- Description
- The Department of Veterans Affairs (VA), Veterans Health Administration (VHA) has a requirement for an online Patient Health Risk Assessment (HRA) Integrated with VHA's Electronic Medical Record and Personal Health Record. VHA is seeking a patient self-administered HRA application integrated with VHA's Personal Health Record and Electronic Medical Record. HRAs are instruments that collect information from individuals through the use of structured questionnaires. HRAs determine risk status via validated algorithms or decision rules and provide tailored feedback to the individual about his or her health status, with specific recommended steps to improve health. Core features of an HRA include assessment of personal information, past medical and surgical history, medications, family history, health behaviors (use of tobacco, alcohol, diet, physical activity, etc.), and receipt of preventive care (screenings and immunizations, etc.). Elements also desirable include information on self- management for common chronic conditions such as diabetes and heart disease, health risks relevant to the elderly, and readiness-to-change for health behaviors that increase patient risk for disease and/or injury. The VHA is also interested in assessing issues relevant to combat Veterans, such as military-related exposures, trauma, or sequelae related to military experience. The North American Industrial Classification (NAICS) for this effort is 541511. This acquisition will be conducted on the basis of full and open competition. The resultant contract will be firm fixed priced and will include one (1) 12-month base period, one (1) 12-month option period and one (1) six (6) month option period. A formal source selection process will be utilized and contract award will be based on the best overall (i.e., best value) proposal that is determined to be the most beneficial to the Government. The Government intends to release a DRAFT RFP on or about April 20, 2011. Potential offerors are reminded that all draft documents are subject to change prior to release of the final solicitation on or about June 1, 2011. The final solicitation will govern over any previously released draft documents. Questions/responses shall be submitted to the contract specialist at Carolyn.Klein@va.gov.
