- Letter to Vendors ENCLOSURE 7 - CONTRACTORS PERFORMANCE CONDITIONS & RESPONSIBILITIES, STANDARDS OF ETHICAL CONDUCT, WORKPLACE VIOLENCE ENCLOSURE 6 - STATEMENT OF WORK ENCLOSURE 4 - PROPOSAL SUBMISSION REQUIREMENTS ENCLOSURE 3 - ADDITIONAL TERMS AND CONDITIONS ENCLOSURE 2 - CONTRACT ADMINISTRATION ENCLOSURE 1 - BID SCHEDULE This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12 of the FAR and as supplimented with additional information inlcuded in this notice. This announcement constitutes the only solicitation; proposals are being requested and a seperate written solicitation will not be issued. Solicitation number 241-COM-11-13 applies and is issued as a Request for Proposals (RFP) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-51 The North American Industry Classification System (NACIS) code is 541990 with a small business set aside of $7M. This is a small business set aside. The Contract Line Items (CLINS) are as shown below. See Enclosure 1, Bid Schedule for price details. CLIN 0001 - 0013 (BASE YEAR PERIOD) - The Contractor shall provide Radiology Interpretation Services for the Rosebud IHS Hospital, Rosebud SD as indicated in the Scope of Work. CLIN 0014 - 0026 (Option Year One Period) - The Contractor shall provide Radiology Interpretation Services for the Rosebud IHS Hospital, Rosebud, SD as indicated in the Scope of Work. CLIN 0027 - 0039 (OPTION YEAR TWO PERIOD) - The Contractor shall provide Radiology Interpretation Services for the Rosebud IHS Hospital, Rosebud, SD as indicated in the Scope of Work. CLIN 0040 - 0052 (OPTION YEAR THREE PERIOD) - The Contractor shall provide Radiology Interpretation Services for the Rosebud IHS Hospital, Rosebud, SD as indicated in the Scope of Work. CLIN 0053 - 0065 (OPTION YEAR FOUR PERIOD) - The Contractor shall provide Radiology Interpretation Services for the Rosebud IHS Hospital, Rosebud, SD as indicated in the Scope of Work. The Indian Health Service (IHS), Rosebud Hospital is seeking Radiology Interpretation Services for Base Year Plus Four One Year Options as stated in Enclosure 6 - Scope of Work. This is a Firm Fixed price Requirements Type of Contract. 52.212-1 Instructions to Offerors-Commercial Items. As prescribed in 12.301(b)(1), insert the following provision: (June 2008) (a) North American Industry Classification System (NAICS) code and small business size standard. The NAICS code and small business size standard for this acquisition appear in Block 10 of the solicitation cover sheet (SF 1449). However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees. (b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show- (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, and telephone number of the offeror; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; (5) Terms of any express warranty; (6) Price and any discount terms; (7) "Remit to" address, if different than mailing address; (8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically); (9) Acknowledgment of Solicitation Amendments; (10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and (11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. 52.212-2 EVALUATION - COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considers. The offeror must included in their proposal the unit price, the list price, the performance period after contract award, the prompty payment, discount terms, the DUNS & Bradstreet Number (DUNS), the Taxpayer Indentification (TIN)l, and the certification of business size. In order to receive an award from the Indian Health Service contractors must have a valid Central Contract Registration (CCR) www.ccr.gov. Proposals must be receipted by IHS no later than May 6, 2011, 3:00pm CST. Interested vendors capable of furnishing the goverment with the item specified in this combined synposis/solicitation should submit their proposal to the below address or email address. The proposal must reference Solicitation Number 241-COM-11-13. All responsible sources may submit a proposal, which if timely received, will be considered by the agency.
