Classification Code: Q -- Medical services NAICS Code: 621 -- Ambulatory Health Care Services/621111 -Center Wellness Department, Center Physician Contracted Service for a Medical Professional at the Gainesville Job Corps Center. A. PERIOD OF PERFORMANCE The period of performance for this agreement will be from June 1, 2011 through May 31, 2012. B. SCOPE OF MEDICAL SERVICES The Medical Contractor agrees to provide for the services of a Physician licensed to practice in the State of Florida, hereinafter referred to as Professional, to be designated the Center Physician. 1) The services of the Professional or designee will be made available to the Center at their medical facilities as follows: a) Such services will be provided to average a minimum of (16) hours per week (Monday through Friday), as mutually agreed upon to include emergency on-call services. The two (2) week periods during Christmas and July 4th are excluded. During such holiday periods appropriate on-call and referral procedures will be needed for medical emergencies. b) The Professional will be fully supported by the Center's in-house medical staff and resources in providing: i) Physical examinations of new students in order to establish a medical assessment record for each, including interpretation of all routine skin (PPD), X-ray (when indicated) and laboratory tests. ii) Treatment of student "Sickle Cell" cases. iii) Treatment and/or consultation of emergency accident situations. iv) Provision for, or the arrangement of the necessary immunizations, as directed in PRH Chapter 5, Section 2.4 and Appendix 601. v) Provisions for all necessary referrals to specialists and institutional care, upon approval. vi) Provision for continuing care of all hospitalized Job Corps students, except those in federal facilities or under the care of another professional for non-medical reasons, for which third party payment may be applied, if qualified. vii) Provision for twenty-four (24) hour emergency coverage. viii) Provision of diagnostic, evaluative and treatment services to students. 2) The Professional may be called upon to offer "emergency" treatment for telephonic advice in the case of emergencies at hours of the day other than those designated in Paragraph B. 1) (a) above. 3) Services shall be available: a. At the Center b. At the Professional's office, or c. At such other places, as directed by the Center Director and the Professional to meet the needs of the student 4) The Professional will monitor the total Center Health Program, to include: a. Advice to the Center Director, or designee and the Wellness Manager on all medical or health related matters. This shall include, but not be limited to, the advisability of medical terminations and the development of sources of care at the lowest possible cost. b. Provision for a comprehensive set of written standing orders to be updated and signed annually for the professional guidance of paraprofessional personnel when primarily acting in the absence of direct supervision by the Center Physician. c. Provision for assistance to the Wellness Manager in coordinating the health components with other Center activities. d. Monitoring of the Center Health Education Program. e. Provision for assistance to the Wellness Manager in developing agreements with hospitals for impatient care and other needed services. f. Provision to ensure that specialty care is timely and delivered at reasonable cost. g. Oversight of the Center's Family Planning Program. C. STAFFING 1. The physician serving the Center must be licensed to practice in the State of Florida. A copy of the physician's current medical license, curriculum vitae and proof of current malpractice insurance are to be provided, as part of this subcontract and renewals provided, as they occur. 2. The Medical Contractor may provide appropriately qualified medical professionals (e.g.; Nurse Practitioner/Physician's Assistant, Interns, etc.), as required. The supporting medical professionals must also be licensed to practice in the State of Florida. A copy of their resume, curriculum vitae, license, and/or certification will be provided as they occur during the lifetime of the subcontract. 3. The Medical Contractor will provide for the services of a physician on-Center a minimum of eight (8) hours per week. The remaining eight (8) hours of service can be provided by a qualified medical professional (Nurse Practitioner/Physician's Assistant). D. FINANCIAL CONSIDERATIONS 1. All funds providing payment for services rendered under this agreement will be furnished by the U.S. Department of Labor Prime Contract and all provisions and regulations of said prime contract are incorporated in this agreement by reference. 2. The Gainesville Job Corps Center agrees to provide payment for services rendered to enrolled Job Corps students by the Professional and/or support medical professionals staff at the rate specified in paragraph F., below for a maximum of (16) hours per week. 3. The Professional may provide more than (16) hours of service per week if deemed necessary for the provision of necessary medical care to the students, but only with the expressed written approval of the Center Director and if the cost for the additional hours of service is within the maximum amount allotted to this Agreement, or any modification thereto. 4. The Medical Contractor will provide a detailed monthly invoice in a format, which is compatible with the Gainesville Job Corps Center financial reporting requirements.
