PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number VA-258-11-RP-0183 is issued as a Request for Proposal (RFP). This solicitation and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-48. This solicitation will be 100% Set Aside for SDVOSB and the associated North American Industry Classification System (NAICS) code for this requirement is 561621 with a size standard of $12.5 Million. See Attachment 2 Schedule of Supplies - Services Doc, Offeror's are to use this attachment to submit pricing information. Scope of Contract: This solicitation will result in a firm-fixed price contract for Fire Alarm System Maintenance at the Phoenix VA Medical Center. Prices awarded will remain in effect for the contract period without an increase in price. Project start date shall be May 1st, 2011. See attachment 1 for Statement of Work. BEST VALUE PROCUREMENT: INSTRUCTIONS TO OFFERORS A contract award will be made based on the overall Best Value to the Government. Technical/Experience and Past Performance (all of equal value) when combined are more important than Price. Price/Cost will be considered in the overall evaluation of the offered services to ensure award is made to the Offeror presenting the best value to the Government. Award may be made to an Offer that is not the lowest price. 1.Your response will also include two (2) separate attachment named a.Schedule of Supplies and/or Services (Attachment 2) b.Technical Proposal (Attachment 7) c.Past Performance (Attachment 7) 2.The Technical Proposal should be in a format which clearly addresses the evaluation factors found on Attachment 7. Each response shall address each factor and clearly identify which factor is being addressed. 3.Offeror must also provide the Past Performance information described on Attachment 7. The Government will not seek out corrections to incomplete or insufficient information on behalf of Offeror's. Unverifiable information will not be considered in the past performance assessment. 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a)The Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1.Qualifications: a.Offers will be considered only from Offerors who are regularly established in the business called for and who in the judgment of the Contracting Officer are financially responsible and able to show evidence of their reliability, ability, experience, equipment, facilities and personnel directly employed or supervised by them to render prompt and satisfactory service in the volume required for all items under this contract. 2.Technical Evaluation Criteria: a.The technical portion of the proposal will be very important in the award of a possible contract and should, therefore, be as complete and specific as possible, as the evaluation team will rate technical proposals only on detailed information as required by the solicitation and as submitted by the Offeror. b.The merits of each proposal will be carefully evaluated in terms of the requirements and in relation to the criteria established below. The evaluation will take into consideration the Offeror's technical and administrative capabilities of the proposal in relation to the needs of the VA and the work to be accomplished. c.A detailed plan should be submitted describing the organization which will support the performance of the contract. The plan should include locations of the offices, staffing, and managerial plans. In summary, information should be provided which will demonstrate the Offeror's understanding of how they intend to manage tasks which may be called for under this contract. 3.Technical Proposal: The technical proposal is to include the following. Failure to address any one of the items listed will result in the rejection of the offer. Proposals will be evaluated on the basis of maintaining program continuity, capability, accessibility, and the availability of required competent professional staff. a.Experience and Training of Technical Service Personnel: i.Provide name(s) of technician(s) to be assigned to this contract. ii.Provide technician(s) education and/or training transcripts. iii. Provide three (3) references for which the technician(s) has/have performed similar duties as will be required under this contract. Include the company's name, address, telephone number and contact person. iv.Submit evidence that personnel are qualified and trained as authorized reps for Edwards Systems Technology and DuKane Corporation. b.Inspection, Testing and Maintenance Documentation: i.Submit proposal indicating how the Offeror's method of inspection, testing and maintenance is in accordance with NFPA 72 and NFAC latest edition. ii.Submit a quality control plan describing the Offeror's plan to ensure timely response to both normal and emergency hour services. iii.Submit evidence the Offeror is EST-3 and Firework Certified to work on the fire alarm system. iv.Submit a Quality Assurance Plan. This Quality Assurance Surveillance Plan (QASP) provides a standard of surveillance for monitoring the Fire Alarm System Inspection, Testing and Maintenance contract and provides the approach the Government will use to conduct surveillance over the performance aspects of the effort. The QASP is used by government Quality Assurance (QA) personnel to insure the standards of the contract are being met. 4.Past Performance: a.The proposal should describe the past experience of the company in providing services specified in this solicitation within the past three (3) years. List three (3) references that can validate the company's level of performance by providing company's name, address, telephone number and contact person. 5.Price including Option Years: a.Price will not be rated however it will be evaluated to ensure it is fair and reasonable. Technical and past performance, when combined, is more important than price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful Offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
