(End of clause) 5352.242-9000 CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (Aug 2007) (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or security police for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and Passport to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Volume 1; AFI 31-105 citing the appropriate paragraphs as applicable. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment. END ADDENDUM 52.212-4 END OF ADDENDUM TO 52.212-4 CLAUSES INCORPORATED BY FULL TEXT 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1. Technical Criteria: i. Must have a two-year degree in Business Administration. ii. Must have computer and desktop publishing skills, specifically Microsoft Office applications such as Word, Excel, Powerpoint, and Outlook. 2. Past Performance Criteria: i. Have one (1) year previous experience as a religious education coordinator - preferred but not required. ii. If the applicant does not have a degree, two years previous experience as a church/chapel programming coordinator is desired. ii. If the applicant has neither a degree nor the necessary experience as a religious education coordinator, then best experience and willingness to learn is considered for hiring. 3. Price (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (End of provision) 52.212-5 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS--COMMERCIAL ITEMS (DEC 2010) (DEVIATION) (a) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (a) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records-Negotiation. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. (2) The Contractor shall make available at its offices at all reasonable times, the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. (3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law. (b)(1) Notwithstanding the requirements of any other clause in this contract, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (b)(i) in a subcontract for commercial items. (End of clause) STATEMENT OF WORK CATHOLIC PASTORAL COORDINATOR Statement of Work (SOW) Appropriated Funds Current as of 28 Mar 11 Contract Position Title. Catholic Pastoral Coordinator (CPC). Scope of Responsibilities. To plan, coordinate, and implement the administration support for pastoral ministry within the Spangdahlem AB Catholic community. 3.0. Education and Experience Requirement. The Catholic Pastoral Coordinator must: 3.1. Have a two-year degree in Business Administration plus one (1) year previous experience as a church/chapel programming coordinator is preferred, but not required. If the applicant does not have a degree, two years previous experience as a church/chapel programming coordinator is desired. If the applicant has neither a degree nor the necessary experience as a coordinator, then best experienced and willingness to learn is considered for hiring. 3.2. Have a knowledge and experience in Catholic liturgical year, liturgy and ministries; good planning and coordinating skills; computer and desktop publishing skills are a must, specifically Microsoft Office application such as Word, Excel, Powerpoint and Outlook. 3.3. Have a minimum of one (1) year on station to be considered for this position. A copy of the sponsor's PCS order listing the applicant will be sufficient to meet this requirement. 3.4. Be able to read, communicate and write effectively in English in order to conduct administrative duties with clarity. 4.0. General Responsibilities. The Catholic Pastoral Coordinator will: 4.1. Be able to clearly communicate in English and conduct administrative requirements in verbal and written clarity as necessary. 4.2. Be able to maintain a professional working relationship with chapel staff, congregations, and other personnel to achieve duties as necessary. 4.3. The person must have a positive work attitude, be self-directed, able to interface well with chapel staff, congregations and other personnel to achieve duties, and have good time-management skills. 4.4. Be willing to work within a community of diverse faith expressions, supporting a religiously pluralistic environment. 4.5. Maintain a neat, clean and well-groomed professional appearance while conducting self in a professional manner. 4.6. Preserve any confidential issues seen or heard within the chapel staff. 4.7. Comply with all Air Force computer and information security requirements. 4.8. Be required to read and sign facility usage, security, key control and safety letter. Upon daily departure, the facility must be returned to a clean condition, properly secured and locked. 4.9. Submit an application for a background check to the chapel Security Manager according to DoD and Air Force Instructions. 5.0. Specific Responsibilities. The Catholic Pastoral Coordinator will: 5.1. Work primarily with the Catholic Chaplain, Catholic parish council and Catholic Religious Education coordinator. 5.2. Attend Catholic parish council, chapel staff, chapel financial working group meetings and other special meetings as necessary. 5.2. Maintain a 30-hour work week to include any meetings, training, office hours, and program attendance necessary to complete duties established in this SOW. 5.3. Serve as the central point of contact for all 52 FW/HC Catholic community related activities, to include preparing promotion/publicity as necessary. Publicize on a monthly and/or as needed basis the Catholic Community calendar and special programs/events through a variety of sources. 