Attachment J-5: Verification of Idaho Worker's Comp form Attachment J-4: Drawings Attachment J-3: Supplemental Project Specifications Attachment 1 - Wage Determination RFQ - Solicitation NOTE: Amendment to socioeconomic set-aside for this project from Total Small business set-aside to HUBZone set-aside (in accordance with FAR 19.13 Historically Underutilized Business Zone Program) Jeru-Lindsey Decommissioning on the Idaho Panhandle National Forests, Sandpoint Ranger District. This project consists of scarification of the road prism, removing and disposing culverts, gates, and barriers. The decommissioning, will consist of Stream Channel Restoration ranging from Category 1 to Category 2, and includes full recontouring of the slopes. In addition, to the earth work, the project will consist of seeding, fertilizing, and/or mulching disturbed areas as designated. This will be a construction project subject to Davis-Bacon. Price Range: $25,000 - $100,000. The project location may be reached by taking HWY 95 north out of Sandpoint Idaho. Travel up along HWY 95 North for approximately 10 miles and turn onto Upper Pack River Road, Forest Road #231. Continue on Forest Road #231 for approximately 8 miles, and turn left onto Forest Road #293. Keep on Forest Road #293 for about 2 miles until you arrive at the project location Project Roads start here as will be shown on the Vicinity Map that will be included with the solicitation. The project work area is located in Sections 32 & 33, Township 60 North, Range 2 West, Boise Meridian. It is anticipated the solicitation will be issued approximately April 20, 2011. The solicitation and all associated attachment will be available free of charge at the Federal Business Opportunities website at http://www.fbo.gov/ The solicitation will state the exact due date and time for responses. This project is a total set-aside for HUBZone small business concerns. The project is expected to start after July 1, 2011 and conclude not later then September 30, 2011.
