Attachment B DOL Wage Determination Attachment A Bus Drawing Request for Quote QMAE-CR-10001-N c.1 statement of work C.1 GENERAL Service with CFM (Contractor Furnished Materials) GSA Fleet Florida Fleet Management Center (FMC) has a requirement for the following: C.1.1 Contractor shall provide for the refurbishment of 5 (five) Chevrolet Express 4500 bus units for the GSA Fleet Florida FMC. The refurbishment service shall include (1) Removal of specified existing items, (2) Installation of specified new items, and (3) Re-installation of specified salvageable items. Contractor Furnished Materials (CFM) as described in Section C.3.1 below will be required as part of the Installation. Contract Type C.1.2 Contractor shall provide services as herein described on a Firm-Fixed-Price basis. Location of performance C.1.3 The buses are located onsite at the FMC facility in Merritt Island, FL. 702 N. Courtenay Pkwy. Unit #106; Merrit Island, FL 32953.Contractor shall coordinate with the Florida FMC Fleet Service Representative to obtain access the bus units. Period of performance C.1.4 All five vehicles must be completed 45 DARO. (Days after receipt of order) C.2 REMOVAL OF SPECIFIED ITEMS C.2.1 Contractor shall remove the following items: •a. wheelchair lift •b. existing wheelchair anchor points •c. existing flooring and plywood C.2.2 Contractor shall later re-install items a, b, C.2.3 Contractor shall provide an appropriate container, vehicle, or trailer for transportation and disposal of item c. in accordance with all state and federal regulations. C.3 INSTALLATION OF SPECIFIED ITEMS C.3.1 The contractor shall install the following items: New plywood of equal or better quality as existing plywood. This plywood must be securely affixed to bus chassis. Cayman colored flooring of equal or better quality as the existing flooring. This flooring must be securely affixed to the plywood. 5 (five) double flip down seats Freedman Part number BV covered in commercial gray vinyl. These seats must be anchored in a perimeter location per bus drawing (Attachment A) C.4. INSPECTION OF FINISHED WORK C.4.1 An individual authorized by GSA Fleet will inspect the vehicle to ensure contract conformance. C.4.2 Labor and workmanship will be done professionally and under warranty. C.4.3 All work must meet DOT compliance and Federal and State vehicle standards.
