Attachment 1: Past Performance Survey Synopsis: REQUEST FOR QUALIFICATIONS (RFQ): Architectural A-E Services, Indefinite Delivery Indefinite Quantity Contracting Office Address: Department of the Air Force, Air Force Space Command, 341 CONS, 7015 Goddard Drive Bldg. 145, Malmstrom AFB, MT 59402-6863 Description: THIS IS NOT A REQUEST FOR PROPOSAL. Architect Engineer (Architectural primary discipline) services are required for the design of miscellaneous operations and maintenance projects and supporting services at Malmstrom AFB, Montana. The Government reserves the right to negotiate and award none, one, two, or three contracts for A-E services to those who respond to this notice. The open-end contracts will be issued for a period of one basic year and four one-year option periods each. Projects will be issued to the A-E firm by delivery orders against the awarded contract. The total amount for the contract issued during the basic year shall not exceed $995,000.00, nor shall the total amount for each of the four one-year option periods exceed $995,000.00, resulting in a cumulative total of not to exceed (NTE) $4,975,000.00 per contract. Individual delivery orders will not exceed $399,999.00. Delivery orders will be issued as the need arises during the course of the contract period. Design services will be in conjunction with minor remodeling, small facility design and maintenance projects, including preparation of drawings and specifications. Various types of A-E services such as investigation, inspection, reports, and cost estimates may also be required as relates to the project design. Projects will primarily require architectural discipline, with support from the mechanical/electrical, structural, and civil disciplines. A firm fixed price A-E contract (indefinite delivery/indefinite quantity) will be used. The estimated award date is between August 2011 and September 2011. Significant selection criteria, in relative order of importance include: (1) Professional qualifications necessary for satisfactory performance of required services; (2) Specialized experience and technical competence in the type of work required, including, where appropriate, experience in energy conservation, pollution prevention, waste reduction, and the use of recovered materials; (3) Capacity to accomplish the work in the required time; (4) Past performance on contracts with Government agencies and private industry will be evaluated using Attachment 1, Architect/Engineer Past Performance Questionnaire and Architect-Engineer Contract Administration Support System (ACASS); (5) Knowledge of and proximity to the locality of the project including Malmstrom Air Force Base; and (6) Dollar value of DOD awards for the last twelve months. Firms that have a lower dollar value of DOD awards during the past twelve months will be given a higher rating for criterion (6) than those having a higher dollar value of DOD awards. The above evaluation factors are listed in descending order of importance. Criterion (1) has 30 points possible, (2) has 20 points possible, (3) has 20 points possible, (4) has 20 points possible, (5) has 10 points possible, and criterion (6) has 5 points possible for a total of 105 points. A Pre-Selection Board will use the evaluation factors above to determine the firms that will be recommended to the Final Selection Board. The Final Selection Board will also use the evaluation factors to score the Pre- Selection Board recommended firms. The Final Selection Board will determine the order of recommending the preferred candidates to the contracting officer. A-E firms which meet the requirements described in this announcement are invited to submit two paper copies (1 original and 1 copy) of their completed Standard Form 330, "Architect-Engineer Qualifications," Parts I and II. Limit your SF 330 along with supporting data for a total package of not more than 100 pages, 8 ½ x 11 paper, and 12 pitch. Pages in excess of this maximum limit shall be removed from the package and will not be evaluated by the selection boards. Experiences identified in Block 19. RELEVANT PROJECTS shall not be more than 5 years old. All information relating to the firm(s) qualifications must be contained in the standard forms. Inclusion by reference to all other material is not acceptable. This is not a request for proposal; this synopsis is for the purpose of source selection. Firms responding to this announcement by the response date (25 Apr 2011, 2:00 PM MST) will be considered. Electronic copies will not be accepted, paper only.
