Description
Presolicitation Notice This requirement from The Veteran's Affairs, VISN 20 Contracting Division, issued as Request for Quotation (RFQ), number VA-260-11-RQ-0665 with amendments, if any follow, is issued as a re-solicitation of VA-260-10-RQ-0279 and will be to provide Ambulance Services to its eligible beneficiaries requiring transportation to and from the VA Roseburg Health System (VARHS), Roseburg, Oregon and Community Based Outpatient Clinics located in Eugene, North Bend, and Brookings, Oregon and Crescent City, California. Services may include Basic Life Support (BLS), Advanced Life Support (ALS) or Specialty Care Transport (SCT) by ground to VA and non-VA facilities throughout Western Oregon, Northern California, and Western Washington. Contract requirements, quotation requirements and evaluation criteria will be covered in detail in the solicitation and Statement of Work. This requirement is for commercial services in accordance with the format in FAR Part 12, as supplemented with additional information as needed. All contractual and technical questions must be in writing (e-mail or fax). Only written requests received directly from the requester are acceptable (Telephone questions will not be accepted). Preference is that you send your correspondence through email at: dennis.riches@va.gov. If requesting by FAX, you may send your request 360-256-0936, ATTN: Dennis Riches. Potential offerors making a written request must include an adequate justification for not obtaining the solicitation via the internet. Telephone requests for diskettes or other inquiries will not be honored. The government intends to award one or more Firm-Fixed Price (FFP) Indefinite Quantities Contract(s). It is anticipated that the period of performance will be for one base year, plus four one-year option periods. Offers will be evaluated technically on Qualifications, Experience, and Capability for Geographic Coverage, and Past Performance, in addition to Price. Hard copies of the solicitation will not be available. The RFQ will only be available on the FedBizOpps website at http://www.fedbizopps.gov/. Accordingly, prospective contractors may view and/or download the solicitation and all amendments from the site. Prospective offerors are responsible for downloading the RFQ, all amendments thereto, and all attachments. It is the offerror's responsibility to daily monitor FedBizOpps for the release of the solicitation and any amendments that may follow. All quotes must include signed copies of all solicitation documents, including all amendments. Amendments, if/when issued will be posted to the EBS page http://www.fbo.gov/ for electronic downloading. This will normally be the only method of distributing amendments prior to closing; therefore, it is the offerors responsibility to check the website periodically for any amendments to the solicitation. Websites are occasionally inaccessible due to various reasons. The Government is not responsible for any loss of Internet connectivity or for an offerors inability to access the documents posted on the referenced web pages. Again, The Government will issue no paper copies. Contract award will be based on the best-value to the Government where technical merit and past performance, when combined, are significantly more important to cost or price. Offerors are required to provide separate documents/packages for the technical/past performance and the price portions of the quotations. The estimated award date is June 1, 2011. This procurement is being solicited on the basis of a "full and open competition", with preference for Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8a or Hubzone, and Small Business. Service Disabled Veteran Owned Small Business (SDVOSB) and Veteran Owned Small Business (VOSB) must be registered in the VetBiz Registry in order to be recognized as such. Visit www.vip.vetbiz.gov to complete your registration. Timely registration is strongly encouraged in order for the VA to certify the company's registration. All responsible sources may submit an offer which shall be considered by the agency. The North American Industry Code for this procurement is 621920 for (Ambulance Service), and the size standard is $7 million in revenues. To be eligible for award, offerors must be registered with the Central Contractor Registry (CCR) at www.ccr.gov or by contacting the CCR Information Center at 1-888-227-2423. Offerors shall identify the size of their business in accordance with this NAICS Code. Additionally, Representations and Certifications must be completed at http://orca.bpn.gov, or if offerors are not registered via the ORCA website, offerors must complete and return a copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with their offer. These representations and certifications will be incorporated by reference in any resultant contract. Quotation must include, Solicitation Number VA-260-11-RQ-0665, Taxpayer Identification Number (TIN), Dun & Bradstreet (DUNS) Number, and all solicitation and proposal documents (including amendments and official proposal documents) must be signed by an authorized company representative.
