- 1. SCOPE: The proposed work on this project consists of providing all plant, labor, equipment and materials to demolish the old Fire Station, Building 111, of the West Virginia Air National Guard (ANG) at the Eastern West Virginia Regional Airport, and convert the existing site to equipment parking. 2. LOCATION: West Virginia Air National Guard, 222 Sabre Jet Blvd, Martinsburg, West Virginia, 25405-7704. 3. WORK TO BE ACCOMPLISHED: The work to be performed includes the following principal features: A. Special Notes - 1) Daily removal of debris shall be accomplished at the end of each work day. No materials may be left on the work site that may be wind blown or picked up by passing aircraft. 2) Welding, cutting and burning operations are anticipated in this project. The Contractor shall contact the Base Fire Department at 304-616-5277 to receive the Burning and Welding Permit prior to starting any operations that produce a spark or flame such as burning, welding, grinding, cutting or drilling. 3) Utilities in the area are Government systems. The Contractor must notify the Base Civil Engineer three business days prior to starting any excavation work. No excavations may begin until the Government has issued an AF Form 103, (Base Civil Engineering Work Clearance Request) and confirms that all known utilities have been located and marked. 4) The Contractor must exercise reasonable care not to damage any unmarked utilities and report any unmarked utilities discovered during excavations to the Base Civil Engineer Operations Section immediately, by calling 304-616-5217. 5) All utilities to the building shall be shut down prior to the Contractor taking possession of the Facility. Shutdown and disconnect responsibilities shall be as follows. a. Electrical Service - shall be disconnected at transformer TR-10 located at the north east corner of the Building 140 and through electrical enclosures EE8 and EE9. Power to the gate operator, key pad, and the pole mounted light is to be maintained. Transformer TR-10 shall remain in service and is not to be disturbed under this contract. b. Natural Gas Service has been terminated to the building. The service line was cut and capped by Mountaineer Gas in January 2011 at the T located to the northeast of the building. Contractor is to remove/cut/cap the service line by Building 111 as per New Work Note 3. c. Telephone, Cable TV, Fiber Optics and or other direct wire communications systems and wire from the underground distribution system to the entrance panels within the Facility shall be removed by the Base Communications Squadron and those components deemed salvageable shall be recovered by the Communications Squadron. d. Fire Alarm Systems determined to be salvageable shall be recovered by the Base Fire Department prior to the Contractor taking possession of the Facility. e. Water to the Facility is supplied through a single distribution point. ANG CE shall shut the water off at the appropriate valves once the Facility has been vacated. The Contractor shall excavate the valve locations, remove the valves and cap the lines per the project drawings. f. Sanitary and Storm Sewer services shall be terminated at the project limit line as defined under New Work. B. Demolition - 1) Once all of the servicing utilities have been disconnected and determined to have been made safe by the Base Civil Engineer, the Contractor may proceed with the demolition of the building. Demolition shall be by mechanical disassembly only. No burning or explosive demolition procedures shall be authorized with the exception of the use of cutting torches, saws and grinders to disassemble structural, plumbing or other metal members. This activity is not considered demolition by burning. 2) The Contractor shall demolish the building in its entirety to include removal of the floors and foundations and footings. 3) All trenches for utilities and foundation removal shall be backfilled with soil in 6" lifts and compacted to meet ASTM 1557 Method D (Modified Proctor). Compaction testing is to be accomplished in accordance to ASTM Method D. A copy of compaction testing is to be submitted to the Base Civil Engineer weekly. Backfill and compaction shall continue in 6" lifts until all areas are level with the surrounding surfaces. The Contractor shall use existing materials from the foundation fill. The Contractor shall bring in any additional needed clean/non-contaminated material for fill and all top soil to complete the project. C. Waste Disposal and Site Clean Up - 1) The Contractor shall dispose of all demolition debris or materials at an off Base location in compliance with all State and Federal and local regulatory guidelines. The Contractor shall submit dump receipts or tickets to the Base Civil Engineer weekly. 2) The Contractor is encouraged to recycle all recyclable materials recovered during the demolition process. The Contractor shall provide the BCE with a list of recycled materials. Include the approximate weight and cubic feet of each material recycled. 3) Prior to final acceptance the Contractor shall perform a final cleanup where all debris and waste materials from demolition and construction activities including packaging and/or surplus materials shall be cleaned-up and removed from the Base and the site graded, seeded and mulched or paved as noted in the plans and under the new work section. D. New Work - 1) The Contractor shall terminate the existing water line as close to the building as possible but not more than 5' from the outside the foundation or at the edge of the excavations required to remove the Building's foundation. The termination shall be made by a blind flange with positive grip locking devices or threaded caps as appropriate for the type of water line found. The Contractor shall place a concrete thrust block at the terminal end of the pipe. Backfill the pit in 6" lifts and compact to ASTM Method D. All backfill throughout the project shall meet this specification. 