Description

This is not a solicitation announcement. This is a sources sought notice only. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, hub zone, 8(a), small disadvantage business, woman-owned, small business, or large business) relative to NAICS 339116. Responses to this notice will be used by the Government to make appropriate acquisition decisions and are not considered adequate responses to a potential future solicitation announcement. The Department of Veterans Affairs Medical Center dental facility at Fort Harrison, Montana is seeking a full service dental laboratory to provide a variety of dental devices and services. Due to time constraints related to the local veteran patient population, and the timeliness of the service needed; the contractor's full service dental laboratory must be located within the greater city limits of Helena, MT in order to avoid the need of shipping everything. The contractor will be able to provide approximately the following units of service on a monthly basis: 1.50 units of crowns/bridges 2.10 partials 3.10 dentures *4. 3 relines 5. 8 mouth guards *6. 8 repairs of damaged appliances * These procedures are critical for patient care and require turn over times of 5 days or less. The following lists the products required by contract between a full service dental lab and Department of Veterans Affairs at Fort Harrison, MT: Fixed A.Crowns/Bridges - Metal (Full,overlays,3/4) a.Precious b.Non-precious B.Crowns/Bridges - Porcelain a.Veneer b.Precious c.Non/Semi precious d.Empress, Zirconia, Cerlon C.Implant crowns/Bridges D.Metal Posts E.Provisional Crowns/Bridges Removable A.Partial denture a.Valplast b.Acrylic (ww, ball clasps) c.Ticonium framework (precision, simi precision attachments) B.Full dentures a.Implant supported b.Regular c.Surgical templates C.Jump/Relines D.Repairs Mouth guard, splints Delivery: Critical element is the turnaround times. Delivery times must fall within these time frames. A.Crowns a. Porcelain14 Days b. Responses should also include company name, address, point of contact and answers to the following questions: (1) Is this product or service available under a federal supply schedule (GSA)? If yes, please provide the contract number. (2) Do you manufacture or directly provide these services? 3) Is your business large or small? If your business is small please indicate if your business is also certified in a subcategory for small business i.e. service disabled veteran owned small business, veteran owned small business, hub zone, 8(a), small disadvantaged business, or woman-owned business.
