Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format under Simplified Acquisitions Procedures at Federal Acquisition Regulation (FAR) Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items; as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Quoters are responsible for monitoring this site for the release of any amendments or other pertinent information. This solicitation, 1D33N01064, is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-51. This acquisition is a Total Small Business Set-Aside. This is anticipated to be a firm-fixed price type purchase order. The following is the Statement of Work (SOW): 1.0SCOPE OF WORK: The Contractor shall provide all materials, equipment, and labor, and other services to rehabilitate a 100,000 gallon elevated water storage tank at Rocky Ridge Boarding School in Kykotsmovi, Arizona. The Contractor shall comply with all applicable UBC, NEC, UPC, NEPA, NESHAP, EPA and all other relevant codes and Government Standards to meet the terms of the purchase order. 2.0WORK DESCRIPTION: a.Work to be accomplished includes furnishing and application of protective coatings and paints to interior and exterior surfaces, and disinfection of interior surfaces, including surface preparation and other work necessary to accomplish the approved end result of a totally protected and usable tank and structure, including all attachments, accessories, exposed piping and appurtenances. b.Work to be accomplished includes the supply and installation of all specified safety, sanitary, security and structural upgrades including surface preparation and coating application in compliance with the requirements specified herein. c.The potential presence of heavy metals in the existing coating system and the inevitable disturbance of these coatings will require the Contractor's strict compliance with OSHA 29CFR 1926.62 worker protection regulations. It is the responsibility of the Contractor to verify the presence of heavy metals. The potential health concerns will require 100% containment of all generated debris in accordance with SSPC Guide 6.The disturbance of existing coatings as a result of exterior spot repair shall be performed using HEPA filtered vacuum attached power tools. On-site handling and temporary storage of hazardous debris shall be performed in accordance with SSPC Guide 7. 3.0 TASKS: The Contractor shall perform the following tasks: Task 1: Interior Coating: a.Perform all proper and necessary confined space procedures per OSHA requirements. b.Remove all sludge and debris by means of existing outlet piping and man ways. c.Perform surface preparation on the entire interior of the existing vessel in accordance with SSPC Sp-10 Near White Metal utilizing dry abrasive blasting methods. d.Apply interior NSF approved multi-coat epoxy coatings in accordance with AWWA D. 102-06 on all previously properly prepared surfaces. All welds/irregular surfaces shall be stripe coated prior to final coat application. e.Interior coatings shall consist of a two component epoxy system, with immersion resistance and NSF-61 approval. The following coatings are considered to be an approved epoxy coating:oDevoe Bar Rust 233V Epoxy; andoTnemec Pota-Pox L140F Epoxy. f.Perform Holiday Detection on all surfaces in accordance with NACE RPO 188. All identified holiday surfaces shall be properly prepared and patched in accordance with the paint manufacturer's recommendations. g.Properly cure and disinfect the vessel interior in accordance with AWWA C.652 "Spray Method #2". h.Replace man way gaskets as necessary and make vessel ready for service. Task 2: Mechanical Upgrades: a.Install new ladder gate assembly located at the access point to the exterior leg ladder. Install new carbon steel ladder assemblies in accordance with AWWA D.100-05 and in accordance with OSHA 29 CFR 1910. b.Remove and dispose of existing liquid level indicator cables. Install new cables for the target system and float. Cables to be of identical size and type and accordance with AWWA D.100-05 requirements. All hardware attachment shall be new. c.Remove and dispose of existing roof rigging hole cover plates. The eight (8) roof perforations shall be installed with a pipe seal welded to the roof extending a minimum of 4" from the roof surface. The pipe will terminate with a threaded end which will be capped. All connections shall comply with the recommendations detailed in the AWWA D.100-05. The penetration of the pipe through the roof and the attachment of the pipe to the roof shall comply with AWWA design criteria. d.Install a new riser gate located at the interface point between the tank and the top of riser complying with AWWA D.100-05 Section 5.1.1 and