- RFP Non segregated facilities Davis Bacon Act REQUEST FOR PROPOSAL The Pinellas County Job Corps Center is soliciting bids for Center Physician. The Pinellas County Job Corps Center is a federally funded vocational and educational training program for youth. The program is open year round and the site is operated by ResCare, Inc. for the US Department of Labor. SCOPE OF WORK This subcontract is a non-personal professional dental hygienist service contract as defined in Federal Acquisition Regulation (FAR) Part 37.101. The purpose of the contract is to assist students in attaining and maintaining optimal oral health by providing basic on-center dental hygiene services to include promotion, prevention, assessment, intervention, treatment and follow up care. Pinellas County Job Corps Center is required by the US Department of Labor to provide basic dental hygiene services with the goal to increase student's awareness of their own personal responsibility in achieving and maintaining oral health and wellness. The Subcontractor shall provide professional dental hygiene services under this contract, at the Pinellas County Job Corps Center, St. Petersburg, Florida. This includes all labor, transportation of Subcontractor's personnel and incidentals (other than those provided by the Government as outlined below) necessary to successfully provide dental hygiene services. The dental hygiene provider performing work must be licensed/registered by the State of Florida. To be considered for award the subcontractor must submit, with their quote, proof of their current State of Florida license, registration, or certification. Proof of current licensure must be submitted during each option year. The subcontractor shall maintain insurance as outlined in 52.228-5 Insurance -Work on a Government Installation, 452.228-71 Insurance Coverage, and 52.237-7 Indemnification and Medical Liability Insurance (of at least $1 million). Proof of insurance will be required prior to Subcontract award and each time an option is exercised. The Subcontractor shall identify a qualified provider as a back-up to provide services in the event that the Subcontractor is unavailable. This secondary provider must be approved in advance, (qualifications should be at a minimum equivalent to those of the Subcontractor, unless a waiver has been given by the Department of Labor). Proof of licensure/registration and proof of liability insurance must be provided in advance and each time an option year is extended. The period of performance for this contract is July 1, 2011 through June 30, 2012, with three 12 month option periods, which may be exercised at the Purchasing Officer's discretion. All work shall be performed in accordance with the following Code of Federal Regulations (CFR) Policy and Requirement Handbook (PRH) Job Corps Health and Wellness Website Center Dentist Desk Reference The Pinellas County Job Corps Center Operating Policy and Procedure Manual Service Schedule - The Subcontractor shall provide dental hygiene services at the center, with established days of the week and working hours to be agreed upon by the Subcontractor and the Purchasing Officer's's Representative. Work will be performed between the hours of 8:00 am - 4:30 pm. The estimated number of hours to be provided shall be 12 hours per week; for the 48 weeks students are in residence at the Center, during the period of contract performance. Payment will be for the actual number of hours worked. Additional days or a change in work schedule must be agreed upon by the Subcontractor and the Purchasing Officer, with the Subcontractor requesting such approval 24 hours in advance for cancellation of services or changes thereof. The Purchasing Officer's Representative will provide the Subcontractor with the center calendar for planning purposes. The Subcontractor shall be responsible for cleaning and performing operator maintenance of the dental equipment. The Government shall be responsible for all maintenance of dental equipment designated to be furnished by the manufacturer, dealer or factory representative. ALL CONTRACT ACTIONS ARE SUBJECT TO THE APPROVAL OF THE UNITED STATES DEPARTMENT OF LABOR IN ACCORDANCE WITH FEDERAL REGULATORY PROCEDURES AND MANDATES OF THE FEDERAL REGISTER.