2) The Contractor shall terminate sanitary and storm water pipes as close to the building as possible but not more than 5' from the outside the foundation or at the edge of the excavations required to remove the Building's foundation. 3) The Contractor shall terminate natural gas pipe as close to the building as possible but not more than 5' outside the foundation or at the edged of the evacuations required to remove the Building's foundation. The contractor is to remove the existing valve and install a mechanical end connector that is approved by the BCE and Mountaineer Gas. The contractor is to locate, excavate, disconnect and repair to existing conditions the 1 " service line that feeds Building 111 from the main line located on the SE corner of Building 110. The contractor is to note the location of the terminations on the copy of the site map noting the exact location distances from a minimum of two land marks. 4) The Contractor is to disconnect and remove electrical and communications connections to the building as shown on the plans. Cap the conduit with an approved mechanical fitting. 5) The Contractor is to note the location of the terminations of all utilities on the copy of the site map noting the exact location distances from a minimum of two land marks. 6) The Contractor shall saw cut the surrounding asphalt/concrete as per the plans. Demolish and remove all sidewalks, parking and approach aprons as show on the plans between the saw cuts and the building. Final locations are to be determined by the Base Civil Engineer. 7) Once all demolition and utility work has been completed the Contractor shall grade the site for a lawn area or pavement as applicable. The site shall be contoured to provide positive drainage and gentle transitions between new work and the non-disturbed areas. 8) The Contractor shall seed the area with a commercially available high sun turf mix lawn seed applied at the suppliers suggested rate. The Contractor shall have the area hydro-seeded with high sun turf blend grass seed and paper mulch. An alternative method may be to hand seed the area and hand place the paper mulch, the Contractor would then water the area to ensure adhesion of the mulch. The use of loose mulch (IE straw) shall not be authorized. 9) The Contractor shall install 6 foot tall chain link fence with outriggers and barbed wire to match existing airfield perimeter fence. E. Hazardous Materials Removal and Disposal - The following information and scope of work is based upon an Asbestos Survey, Testing and Report performed and prepared by the Air National Guard Civil Engineering Support Center, in Minot North Dakota (related sections attached) and the survey conducted by Environmental Consulting and Training Services (ECATS) dated 30 Sep 2008 (related sections attached). 1) Asbestos Abatement and Disposal - The Contractor shall be responsible for compliance with all regulations regarding the removal, transportation and disposal of all asbestos materials. A copy of the Minot and ECATS Report will be available during pre-bid walk through of the facility and shown as Attachment 2. a. Per the ECATS report, except for the mudded thermal insulation on the domestic water line above the water heater, all pipe insulation in Building 111 (approximately 640 linear feet) is considered ACM. A majority of the thermal insulation systems are sheathed in aluminum cladding. All steam piping leading from Bldg 110 to 111 was demolished by others. b. Per the ECATS report various samples of the floor tile in the building had readings that indicated that the tile or the mastic where hot for containing asbestos. Due to the age of the installation and the construction plan finish schedule, all of the floor tile and mastic shall be considered ACM or cross contaminated. Removal and disposal of floor tile shall be performed accordantly. c. The Contractor may perform additional testing of suspected materials for asbestos containment at his expense. Should positive containment readings result the contractor shall stop demolition or removal efforts and report the findings to the BCE. No abatement efforts above those noted here shall be paid for with out a negotiated modification of the contract. d. A copy of the approved WV Notification of Abatement, Demolition or Renovation Plan, the waste shipment manifest, with the name of the sanitary landfill, location, type and quantity of materials disposed shall be submitted to the Base Civil Engineer within ten days of removal of the materials. 2) Polychlorinated Biphenyl (PCB) Lighting Ballast - No survey has been completed to determine the types or number of PCB contaminated lighting ballast that may be installed in this facility. The Contractor shall check each lighting fixture for PCB suspected ballast and determine his course of action regarding removal and disposal. Should the Contractor chose removal and disposal of the suspect ballast he shall remove, package and dispose of the ballast in accordance with all applicable Federal, State and Local regulations. A report of the final disposition of removed ballast and copies of disposal receipts shall be submitted to the BCE before the closing date of the contract in the same manner as described in paragraph E.1) d.. 3) Light bulbs - all types of light bulbs shall be collected and properly disposed of in accordance with Universal Waste regulations by the contractor. A report of the final disposition of removed light bulbs and copies of disposal receipts shall be submitted to the BCE before the closing date of the contract in the same manner as described in paragraph E.1) d.. 4) Lead Based Paint - due to the age of the facility all painted surfaces are presumed to be lead based paint and shall be handled in accordance with federal and state regulations. A report of the final disposition of removed lead based painted surfaces and copies of disposal receipts shall be submitted to the BCE before the closing date of the contract in the same manner as described in paragraph E.1) d.. F. Testing and Certifications - The following test and or inspections shall be conducted and certification provided to the Contracting Representative. 