Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in Subpart 12.6 of the Federal Acquisition Regulation (FAR), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is N40442-11-T-7408, and it is being issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-48, effective 30 DEC 2010. NAICS code 333922 applies. This is a small business set-aside. Military Sealift Fleet Support Command (MSFSC), Norfolk, VA intends to award a sole source firm fixed price purchase order to Federal Equipment Company for USNS BYRD for the following services. Original equipment manufacturer (OEM) parts are required. Any offer for other than OEM parts will not be considered: The requested period of performance for the below service is 07-21 June 2011 1.0 ABSTRACT This item describes technical representative services for the Helicopter hanger doors on the USNS BYRD. 2.0 REFERENCES: 2.1 USNS BYRD helo Door Repair.PDF 3.0 ITEM LOCATION AND DESCRIPTION 3.1 Location/Quantity. 3.1.1 Location: USNS BYRD, Guam 3.1.2 POP : 07 June thru 21 June 2011 3.1.3 Quantity: Port Helo Hanger Door 3.2 Item Description/Manufacturer's Data: 3.2.1 None 4.0 GOVERNMENT FURNISHED EQUIPMENT/ MATERIALS/ SERVICES: None 5.0 NOTES 5.1 Port Engineer Contact is Willie Lombos, Mobile: 757-943-7949, 5.2 Provide an access list of all personnel boarding the ship. The list shall include the following: 5.2.1 Persons full Name 5.2.2 Social Security Number or naturalization number. 5.2.3 Passport Number 5.2.4 Place of birth 5.2.5 Date of Birth 5.2.6 High Voltage Awareness Trained? 5.2.7 The Access list shall be faxed to Nikia Rorie, at 757-443-1493. (Norma.Pilman@fe.navy.mil) The info needed are: 1. U. S. Citizen Contractors: Full Name; Full SSN; Ship Attending; Reason For Visit; Dates Requiring Access. 2. Non-U. S. Citizen Contractors: Full Name; Date Of Birth; Nationality; SSN, Passport No. or ARC No.; Ship Attending; Reason For Visit; Dates Requiring Access. 5.4 This ship contains High Voltage (HV) electrical systems. The contractor and subcontractors shall obey all posted and verbal instructions regarding safety and exclusion from High Voltage areas. At no time shall a contractor or subcontractor approach, work on, or enter a High Voltage area with out proper authorization. 5.5 THE USNS RICHARD BYRD ELECTRICAL COLOR CODING DOES NOT FOLLOW CONSISTENT COLOR SCHEME. THE SHIP IS KNOWN TO HAVE CURRENT CARRYING CONDUCTORS COLORED GREEN, BLUE, BLACK AND WHITE. THE CONTRACTOR SHALL ASSUME ALL ELECTRICAL CABLES ARE HOT UNTIL TESTED OTHERWISE. THE CONTRACTOR SHALL TEST ALL ELECTRICAL CABLES TO ENSURE THEY ARE DEAD PRIOR TO WORKING ON THE ITEM. 5.6 All shipboard lockout tag out procedures shall be followed by the contractor. 5.7 If Performance Location is in Vietnam, a Visa will need to be obtained prior to traveling to Vietnam. 6.0 QUALITY ASSURANCE REQUIREMENTS: None additional. 7.0 STATEMENT OF WORK: 7.1 Provide the service of, atleeast, one (1) Authorized/Qualified Federal Equipment Representative who is experienced with the Ship's Helicopter Hanger door. 7.2 Order the parts IAW Reference 2.1. 7.3 Ensure parts have been delivered to MSC BATS (San Diego) by Mid-May to ensure there is enough time to be sent to SSU Guam. 7.4 Repair IAW Reference 2.1 7.4.1 Replace the Port side Helicopter hanger door Spring barrell assembly and and electric motor operator / gearbox assembly with new parts. 7.4.2 Adjust all limits and perform full operational test. 7.5 Inspect the Starboard Hanger Door and the Starboard Helo doors are functional. 7.6 Provide a trip report that includes the "as-found" condition before the repair. 7.6.1 Ensure both the Chief Engineer and Port Engineer Recieves the report. 7.7 Preparation of Drawings: None additional. 8.0 GENERAL REQUIREMENTS: None additional.
