Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91WAW-11-T-PN05 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. This procurement will be open competition NAICS: 713940, size standard $7.0m. Responses should be quoted in accordance with the Statement of Work, at Attachment 1. Performance period is for a base period of 12 months and one option year of 12 months as follows: CLIN 0001, 25 each, fitness memberships x 12 months = total amount. Anticipated date of award 1 May 2011 through 30 Apr 2012 CLIN 0002, 25 each, fitness memberships x 12 months = total amount. Anticipated date of award: 1 May 2012 through 30 Apr 2013 for option year 1. Attachment 1, Statement of Work: 1.0 DESCRIPTION OF SERVICES 1.1 Scope of Work: The contractor shall provide membership benefits to personnel assigned to the Special Inspector General Office for Iraq Reconstruction (SIGIR), Arlington, VA. Eligible participants are defined as civilian personnel who are assigned to SIGIR at 2011 Crystal Drive, Arlington VA 22202. The objective of this contract is to provide membership to a commercial fitness facility similar to the Pentagon Officer Athletic Center (POAC) located at the Pentagon or the facility located at the Crystal City Annex for approximately 25 active full fitness memberships for civilian personnel in accordance with this Statement of Work. 1.2 Period of Performance Base Year: 1 May 2011 - 31 Apr 2012 Option Year One: 1 May 2012 - 31 Apr 2013 1.3 Contractor Requirements The Fitness Center must have accommodations similar to the Crystal City Annex and the Pentagon's fitness centers. In order to support SIGIR's mission effectively, the contractor's fitness staff must be knowledgeable of preventative health maintenance and weight management. The contractor's facility must offer core fitness programs that contribute to cardiovascular fitness, strength conditioning, and flexibility, i.e. total fitness. As a minimum, the following fitness programs/equipment must be offered: state of the art cardiovascular equipment, strength and free weight conditioning equipment, aerobic, cycling, and walking programs. The facility offers a broad scope of group exercise classes that varies based on members' requests (i.e. Pilates, Yoga, Dance, Step, Kickboxing, Spinning, Flexibility, Core Strength, Muscle toning and Back/Abdominal strength classes). The fitness staff is certified by national personal training organizations. Members' amenities needs are met with steam rooms, saunas, workout and shower towels, shampoo, conditioner, body wash, lotion, shaving cream, deodorant, blow dryers and Q-tips. The facility shall have clean, well ventilated, operating showers and separate men/women locker rooms. The facility will have lockable and serviceable lockers for daily use. The commercial fitness facility shall be capable of providing an indoor room or space large enough to allow members to safely conduct calisthenics, aerobics, and use their treadmills, weights, and fitness equipment in a safe environment. The facility shall be located within a ten-minute walking distance from SIGIR 2011 Crystal Drive, Arlington, VA 22202 to allow members sufficient time to work out in accordance with Army guidelines and return to work with the least disruption to the duty day. 1.4 Deliverables 1.4.1. Full Membership Benefits in the fitness facility shall include: All standard commercial services offered by the fitness facility. Unlimited access allowed during normal operating hours. Hours of operation must include at a minimum: 5:00 am to 7:00 pm, Monday through Friday. Additional discounts normally extended to members. Provide personnel training plan and advice to members initially and as required. Membership does not include the use of health spas, massages, racquet clubs, and golf courses, bowling centers, sporting clubs, and swimming facilities unless included in a standard commercial membership. 1.4.2. Membership Cards: Membership cards issued by the contractor will be the same provided to commercial customers. Any replacement fee for lost cards is the responsibility of the participant. The membership cards will be issued to SIGIR personnel at no cost to the Government and must be presented to gain access. The Program Manager or Contracting Officer Representative for SIGIR will verify each individual's eligibility, grant authority to use this agreement, and will coordinate and control the memberships. In addition to the memberships, SIGIR will receive (5) complimentary daily guest passes per month controlled by the contract POC. Memberships shall belong to SIGIR. SIGIR will pay the agency's portion of memberships on a monthly basis. Individual co-payments will be the contractor's responsibility to collect. Neither SIGIR nor any other Government entity shall be liable to the gym for any portion of the individual co-payment now or in the future. 1.4.3. The contractor shall submit a monthly member's usage report NLT five (5) days from the end of each month to the SIGIR Program Manager. The agency pays only for those memberships in an active status. 2.0 GENERAL INFORMATION 2.1 Procedures 2.1.1 Memberships shall run from the first day of the month for a 12-month period. The members will register through their respective SIGIR Fitness Program Manager and the contractor's fitness staff. Each customer will review the membership rules and submit application through the contractor's fitness staff. The contractor will issue commercial membership cards to each member. 2.1.2 The contractor shall submit a monthly member's usage report NLT five days from the end of each month to the SIGIR Fitness Program Manager. 2.1.3 The fitness facility shall submit a monthly invoice to the SIGIR's Fitness Program Manager. Invoices for memberships must be identified in a monthly invoice with a listing of member's names or card numbers. The Government shall not be responsible for making a payment until the SIGIR Program Manager receives a completed list by name or card number with the invoice of services. The fitness facility must use (be on-line with) the Wide Area Workflow (WAWF) system for billing and payments. 2.1.4. In addition to the membership, SIGIR office will receive (5) complimentary daily guest passes per month controlled by the contract POC. 2.1.5 Additional memberships may be added to the list as required. The Government shall not be responsible for making a payment until the SIGIR Fitness Program Manager receives a completed list by name with the invoice of services. 2.1.6 The SIGIR Program Manager will manage membership cards and a list of active participants will be provided to the contractor. The SIGIR Fitness Program Manager will update the active participant listing quarterly. 2.1.7 Payments for civilian participants may be temporarily suspended due to overseas commitments. The contractor will be provided documentation to support the member's absence. 2.2 Security Requirements: Contractor shall report to an appropriate authority any information or circumstances of which they are aware of which may pose a threat to the security of DoD personnel.
