Description
LMS2615 OSHA Competent Person Designation and Acknowledgement Health and Safety Worksheet Representations and Certifications Offeror's Price Schedule LMS2125 Services Terms and Conditions S07554 Bluewater Drilling Statement of Work RFP 4332 Cover Letter and instructions to offerors S.M. Stoller Corporation, Legacy Management support contractor to the Department of Energy, Office of Legacy Management, has a requirement to drill and install two monitoring wells at their Bluewater Title II disposal site, located near Grants, New Mexico. The requirement is to provide all labor, equipment, tools, materials, transportation, services and incidentals as indicated in the Statement of Work S07554, Monitoring Well Installation at Bluewater, NM, Disposal Site. The wells will be completed in alluvial deposits underlying basalt. The subcontractor will be responsible for completing the applicable well records and submitting them to the New Mexico Office of the State Engineer. All drilling and well completion activities will be performed in accordance with the drilling rules and regulations of the New Mexico Office of the State Engineer. The well drilling company will be required to have a license issued by the State Engineer. Drill rig supervisors must also be registered by the State Engineer. The subcontractor shall be knowledgeable of general and local conditions that may affect the cost or quality of the performance of the work, including the ability of the subcontractor's equipment to perform the work. Because of the technical nature of the work, the subcontractor shall be a first- tier subcontractor to the Contractor, shall have a minimum of five (5) years business experience in environmental water well drilling, and shall have the ability to provide the necessary and required drilling equipment. The subcontractor's Drill Rig Operator shall have a minimum of three (3) years drilling experience involving environmental well installations, and shall have a currently active license issued by the New Mexico State Engineer. The subcontractor shall have a minimum of two (2) trained and experienced driller's helpers for this job. Firms submitting proposals under this Request for Proposal will submit their offer per the requirements as listed in the Statement of Work S07554 and RFP Cover Letter for the solicitation. Submissions will be evaluated based on the most responsible and responsive offer considering all elements. In order to receive award under this RFP the successful offeror shall provide all submittals as requested in the solicitation package. All responsible sources may submit a proposal which shall be considered by the Contracts Administrator's office for S.M. Stoller. This is a synopsis only; the full solicitation includes specifications, drawings and submittal requirements and will be issued on or about April 21, 2011.
