Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE; THE VA IS NOT SEEKING PRICING OR OFFERS. The Department of Veterans Affairs, VISN 20, in support of VA Puget Sound Health Care System, is performing market research to compile data regarding: (1) The availability and capability of qualified small business concerns; (2) Identify if there are sufficient number of qualified Service-Disabled Veteran-Owned Small Business, Veteran Owned Small Business, HUB Zone 8(a), Women-Owned Small Business, or Small Disadvantaged Business, Small Business concerns relative to the subject North American Industry Classification System (NAICS) code. Responses to the information requested will assist the Government in determining the appropriate procurement method, including any set-aside decision. Respondents should clearly describe their capabilities to include management experience, equipment and personnel or the ability to obtain them. Furthermore, interested sources must also meet all requirements of Federal, State, and Local Municipal codes regarding operations of this type service. The NAICS code assigned is: 562211 Regulated Medical Waste Removal and the small business size standard is $12.5M. Not responding to this Sources Sought does not preclude participation in any future or potential solicitation, or Request for Quotation. It is the intent of VA Puget Sound Health Care System to use the information gathered for market research purposes only. If a formal solicitation is released, it will be posted via the Federal Business Opportunity (http://www.fbo.gov) DESCRIPTION OF THE REQUIREMENT: The Contractor shall furnish all personnel, equipment, insurance, proper licensing, tools, materials, travel, transportation, and supervision necessary to provide Regulated Medical Waste Removal at the VA Medical Center, Puget Sound Health Care Systems, 1660 S. Columbian Way, Seattle, Washington, 98108, VA Medical Center American Lake, 9600 Veterans Drive, Tacoma, Washington, 98493 and Mount Vernon Community Based Outpatient Clinic (CBOC) 307 S. 13th Street, Mount Vernon, WA 98273. The contractor shall have and maintain the appropriate state and Federal license(s) required to perform in accordance with (IAW) the attached Statement of Work. The projected performance period consists of: One year base period (Anticipated start date is 1 June 2011 to 31 May 2012) with up to Four (4) option years. These dates may be adjusted based upon actual date of award. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1. Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size i.e. SDVOSB, VOSB, 8A, etc., and DUNS Number. Proof of Central Contractor Registration and Vet Biz registry (if applicable) is highly desirable. Identify all Joint Ventures and other Teaming arrangements. 2. A brief capability statement demonstrating past performance with relevant contract services in Prime Contractor role. 3. Disclaimer: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. After review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities (http://www.fbo.gov). 4. Submit Capability Statements to: Department of Veterans Affairs, VISN 20 Attn: Ross Byrne, no later than 12:00 PM, 15 Apr 2011. Responses must be submitted via e-mail to: Ross.Byrne@va.gov Note: Do not include Proprietary, classified, confidential, or sensitive information in responses. B.3 STATEMENT OF WORK-Draft 1. SCOPE: Contractor shall furnish all personnel, equipment, insurance, proper licensing, tools, materials, travel, transportation, and supervision necessary to provide Regulated Medical Waste Removal at the VA Medical Center, Puget Sound Health Care Systems, 1660 S. Columbian Way, Seattle, Washington, 98108, VA Medical Center American Lake, 9600 Veterans Drive, Tacoma, Washington, 98493 and Mount Vernon Community Based Outpatient Clinic (CBOC) 307 S. 13th Street, Mount Vernon, WA 98273. The companies providing capability statements to this RFI shall be in compliance for all duties entailing regulated medical waste removal from cradle to grave. 2. BACKGROUND INFORMATION: The Department of Veterans Affairs, Puget Sound Healthcare System (hereinafter called the VA) provides services to veterans of the Uniformed Services in and around the Seattle and Tacoma, Washington area. The Contractor shall provide Regulated Medical Waste Removal Service on the Seattle, American Lake, and Mount Vernon Campuses. 3. PERSONNEL: Personnel performing duties pursuant to this requirement shall be properly licensed to operate the Machinery, Equipment, and Vehicles necessary to fulfill the requirement. The employees shall wear proper attire, such as a uniform with company name and logo. 4. SAFETY: The Contractor shall obtain all necessary licenses and/or permits required to perform this work. The successful contractor shall comply with all Federal, state, county, and city laws, regulations, and governing bodies. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to any Contractor's representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the Contractor's fault or negligence. 5. PHYSICAL SECURITY: The Contractor shall safeguard all Government property in the work area. At the end of each delivery or pickup, facilities and equipment shall be restored to their original condition. Each facility site may have multiple collection and supply locations. The COTR shall determine the appropriate delivery of supplies and collection of regulated medical waste. 6. QUALITY CONTROL: The Contractor shall establish and maintain a complete Quality Control Plan to ensure that the requirements of the contract are provided as specified. One copy of the contractor's Quality Control Plan shall be provided to the contracting officer not later than the pre-performance conference. An updated copy must be provided to the contracting officer on the contract start date and as changes occur. a.The plan shall describe the methods for identifying and preventing defects before the level of performance becomes unacceptable. b.Upon request, the contractor shall furnish a copy of the latest OSHA, Labor of Industries, Washington State Utilities and Transportation Commission, Public Health Department, and any other governing body training records. 