Description
Quote Form Representations and Certifications Statement of Work This is a Request for Quotation (RFQ) for Grit Removal Services for Grit Removal, Building 34, at the US Coast Guard Shipyard in Baltimore, MD. The solicitation number for this requirement is HSCG40-11-Q-60773. In addition to this document which includes required certifications, provisions, and Coast Guard specific terms and conditions, there are three additional documents which should be reviewed as part of this RFQ: 1. Quotation Form (one page) - This form has specific information concerning the period of performance and quantity of grit. Offerors may use this form to submit their quotation. 2. Statement of Work for Non-Hazardous Grit Removal and Disposal (one page) - This document states specific requirements for the collection and the disposal of the grit. 3. Offeror Representations and Certifications (16 pages) - To be submitted with offer. The point of contact for this procurement is Sandra (Sharkie) M. Burgert, Contract Specialist, Contract & Procurement Division 3, Surface Forces Logistics Center, 410-762-6546. Quotations are due no later than 8:00 am, Friday, April 1, 2011. Quotes may emailed to sandra.m.burgert@uscg.mil; faxed quotes will not be accepted. Contractors are responsible for verifying receipt of their proposals. It is anticipated that the purchase order being issued as a result of this RFQ will be awarded no later than 4:00 pm, Tuesday, April 5, 2011. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50 (16 Mar 11). This procurement is a 100 percent small business set aside. Place of Performance: U.S. Coast Guard Shipyard, Building 34, 2401 Hawkins Point Road, Baltimore, MD 21226. ATTENTION: R. Stanley. Special note on time and period of performance: It is anticipated that work to be performed under the purchase order issued as a result of this RFQ will take place during dayshift hours, 7:00 am - 4:30 pm. The anticipated dates of performance are every other Wednesday, April 13, 2011 - May 25, 2011 (April 13, April 27, May 11, and May 25). Evaluation of Quotations - The evaluation and award procedures in FAR 13.106 - 2 apply. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Meeting the period of performance requirement, price and price related factors, as well as past performance will be use to evaluate offers. The ability for the offeror to meet the period of performance is more important that price and past performance. Past performance and price are of equal value. Award will be made on an all or none basis. The FAR clauses listed below and included in this paragraph apply to this solicitation. (d) Comptroller General Examination of Record The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. (2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months.
