Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number, VA-260-11-RQ-0627, is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43 effective August 2, 2010 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to January 7, 2010. The North American Industry Classification System (NAICS) Code for this acquisition is 811219, Size Standard: $19.0 Million. This procurement is full and open competition. Full Service Parts and Maintenance agreement for Sterrad 100NX Scope Sterilizer System, S/N: 1041090424, located in the SPD Department Background. Biomedical Department does not have the necessary specialized tools and training to perform maintenance and repair on the Sterrad 100NX Scope Sterilizer System Scope. Contractor shall furnish all supervision, labor, travel, equipment, parts, and supplies to furnish preventative maintenance services and corrective / repair services for the Sterrad 100NX Scope Sterilizer System. The Sterrad 100NX Scope Sterilizer System is located at the Boise VA Medical Center (BVAMC) SPD, 500 Fort Street, Boise, ID. Service will be provided between 8:00am to 5:00pm, Monday through Friday, excluding holidays. The system does not contain patient identifiable information and is not connected to the VA's Computer Network and the Vendor does not have the ability to log into the system remotely. The Vendor shall comply with the requirements of VHA policies pertaining to sensitive information and sensitive systems. Specific Tasks. a)Contractor shall provide a copy of all Manufacturers specified scheduled maintenance as published in the Manufacturer's Service Manual to the BVAMC assigned COTR. b)Contractor shall perform all Manufacturer specified scheduled maintenance as published in the Manufacturer's Service Manual. c)Scheduled Maintenance shall be performed at the Manufacturer's recommended frequency as published in the Manufacturer's Service Manual during the annual contract period. d)Contractor shall furnish all supervision, labor, travel, equipment, parts, software support with upgrades, and supplies to accomplish the preventative maintenance services, and corrective call back services during the annual contract period. e)Contractor shall be escorted by a VA employee and all work will be observed and monitored while performing work at the BVAMC. f)Contractor shall adhere to all VA Policies pertaining to sensitive information and sensitive systems while performing work on the Sterrad 100NX Scope Sterilizer System. g)OEM parts shall be used for all maintenance and repair services. h)Contractor shall provide a copy of the Service Report, either by email or by fax, no later than 5 work days after the completion of the scheduled maintenance or repair service. i)The Service Report shall contain the following information: 1)Contract purchase order number 2)Date service initiated 3)Date completed 4)Equipment make and unique identification number/code 5)Description of original complaint 6)Problems found 7)Part/quantities installed 8)All work performed fully described Performance Monitoring: Performance will be monitored by periodic inspections and observation of work. Work will be observed by the COTR, or designee, during performance of scheduled maintenance service and inspections and/or during repair service. Equipment operators will be contacted to report on contractor performance when COTR, or designee, is not available. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). No Government supplied Equipment or Information is required or anticipated. Other Pertinent Information or Special Considerations. Only Manufacturer Trained Service Engineers/Technicians or Manufacturer Authorized Service Engineers/Technicians will perform services and repairs. Inspection and Acceptance Criteria. COTR, or designee, will verify that the unit is working within Manufacture's specifications after completion of service or repair. Risk Control. a)The Contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to any contractor representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the contractors fault or negligence. b)The Contractor will not discuss any patient issues that may have been observed during the course of the work. The Contractor shall be escorted by a VA employee and all work will be observed while performing work at the BVAMC. Contractor shall adhere to all VA Policies pertaining to sensitive information and sensitive systems while performing work on the Sterrad 100NX Scope Sterilizer System. Place of Performance. All service will be performed at the Boise VAMC, 500 W Fort Street, Boise, ID in the SPD Department. Period of Performance. (Dates may be adjusted slightly dependant on award date to reflect 12 month periods per year) Base Year:Award Date - March 15, 2012 Option Year One:March 14, 2012 - March 15, 2013 Option Year Two:March 14, 2013 - March 15, 2014 Option Year Three:March 14, 2014 - March 15, 2015 Option Year Four:March 14, 2015 - March 15, 2016 Delivery Schedule: Service Reports will be emailed or faxed to COTR by the 5th workday after completion of the work. Basis of Award: This contract will be awarded as a firm fixed price contract to the vendor that offers the best value to the government. Award will be made to the offeror representing the best value to the Government. Award will be based on Availability of OEM Factory Trained/authorized repair technicians, Past Performance and Price equally. Each quote must include current and relevant past performance references showing experience of similar scope and size. References must be within the last five (5) years to be considered current. Submit at least three (3) references, but not more than ten (10). Submit technician training certificates. Failure to submit all requested documentation will result in a non-responsive quote.