7. QUALITY ASSURANCE: The Government shall evaluate the contractor's performance under this contract using the method of surveillance. All surveillance observations shall be recorded by the Government. When an observation indicates defective performance, the COTR shall obtain the contractor's representative's initials on the record of the observation. a. Performance Evaluation Meetings: The contract manager may be required to meet periodically with the COTR and contracting officer at the beginning of contract performance. Meetings shall be scheduled as needed. The contractor may request meetings whenever a Contract Discrepancy Report is issued. The written minutes of these meetings shall be signed by the contractor's manager, contracting officer and COTR. If the contractor does not concur with the minutes (s)he shall state any areas of non-concurrence within 5 business days of receipt of the signed minutes. 8. ADMINISTRATIVE ISSUES: a. Payment shall be made in arrears of the specified billing period after receipt of a properly prepared invoice, provided all required work, including delivery of supplies, has been performed in accordance with contract stipulations, and can be verified with service reports signed by VA Personnel. b.Contracted services are to be accomplished subject to emergency situations-as defined by VA-which may require alteration of schedules. In the event that the VA declares a medical facility emergency or disaster, or participates in a federal and/or DOD drill, the contractor shall perform services as required by the medical facility disaster plan. c.Parking is available for Contractor vehicles on the VA grounds. It is the responsibility of the Contractor to park in the appropriate designated parking areas and to observe all traffic regulations while on VA facility. The VA shall not invalidate or make reimbursement for parking or traffic violations of the Contractor under any conditions. Information on parking is available from the VA Police Section (Seattle campus Ext. 6-3113, American Lake campus Ext. 7-6757). d.Contractor shall specify a point of contact (with phone number ) as the designated contact for VA, and the parties shall meet as needed. The point of contact can be reached during normal business hours and off-duty hours, and be able to respond within one hour of notification by the VA and take prompt action on matters pertaining to the contract. 9. HOURS OF OPERATION: Normal Hours: Monday thru Friday, Excluding Federal Holidays, 8:00 am to 4:30 pm. The following is a list of Federal Holidays that may affect the contractor's ability to access the Government facilities: New Years Day Martin Luther King, Jr. Holiday Presidents Day Memorial Day Independence Day Labor Day Discoverers Day Veterans Day Thanksgiving Christmas 10. EMERGENCY SERVICES: In the event of a natural disaster, flood, matter of national security, alert, etc., the Contractor shall provide emergency pickup within 24 hours of the emergency call. 11. STANDARD DEFINITIONS: a.Contract Discrepancy Report. A written report provided to the Contracting Officer when the Contracting Officer's Technical Representative (COTR), discovers that there is a discrepancy between the contractor's performance and the actual contract. b.Contracting Officer (CO). A person duly appointed with the authority to enter into and administer contracts on behalf of the U.S. Government. c.Contracting Officer's Technical Representative (COTR). An individual designated by the contracting officer to act as his representative to assist in administering a contract. The source and authority for a COTR are contained in the written letter of designation. d.Deduct Schedule. Table utilized to indicate the amount that the Government shall deduct from monthly invoices for non/poor performance. e.Government Property. All property owned or leased to the government or acquired by the government under the terms of the contract. Government property includes both government furnished property and contractor acquired property as defined in FAR 45.101. f.Quality assurance (QA). Those actions taken by the government to assure services meet the requirements of the Statement of Work (SOW) and all other service outputs. g.Quality Assurance Surveillance Plan (QASP). An organized written document used for the quality assurance surveillance. The document contains specific methods to perform surveillance of the contractor. h.Quality Control. Those actions taken by a contractor to control the performance of services so that they meet the requirements of the SOW. i.VA. Veterans Administration 12. SPECIFIC PERFORMANCE REQUIREMENTS: Calls for Service: a. Contractor shall respond only to calls for service placed by the following VA personnel: Contracting Officer Contracting Officer's Technical Representative (COTR) Chief or Assistant Chief, Facility Management Service Administrative Officer of the Day (AOD) Utility Systems Operator Maintenance & Operations Supervisor (M&O) b. A list of names shall be provided to the Contractor after award. Included Work: a. The Contractor shall provide the required containers (receptacles) to be utilized to discard the waste materials, as well as boxes, bags, and supplies. b. The Waste shall be placed into a receptacle at the facility to be picked up no later than 7 calendar days from the date of the previous/last pickup. In the event of a holiday, natural disaster, or any event that causes a delay in schedule, the receptacle shall be picked up no later than 24 hours after such event occurs. 13. SECURITY REQUIREMENTS All contractor employees are subject to the same level of investigation as VA employees. The level of background investigation commensurate with the level of access needed to perform the statement of work is: NACI. The Contractor is required to obtain a favorable Special Agreement Check (SAC) adjudication and a National Agency Check with Written Inquiries (NACI). This requirement is applicable to all subcontractor personnel requiring the same access. The contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM) through the VA, the contractor shall reimburse the VA within 30 days. Contractor Personnel. Failure to comply with the following contractor personnel security requirements may result in termination of the contract for default.