OSHA 29 CFR 1910 requirements. e.Install new locking assembly and gaskets on roof hatches. Replace with an aluminum roof hatch that complies with AWWA D.100-05 Section 5.4.3.1. The new tamper proof lock assemblies and shall be isolated from the carbon steel roof opening through the use of gasket material. The hinge assembly can be either carbon steel or aluminum and must be isolated from the adjacent dissimilar metals. A fastening device must be fabricated and installed to secure the hatch in the open position to avoid inadvertent closing. Task 3: Installation of Signs: a.Entry signs shall be securely mounted to the tank and riser directly adjacent to each entry point. These signs shall identify the interior of the tank and riser as a confined space and that OSHA 29 CFR 1910 compliant entry procedures must be employed prior to entry and during entry. b.Parameter safety and warning signs shall be installed on exterior fence. Signs shall be securely mounted to the parameter fencing at 4'-5' above grade. Signs shall be mounted on each gate or access point. All signage must comply with bio-terrorism risk criteria referenced in House Resolution 3448. Task 4: Warranty Inspection: Warranty inspection shall be conducted on the eleventh month (July 2012) of the twelve month warranty following completion of all coating and painting work using the following provisions and in accordance with AWWA D.102 Section 9 recommendations: a.Notification: The COTR shall establish the date for the Inspection and shall notify the Contractor at least 30 days in advance. b.Inspection: The entire interior coating system shall be visually inspected by the COTR (or his designee) and the Contractor with the tank drained. Expenses related to the draining of the tank shall be the responsibility of the tank Owner. c.Remedial Work: Any location where coating or paint has peeled, bubbled, or cracked and any location where rusting is evident shall be considered to be a failure of the system. The Contractor shall make repairs at all points where failures are observed by removing the deteriorated coating or paint, cleaning the surface, and recoating or repainting with the system. If the area of failure exceeds 25% of a specific coated or painted surface, the entire coating or paint system may be required to be removed and recoated or repainted in accordance with the original specification and AWWA D.102-06 recommendations. d.Costs: All costs for the Contractor's warrantee inspection and all costs for repairs required as a result of this inspection shall be borne by the Contractor. Re-disinfection of the tank upon completion of the repair procedures is the responsibility of the Contractor. 4.0PLACE OF PERFORMANCE: The place of performance is Rocky Ridge Boarding School in Kykotsmovi, Arizona. 5.0PERIOD OF PERFORMANCE: The period of performance shall be June 27, 2011 through August 5, 2011. The project shall take place after summer school is completed and/or when the school principal gives the approval to begin the project. 6.0ISSUES AND CONCERNS: The Contractor shall coordinate and facilitate with the Contracting Officer's Technical Representative (COTR) to work out any concerns or issue that may arise during the project. The COTR for this project is Mr. LeRoy Begay, Western Navajo Agency Facility Management. 7.0DELIVERABLES: The Contractor shall provide a daily progress update to the COTR until project completion. A substantial and final walk thru shall be conducted when project is nearing completion and when appropriate. 8.0WORK SCHEDULE: The Contractor shall facilitate with the COTR or the school principal if work is planned for the weekend. An approval must be in place for work to be done during weekends. 9.0 DEBRIS DISPOSAL: The Contractor shall remove all debris generated by or caused by the work in this purchase order. The Contractor bears all responsibility for all associated or related costs with the removal and disposal of debris from properties of the United States Government. The Contractor is required to comply with applicable Environmental Protection Agency (EPA), Federal, State, and local Government laws, policies and regulations while engaged in the removal and disposal debris. The Contractor shall transport the debris off the boundaries of the Navajo Reservation to an approved EPA landfill. The Government is not responsible in any way, financially or otherwise, for the success or failure of arrangements for disposal of debris between the Contractor and other parties. 10.0INSURANCE REQUIREMENTS: The Contractor shall assume full responsibility for accidents to himself and his employees. 