveillance Plan Offerors please propose prices based on the services requested. Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The NAICS Code for this procurement is 622110 and the Small Business Size Standard is $34.5 million. This solicitation number is RFQ1849064 and the solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50. The National Institute on Drug Abuse (NIDA) on behalf of the National Institute of Aging (NIA) intends to issue a purchase order for Occupational Medical Services with the inclusion of the administration of NIA IRP's Animal Surveillance Plan. The Contractor shall perform the following tasks (See Attached Statement of Work): (1) Occupational Medical Services; (2) Administration of NIA IRP's Animal Surveillance Plan. An indefinite delivery/ indefinite quantity (IDIQ) contract is anticipated to result from the award of this solicitation to the responsible and technically acceptable Offeror whose offer provides the greatest overall value to the Government. Quotations will be evaluated on the following criteria: (1) Technical Approach: Ability to meet technical qualifications. (2) Proposed personnel shall have experience treating occupationally acquired illness and injury. This would include expertise in treatment of animal-inflicted wounds and animal-inflicted allergies, knowledge of the signs and symptoms of suspected occupational diseases, including work-related infections and chemical contact allergies. (3) Past Performance: Experience with similar services in the past five years. (4) Price/Cost: Price will be evaluated to determine price reasonableness. Price reasonableness will be established by competition and determined primarily by comparison with other offers submitted. The proposed cost/prices will also be compared to the Independent Government Cost Estimate (IGCE). Offerors shall propose prices based upon the services requested as outlined in the Statement of Work (See spreadsheet attached). Contractor shall provide the NIA Project Officer, prior to award of this agreement, with: •Evidence of current malpractice insurance •Laboratory certification •X-ray facility and B-Reader certification •Credentials, certifications and licenses of the medical staff •Post HIV Exposure Surveillance Protocol •In addition, one copy of all renewed certifications, licenses and insurance shall be provided to the Project Officer within one week of receipt of same, for the duration of this agreement. Quotations are to be sent electronically to larochek@mail.nih.gov. The Period of Performance is 12 months with 3 additional 1-year option period(s). The following FAR provisions shall apply to this solicitation: 52.212-1, Instructions to Offerors--Commercial Items, 52.212-2, Evaluation--Commercial Items. Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items, with its offer. Clause at 52.212-4, Contract Terms and Condition Commercial Items, applies to this acquisition. Clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition.
