Description
HIPAA Solicitation No.11-242-SOL-00026 CRIME CONTROL ACT FORM The Taos-Picuris Health Center has a requirement for Diabetes Case Management services for its health care facility located at Taos, New Mexico. The requirements are for an estimated 2080 hours per contract year. The contract will be awarded as a Blanket Purchase Agreement not to exceed twelve months. The Offeror must have a bachelor degree or license/certification in one of the following: Community Health Education, Nursing, Public Health Nutrition/Dietetics, or other relevant health or community education field; Certified Diabetes Educator, or Certified Health Education Specialist preferred. The Contractor shall have the ability to apply specialized knowledge related to the care of persons with type 2 diabetes in America Indian and Alaska Native communities; ability to independently assess patient's educational needs to develop individualized learning and behavior-change plans for diabetes self-management; knowledge of educational principles in adult learning theory, behavior change concepts, empowering models, motivational interviewing techniques, learning styles and teaching strategies; ability to independently adjust teaching methods for unique patients and families, paraprofessionals, professionals and special situations; ability to independently perform a clinical assessment, implement needed care and report/document findings; ability to communicate effectively, both verbally and in writing, with patients, family members, IHS and non-IHS health professionals, paraprofessionals, and other staff members; ability to use computer programs such as Microsoft Word, Microsoft Outlook, Internet information acquisition, and a clinical data system; understanding of the social and cultural context of the American Indian/Alaska Native communities served at the Taos-Picuris Service Unit; and ability to integrate this knowledge of the community into project activities. Offeror must be established providers under an LLC, Inc. or other allied medical organizations. Without these requirements the Government cannot award a contract to the successful offeror. All interested offerors must meet the Responsibility Factors. The Request for Quote (RFQ) No. 11-242-SOL-00026 will be posted at www.fbo.gov on April 05, 2011. The solicitation document comprise of all information including the estimated requirements, scope of work, and applicable attachments. The Offerors are required to submit, by US Postal Service or hand delivered, hardcopies of their proposals to the following address: Attention: Bradley Platero, Contract Specialist, Albuquerque Area Indian Health Service, Division of Contract and Grants Management, 5300 Homestead Road NE, Albuquerque, New Mexico 87110. The RFQ due date is close of business 4:30 pm MT on April 18, 2011. Late Quotes may be considered if they were mailed via USPS and post marked prior to the time and date of the due date, and/or it is the only Offer received.
