- RFQ: 246-2011-Q-0004 Release April 18, 2011 Close May 5, 2011 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, (includes amendments thru FAC 2005-50, effective April 15, 2011) as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. NAICS CODE: 339114, SIZE STANDARD: $5M Set Aside: Total Small Business BACKGROUND: The Indian Health Service (IHS) National Supply Service Center (NSSC) located in Oklahoma City, Oklahoma has a requirement for portable dental equipment identified in this combined synopsis solicitation. Brand name or equal equipment must be quoted. The following equipment brand is Aseptico. CLINs: There are seven (7) CLINs as shown below: ITEM QTY DESCRIPTION PRICE EACH TOTAL ADC-01 20 Patient chair in soft case ADC-08 20 Operator stool in soft case AEU-425 20 Transport II System ALU-40 LED 20 LED portable light in soft case ASC-10 5 Scaler (for 5 of the units) AHP-101 20 Lowspeed handpiece for prophy AHP-71TI 20 Highspeed handpiece for operative Shipping 1 Shipping-if applicable Grand Total: The Government intends to award one Firm Fixed Price contract. The anticipated date of award is May 15, 2011. The following provisions and clauses apply to this acquisition: FAR 52.212-1, Instructions to Offerors-Commercial Items. No addenda. FAR 52.212-2, Evaluation-Commercial Items. The following factors shall be used to evaluate quotes: (1) Price, (2) Technical Capability. Factors are of equal importance. Award will be made to the responsible quoter whose quote is technically acceptable, responsive to the requirements of this solicitation, and presents the best overall value to the Government. Only one award will be made. The Government intends to evaluate quotes and make award without discussions; however, the Government reserves the right to hold discussions if necessary. FAR 52.212-3, Offeror Representations and Certifications - Commercial Items. Quoters shall include a completed copy of these provisions with its offer. FAR 52.212-4, Contract Terms and Conditions-Commercial Items is incorporated by reference. Date/Time/Place for submission of quotes: May 5, 2011, NLT 4PM CST. 1) Prices for each CLIN and a grand total by completing the schedule above; 2) Estimated delivery date; 3) A completed copy of provision 52.212-3, Offerors Representations and Certifications - Commercial Items and 52.222-52; 4) Vendor DUNS & EIN numbers; 5) CENTRAL CONTRACTOR REGISTRATION: All quoters must ensure they are registered in the Central Contractor Registration. To register, please go on-line to http://www.ccr.gov. 6) A point of contact for the quote that includes name, title, telephone/email/fax and address. Questions regarding this requirement must be emailed to sara.marcus@ihs.gov by 4 p.m. CST April 21, 2011. Questions received after 4 p.m. CST April 28, 2011 will not be accepted. Answers to questions and clarifications will be posted as an amendment to the solicitation. All responsible sources may submit a proposal which shall be considered.
