PERIOD OF PERFORMANCE The period of performance for this AGREEMENT will be from June 1, 2011 through May 31, 2012. B. SCOPE OF OPTOMETRY SERVICES 1. The service of the Professional or designee will be made available to the Center, as follows: a) Eye examinations and routine vision care. 2. All routine care will be performed at the office of the Professional. Specific hours for eye examinations will be mutually agreed upon by the Professional and the Center Director or designee. Any third party payments will be credited to the Center; and the services to be provided will include, but not be limited to, the type needed in accordance with the priority system listed in Job Corps Technical Assistance Guide. These include provision of the following: a) Provision of routine eye examinations at the professional site between the 45th and 75th day after the enrollee's arrival on Center. b) Provision of routine vision care, as defined by the Job Corps Technical Assistance Guide (TAG 6-A), as approved by the Center. c) Provision of necessary vision health procedures, which will promote the vision care of all enrollees. d) Provision of an appropriate referral system to ensure the receipt of special care, as defined by Technical Assistance Guide (TAG 6-A). e) Provision for coverage by a qualified substitute, approved by the Center, during the absence for the Professional. 3. The development, implementation, and monitoring of the total Center Vision Health Program to include, but not be limited to: a) Advice to the Center Director, Medical Director and Wellness Staff in all vision care, or vision-related matters. This shall include the advisability of terminations for vision reasons, the cost of services, and any other areas deemed appropriate. b) Provision for assistance in developing the coordination and integration of the Vision Health Program with the other health components and other Center activity. c) Insurance of the maintenance and confidentiality of all required vision care records. d) Provision of assistance in the development and implementation of the vision education component of the Health Education Program. e) Identification and utilization of the most appropriate and least expensive sources for obtaining necessary vision care supplies. f) Insurance and maintenance of signed and dated Standing Orders (annually) for vision care in accordance with the Policy and Requirements Handbook (PRH), Chapter 6. C. STAFFING 1. The Professional will provide services at office facility as per appointment. D. THIRD PARTY PAYMENT The majority of Job Corps enrollees meet the eligibility criteria for third party coverage (e.g., MEDICARE). The Professional will process and collect all third party payments independent of the Gainesville Job Corps Center. All such income belongs to the Optometry Contractor.