5.4. Maintain the Catholic database (subject to the Privacy Act) to include contact information, family members info, volunteer status, etc... 5.5. Coordinate and maintain Catholic calendar, funds and facility requests for Parish council ministry coordinators and maintain log. 5.6. Help implement the Catholic parish council plan, goals, objectives for Catholic liturgies and other activities. 5.7. Assist in recruiting and training volunteers for Catholic programs as needed. 5.8. Ensure chapel staff is kept abreast of on-going Catholic activities. 5.9. Ensure weekly Catholic bulletins and inserts are prepared, proofed and printed. 5.10. Coordinate Parish council training and meetings; ensure agenda is created and meeting minutes are taken. 5.11. Ensure an annual Catholic Volunteer Appreciation event takes place. 5.12. Perform administrative duties as required and serve as main focal point for the Catholic community during the absence of the Catholic chaplain. 5.13. Keep the assigned CPC office in a clean, orderly and safe manner. 5.14. Submit CTOF purchase request form, AF 4356 when ordering Catholic supplies and/or equipment. All order/purchases must be approved by the Catholic chaplain. 5.15. Manage all Catholic materials by ensuring they are properly stored in user friendly manner, clearly categorized and labeled. Establish and maintain computerized resource center database to adequately reflect all resources. 5.16. Coordinate Catholic mass (LEMs, Lectors, Altar servers, ushers) support for all the Catholic masses (currently 0900 & 1800 Sunday mass), Holy Day of Obligations, special services (Penance service/First Communion/Confirmation) and programs (Christmas play) held at Spangdahlem or other locations not yet specified. 5.17. Prepare and maintain sacramental records and for the Catholic chaplain and act as liaison to the Archdiocese for the Military Services, USA. 5.18. Coordinate with chapel security manager to ensure all volunteers working with youth below 18 years of age receive a background check. 5.19. Be accountable for paying any Social Security payments, Federal or Local Income Taxes, and State Unemployment Insurance premiums for themselves, as well as any individual employed or subcontracted by them to provide services under this contract. 6.0. The Wing Chaplain. 6.1. Provide office space with desk, chair, computer, printer, filing cabinets, bookcases phone, expendable office supplies and other items deemed necessary to perform the services described in this SOW. 6.2. Provide CTOF for at least one training event that relates specifically to CPC activities, when funding is available and cannot be provided by appropriated funds. 7.0. The Quality Control Monitor (QCM). The Quality Control Monitor for this contract is the NCOIC, Chapel Resources or NCOIC, Chapel Operations. The Quality Control Monitor or his/her designee will monitor and inspect the contractor's performance. The contractor will be notified in writing of all noncompliance work, and given an opportunity to correct nonconforming work. The contractor will be given an opportunity to respond in writing to any notice of noncompliance. The Quality Control Monitor will then forward the written notifications to the 52 CONS. 8.0. Criminal History Background Check. In accordance with USAFE Instruction 52-104 para 5, the Contractor must complete a Child Care National Agency Check and Inquiries (CNACI) and local Installation Records Check (IRC). Upon favorable completion of the IRC, the contractor may perform their duties pending the completion and favorable adjudication of a submitted CNACI, provided they are within line-of-sight-supervision of an individual with a favorably completed CNACI and IRC at all times. Contractors may not perform duties without supervision until favorable completion of both a CNACI and IRC. A negative CNACI results in immediate termination of contract. Working Hours and Extended Periods of Absence. The minimum work hours to be performed are thirty (30) hours weekly. The contractor will provide regular in-office customer service hours. The amount will be worked out with the Catholic Chaplain. The remaining hours to accomplish work as described in this SOW can utilize flextime, at times the contractor is required to work on weekends. The contractor needs to be aware that some months carry a significantly heavier workload than others. Contractor arrives fifteen (15)-minutes prior to the start of the event for which responsible. The contractor is required to give the chapel staff a two (2)-week notice of any extended period of absence. The contractor insures a suitable substitute with at least equal qualifications is provided in the case of any extended period of absence. Sponsoring chaplain must approve substitute. Security of Facility. The contractor is required to read and sign facility usage, security, key control, and safety letter. Upon departure, (in the situation where no other scheduled event(s) follows), the facility must be returned to a clean condition, properly secured and locked.
