Description

This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in Subpart 12.6 and Far Part 13 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is being issued as Request for Quote # VA-257-11-RQ-0206. The solicitation document and incorporated provisions and clauses are those in effect through the most recent Federal Acquisition Circular. The NAICS Code is 423450. The Department of Veterans Affairs Medical Center, Dallas, TX has a requirement for Suction Regulators and intends to solicit quotes and award a firm fixed price contract. Contractor shall provide the following equipment/supplies: (1) Platinum Series Continuous Regulator P/N 3800, QTY 17 EA; (2) Platinum Series, Intermitting/Continuous Regulator 0-200mm HG W/Selector Valve P/N 3804, QTY 35 EA; (3) Diss Hand-Tight Nut and Gland P/N 1504, QTY 20 EA; (4) High Flow Bubble Barb Patient Adapter P/N 2469, QTY 52 EA; (5) NCG Rectangular Wall Plate Adapter P/N 1493, QTY 32 EA. This solicitation is set aside for Service Disabled Veterans Owned Small businesses. The following provisions apply to this solicitation: 52.211-6 Brand Name or Equal, 52.212-1, Instructions to Offerors-Commercial, 52.212-4, Contract Terms and Conditions-Commercial, Commercial Advertising, 852.271-70, 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial, including clauses incorporated by reference under Section b (1)(5)(11-16)(23-24); Offerors are to submit pricing. Process of determining contract award: award will be to the lowest-priced responsive, responsible bidder who can provide the Suction Regulators. This information should be specific, concise, and complete and provide sufficient information to demonstrate the offeror's capacity to meet or exceed the requirements of the contract. The information required in provisions 52.212-1, 52.212-2, a completed copy of provisions 52.212-3, Offeror Representations and Certifications, Commercial Items, 852.219-70,Offerors desiring to obtain copies of the reference provisions may contact this office or access clauses electronically at http://www.arnet.gov/far/ or http://www.va.gov/oa&mm/vaar.
