Description

THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY; THIS IS NOT A SOLICITATION. The Department of Veterans Affairs (DVA) requests information from parties and/or entities capable of providing Tele-Radiology support for Diagnostic Imaging Studies. Also required are on-site radiology services on an 'as-needed' basis for the Department of Veterans Affairs Medical Center Chillicothe, 17273 State Route 104, and Chillicothe, Ohio 45601-9718. 1.This notice is part of Government Market Research, a process for obtaining the latest information from industry with respect to their current and near-term abilities. All company proprietary information contained in a response shall be separately marked. Proprietary information contained in response to this request will be protected from unauthorized disclosure. The Government will not inadvertently restrict competition by the use proprietary information from any one select entity in order to establish requirement(s) and entity capability(s) for future systems acquisitions. 2.Requirement descriptions that consist of an Entity providing Teleradiology and On-Site/As-Needed Radiological Services for a period of One (1) Base Year and Four (4) -One (1) Year Option Years including: I.Provide preliminary reading interpretations including: a.Magnetic Resonance Imaging - {MRI/MRA} b.Computed Topography - {CT} c.Computed Topography - Stat d.Ultrasound {Doppler/Duplex} e.General Radiographic {Plain Film} f.On Site Coverage - {As Requested Only}; II.Present Capabilities / Qualifications Statements; III.Small business concerns as well as a.Service Disabled Veteran-Owned Small Business [SDVOSB]; and b.Veteran-Owned Small Business [VOSB]; should indicate by providing to the Contracting Officer IV.Provide Proof of Licensure by: a.State of Ohio Licensing Board V.Provide Proof of Professional Medical Liability Insurance {Malpractice} VI.Provide Industry Point-of-Contact {POC} for technical inquiries and/or contractual issues. VII.Must have capability to provide on-site coverage services within.5 hours of request, at start date/time of request without lapse in service until the end of the request period. 3.All interested respondents please address all items under number "2" in detail for this request and submit with: A.Name, Address, CAGE CODE and Telephone Number of the Entity. B.Size Status - Entities shall confirm their status in writing in being a U.S. A. - based company and state the company business size. The NAICS Code pertaining to this request is 621111. C.Provide Literature indicating product/service technical capabilities. D.Entities shall be registered in the Central Contractor Registry [CCR] 4.Solicitation Release Date is on or about Monday, April 4, 2011 with responses due back April 30, 2011. Responses should be submitted via Electronic Mail to Michele E. Wilson at Michele.Wilson@va.gov. All Responsive Offers will be considered by the agency. 1.One-on-One information gathering sessions with respondents are currently not contemplated. The Government is under no obligation to conduct these sessions. The Government does not commit to providing a response to any comment or questions. Any revision to this notice based on comments received remains solely at the Government's discretion. These initiatives may ultimately result in the Government developing and issuing a solicitation for respondents. The above information is for source sought only. 2.Faxed responses will not be accepted. 5.Responses to this notice will be accepted within five (5) business days of the date of publication of this notice, and shall be transmitted electronically to Michele.Wilson@va.gov. Entities should use up to - but no more than seven (7) number of 8.5" by 11" double-sided pages with 12 Times New Roman point font to include attachments in describing with detail the requested information using a Microsoft Office-compatible electronic format.
