- "This request for sources sought is for informational and market research purposes only. All information and action provided is completely on a voluntary basis only. Nothing stated in this sources sought by any Government Official shall be treated as direction or request to the vendor for action. All direction or requests will come from the Contracting Officer; as only they have the legal authority to bind the U.S. Government." STATEMENT OF WORK FOR Technical Inspection & Annual Maintenance Inspection on CAT 3516 Engines / Generators SCOPE: The contractor shall furnish all labor, transportation, materials and tools necessary to perform the Annual Maintenance Inspection, and the Technical Inspection on all seven (7) CAT 3516 Engines and Generators at the James A. Haley Veterans Hospital (JAHVH), 13000 Bruce B. Downs Blvd., Tampa Florida in accordance with the paragraphs below. QUALIFICATIONS OF INSPECTION PERSONNEL 1.The contractor shall provide an Certified inspector / technician with a minimum of 5 years field inspection experience on the CAT 3516 Engines and Generators to perform Technical Inspection and Annual Maintenance Inspection 2.The following line items are a minimum required to maintain the 5 year warranty on the CAT 3516 Engines and Generators a-Performing the 52 point Technical Analysis from form #565E (will provide on request) b-Chemically test engine coolant c-Take a sample to have the Laboratory analyze d-Temperature verification, generator and radiator e-Change engine oil filter(s), Change fuel filter(s) f-Drain engine crankcase oil and refill to proper capacity g-Test run of engine to ensure no leaks, will prime fuel system if necessary h-Dispose of used oil and filter(s) adhering to EPA regulations i-Provide service report #565, this will advise of any problems noted with unit, and differentiate whether it is covered by the warranty. j-We will advise and secure your authorization before proceeding with repairs 3.The inspector must be proficient in all NFPA, JACHO, OSHA, HIPPA, UL and VA codes and policies associated with the James A. Haley VA Hospital. SERVICE TO BE PROVIDED 1.To perform Technical Inspection and Annual Maintenance Inspection on the CAT 3516 Engines and Generators at building 54 (Power Plant) at James A. Haley VA Hospital 2.Compile a report of findings on the form #565E. This report shall be compiled by a Certified inspector / technician with a minimum of 5 years field inspection experience 3.The contractor will be responsible for creating and maintaining an Annual inspection schedule per manufactures specifications. SCHEDULE OF WORK 1.The inspections will be performed during normal VA working hours, Monday through Friday from 7:00am to 4:00pm, excluding Federal Holidays 2.Work may be required outside normal working hours for hospital and patient reasons. 3.When the James A. Haley VA Hospital deems necessary, work may be delayed, canceled or rescheduled for any time 24/7 SPECIAL INSTRUCTIONS 1.Contractor Check In: The contractor will report to the Lead Power Plant Operator prior to performance of any work. 2.Documentation: At the conclusion of the inspections and or visit, the contractor shall provide the report to the Lead Power Plant Operator within 5 business days. 3.Inspection of Equipment: Prior to opening of any (13.2) KV equipment, the inspector shall make sure that the Power Plant Operator is notified of the work being done and that the equipment is removed from service and locked out. 4.Smoking: Smoking within the Healthcare System Facilities is not permitted and is only permitted at designated areas. 5.Before removal of any panel cover resulting in the exposure of "LIVE PARTS", a hot permit must be applied for and granted by the James A. Haley VA Hospital. 6.The James A. Haley VA Hospital will only direct the contractor to the equipment to be inspected; all other assistance will be provided by the contractor. (2) Specific Instructions Regarding Technical Proposal Preparation: a.To provide a basis for sound evaluation by the Government, the purposed Contractor must submit complete technical proposal. The information provided shall be concise, factual, and complete. Contractors should be established in the business and in the judgment of the Government deemed financially responsible and able to show evidence of experience. b.The technical qualifications must include: 1.Technical Capability/Qualifications- "Company background "Certification of Compliance for CAT 3516 Engine & Generators "Copy of all current licenses/certifications for CAT 3516 Engine & Generators and associate equipment "Evidence of Insurance Liability "Evidence that your company has the specialize equipment and personnel required for conducting this type of services on CAT 3516 Engine & Generators and associate equipment 2.Past Performance "Examples of similar works performed (photos preferred) "Three work related references with telephone numbers and address to verify them "Letters of recommendations Recurring Federal holidays currently established are: A. New Year's Day - January 1st B. Martin Luther King Day - the 3rd Monday in January C. President's Day- the 3rd Monday in February D. Memorial Day- the last Monday in May E. Independence Day- July 4th F. Labor Day- the first Monday in September G. Columbus Day- the second Monday in October H. Veteran's Day- November 11th I. Thanksgiving Day- the fourth Thursday in November J. Christmas Day-December 25th (3)