1) Concrete - No cylinder break samples shall be required however certification of mix design shall be submitted for the Material Submittal process and a copy of the batch mix tickets shall be supplied to the Office of the Base Civil Engineer upon delivery of the concrete. 2) Crane Operations - The Contractor shall provide the BCE with a photographic copy of any Crane Operators certification prior to employing crane operations on the Base. 3) Asbestos Operations - The contractor shall provide the BCE with a photographic copy of the licenses from the state of West Virginia Department of Environmental Protection for the Asbestos Project Designer, Industrial Hygienist, Air Monitor, Asbestos Contractor, Asbestos Supervisor (s), and Asbestos workers. 4. TIME FOR COMPLETION: a. The Contractor shall submit the "Asbestos Abatement Plan" for approval by the State of West Virginia within 10 days of the notice of award of the Contract. b. The Government shall allow for the 14 day "Asbestos Abatement Plan" review to be completed before issuing the Notice to Proceed. c. The Contractor will be allowed to mobilize and store his equipment on the project site during the asbestos abatement review. d. The entire work shall be accomplished within 90 days after date of issuance of the Notice to Proceed. e. Work hours - The contractor may work Monday through Friday 0630-1700. Any work outside these hours must be coordinated with and approved by the BCE Staff. 5. TOUR OF THE JOB SITE: Tours may be arranged by contacting the Construction Management Section of Base Civil Engineering, West Virginia Air National Guard, 222 Sabre Jet Blvd, Martinsburg, West Virginia 25405. Telephone: 304 616-5198 or 304-616-5233. Normal duty hours are 0630 to 1600 Tuesday through Friday. 6. PERFORMANCE: a. Submission of a bid by a Contractor shall be accepted as prima-facie evidence that he has examined the specifications and has satisfied himself as to the nature and location of the work and all other matters which can in any way effect the work or cost thereof under this contract. Any failure of the Contractor to acquaint himself with all available information, including a physical survey of the site of the proposed work, will not relieve him from successfully performing all the work required for a complete finished job. Estimated quantities are provided for reference only and should not be used to bid the job. b. The above brief outline of principal features of the work in no way limits the responsibility of the Contractor to perform all work and furnish all plant, labor, and materials required by the specifications and plans referred to herein. c. Installation, workmanship, and all materials shall be guaranteed in writing by the Contractor to be free from defect under normal use for a period of at least one year. All repairs by the Contractor shall be performed at no expense to the Government. d. Material submittals shall be accomplished in accordance with instructions pertaining to AF Form 3000, "Material Approval Submittal". Contractor is to submit all shop drawings and material submittals for this contract within ten (10) calendar days of issue of the Notice to Proceed, or as directed by the Contracting Officer, to the Contracting Officer Representative. All submittals shall consist of one completed AF Form 3000 and four copies of material specifications and Material Safety Data Sheets "MSDS". e. Contractor is to submit shop drawing in four copies to the Contracting Officer Representative. Two copies will be returned to the Contractor indicating approval or disapproval and the date of such action. The Contractor will not proceed with work that is detailed on shop drawings until the drawings are approved. f. The contractor shall provide all safety equipment and instructions for use by his/her employees, and ensure that a copy is provided to the Contracting Officer Representative prior to commencing work. g. Material Safety Data Sheets - A copy shall be submitted with the AF Form 3000 Material Approval Submittal form and the Contractor shall have a copy on display at the job site for all materials brought to and or stored at the job trailer, site office or at the site storage location. The Contractor shall ensure that copies have been provided to the Contracting Officer Representative prior to transporting any materials on to the Base. h. All construction materials and chemical compounds shall be stored in the original container or properly marked containers and approved storage containers supplied by the Contractor. All hazardous or flammable compounds shall be stored in approved lockable storage boxes that shall be locked when not in the Contractors' or his employee's direct supervision. All surplus materials and or compounds shall be removed from the Base when work on the project is complete. i. All work must comply with the International Building Code, National Electrical Code, National Fire Protection Association Code, WV Division of Highways Specifications, and all other applicable codes. j. Contractor must submit a Contract Progress Schedule (AF Form 3064) 10 days prior to beginning the project. Contractor must submit a Contract Progress Report (AF Form 3065) monthly in conjunction with application for payment. k. Progress meetings will be conducted as a minimum monthly or as frequently as needed. The contractor is to set up, conduct, and produce minutes to the BCE within 7 days each meeting. 7. SECURITY: Contractors will need to provide a listing of work personnel to Security Forces for background checks and contractor badges according base procedures. 8. SAFETY: Project safety is the responsibility of the contractor. The Contractor is to submit their safety plan and it must be approved by the BCE prior to the commencement of any work. 9. PAYROLLS AND PAYMENT: Certified payrolls will be required. A paper copy of the invoice will provided to the Contracting office and approved before an invoice is placed in the Wide Area Workflow (WAWF) system.
