A2079 Mayfield Infinity XR2 Base Unit 1 each A2114 Mayfield Infinity XR2 Skull Clamp w/ Acc. 1 each A2111 Mayfield Infinity XR2 Tri-Star Adaptor 1 each Notice to Contractor: Solicitation number N00259-11-T-0246 is issued as a notice to procure from only one source IAW FAR 13.106-1(b). This proposed contract action is used for Neurosurgical Surgeries for Brain Tumors, consumables that are compatible with the instrumentation that Naval Medical Center San Diego already owned. The Naval Medical Center San Diego intends to solicit with only one source under the authority of FAR 13.106-1(b). The source is Integra LifeSciences Corporation, CAGE Code: 1R0B9, NAICS: 339112. Justification: As required by FAR 13.106-1(b), the Contracting Officer has determined that Integra LifeSciences Corporation ™s devices is the only company that manufactures and distributes the supplies that is compatible with equipment currently used by NMCSD. IAW FAR 5.207(c)(15)(ii), all responsible sources may submit a quotation, which shall be considered by the agency. The government will only consider firm, fixed price quotations. The attached solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-50. It is the contractor ™s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisition.gov and/ or www.acq.osd.mil. Please fax (619-532-5596) or email your quote to Merlinda Labaco at merlinda.labaco@med.navy.mil on or before 04/23/2011 07:00 AM PST. Email submissions are limited to 2MB. The submitter should confirm receipt of facsimile and email submissions.