11.0SITE VISITATION: A site visit will be conducted at Rocky Ridge Boarding School at 10:00 a.m. (MST-Arizona) on Wednesday, April 20, 2011. All prospective Quoters are urged to attend this site visit and are requested to contact LeRoy Begay,Western Navajo Agency Facility Office, Ph: 928-283-2303 no later than 3:00 p.m (MST-Arizona) on Monday, April 18, 2011 if they plan to attend. All responsible sources may submit a quote, which shall be considered. 12.0NAVAJO NATION & STATE TAXES The Contractor is responsible and required to pay Navajo Nation Tribal Tax, which is equal to 6 percent of their total quote amount. In addition, the Contractor shall also pay local and state gross receipt taxes. 13.0 INSTRUCTION TO QUOTERS AND EVALUATION CRITERIA: All Quoters shall submit a quote consisting of a separate technical and price volume. The volumes shall be as follows: Volume One: Technical Quote and Volume Two: Price Quote. Each volume shall be separate and complete in itself so that evaluation of one may be accomplished independently of evaluation of the other. a.Technical Quote The technical quote shall contain the following: 1.The Contractor shall describe its technical approach to accomplish the tasks delineated in the Statement of Work; and2.The Contractor shall demonstrate that current and proposed staff has have experience completing the same or similar services in this statement of work. b.Price Quote The Quoter shall prepare a firm fixed price quote that contains all information necessary to allow for a comprehensive evaluation of the costs proposed. The price quote shall include unit and total costs for the following: 0001: Labor = $_____/hour x ____ hour= $_________ 0002: Materials = $_________ 0003: Equipment = $________ 0004: Other services for Restoration = $_________ TOTAL: CLIN 0001-0004 =$_________ The Contractor shall provide a complete explanation for each item detailing the rationale used in developing the cost. In addition, the Quoter shall submit supporting documentation that will facilitate the determination of whether the amounts proposed are fair and reasonable, e.g., vendor quotes for commercial-off-the-shelf products or services. These items are critical, as the Government will evaluate the Quoter's quote for reasonableness and completeness/accuracy. An analysis will be done on a "total price" basis consistent with a fixed price methodology. c.Technically Acceptability Evaluation Technically acceptability will be determined by the quoter's relevant experience for the same or similar services as those listed in this statement of work. Relevant experience must be clearly demonstrated in the quote and not left to speculation. The Contractor shall identify the projects it has successfully completed that it considers offers evidence that proves its firm has the experience required to complete this project. The evaluation will be based on a Pass/Fail Basis. Failure to submit proof may result in rejection of the quote. d. Price Evaluation All quotes will first be evaluated for technically acceptability, and all quotes with a "Pass" for technically acceptability will then be evaluated for price. The price will be evaluated on the total price proposed. e.Basis of Award The Government will make a firm-fixed price award to the responsible Quoter whose quote represents the lowest price technically acceptable quote. f. Questions Submission: Questions must be submitted electronically via e-mail to Darren.Nutter@bia.gov in Word or compatible read-only format. Quoters shall also reference the RFQ number in the subject line of their quote. The RFQ Due date (closing date) for question submission is on or before 4:00 pm EST, Thursday, April 21, 2011. f.Quote Submission: Quotes must be submitted electronically via e-mail to Darren.Nutter@bia.gov in Word or compatible read-only format. Quoters shall also reference the RFQ number in the subject line of their quote. The RFQ Due date (closing date) for quote submission is on or before 4:00 pm Eastern Time, Thursday, April 28, 2011. 14.0 CLAUSES: This solicitation incorporates one or more solicitation provisions by reference with the same force and effect as if they were given in full text. Upon request, the CO will make their full text available. The provisions at FAR 52.212-1 Instructions to Offerors-Commercial Items (June 2008); 52.212-2 Evaluation-Commercial Items (Jan 1999); and FAR 52.212-3 Offeror Representations of Certifications (Aug 2009) apply to this acquisition. Quoters must include a complete copy of the provision FAR 52.212-3, Offeror Representations and Certification/Commercial Items with their quote. If the Quoter does not have a copy of this provision, you may obtain a copy at www.arnet.gov or contact the CO to receive a copy.
