- The Department of Veterans Affairs (VA) is requesting information from appropriate companies or parties currently capable of contract support in providing a Statement of Conditions follow up survey to identify and address Life Safety Code deficiencies for Bay Pines VA Healthcare System at Bay Pines, FL. 1. PURPOSE: This notice is part of Government Market Research, a process for obtaining the latest information from industry with respect to their current and near-term abilities. All company proprietary information contained in the response shall be separately marked. Any proprietary information contained in response to this request will be properly protected from unauthorized disclosure. The government will not use proprietary information submitted from any one firm to establish the capability and requirements for any future systems acquisition so as to not inadvertently restrict competition. 2. OBJECTIVES: Initial market research has indicated many sources are available to fulfill this requirement. This notice is being sent to the public to generate interest and obtain additional information from other companies in fulfilling this requirement. Please contact Jennifer Rohrbach with any information regarding your firm's ability to fulfill this requirement. The following description is provided to assist offerors in determining their interest in providing the requested services. 3. REQUIREMENTS: The Contractor shall conduct a Statement of Conditions follow up survey to assist in the preparation for the Joint Commission triennial surveys for Bay Pines VA Healthcare System at Bay Pines, FL. a.All buildings/space, on and off campus, for which Joint Commission requires a Statement of Conditions shall have a follow up survey. Evaluate newly constructed areas that were not assessed and include into the Statement of Conditions as appropriate. Conduct a random survey of fire and smoke barrier walls above the accessible ceilings. (As an infection control precaution, we will not survey above ceilings in patient occupied rooms). On redlined drawings, denote wall deficiencies noted for future repair. b.Deviations from National Fire Protection Association (NFPA) 101 shall be identified and documented so that maintenance related work orders and a Part IV; Plan for Improvement can be developed by the vendor or facility. c.Conduct a follow up of Plans for Improvement identified survey of the Buildings in order to determine compliance with the 2009 edition of NFPA 101, Life Safety Code. d.Provide recommendations for the appropriate occupancy classification for each section of each building. e.Review electronic Joint Commission data and assist in its update. f.Upon completion of the inspection, the surveyor shall provide an overview of findings and identify any serious deficiencies that need immediate attention to hospital leadership and Environment of Care staff. Deviations from NFPA 101 shall be identified and documented so that maintenance related work orders and a Part IV, Plan for Improvement can be developed. All minor deficiencies that are best corrected through normal maintenance efforts or work orders are to be provided by separate letters and lists. These deficiencies shall only be included in the draft statement of conditions documents. g.The following data is to be presented in the four part Joint Commission format using Microsoft Word. 1.Part I - Introduction, Part II - Basic Building Information, Part III - Life Safety Assessment and Part IV - Plan for Improvement forms. 2.Information provided by the medical center regarding any equivalencies approved by the Joint Commission (if any). 3.Small scale floor plan drawings showing the current room layout, fire walls, smoke barrier walls, smoke compartment names or designations and occupancy classification for each floor. Also identify suites, suite sizes, hazardous rooms, areas open to healthcare egress corridors and key note any special life safety features. 4.Prepare a preliminary Part IV, Plan for Improvement in the draft submittal for any items that are not correctable under the facility work order system. After receiving the medical center's input, complete the Part IV for inclusion in the final statement of conditions documents. h.Provide one master and two numbered copies of the statement of conditions documents and two computer CD's containing the final statement of conditions documents and drawings shall be provided. i.Follow-up technical support shall be provided by telephone/electronic mail to Bay Pines staff on Statement of Condition or other fire related issues. j.Provide review and reports of findings for construction projects requiring a Fire Protection Engineer assessment. k.Dates of the surveys may be negotiated with the facility. However, all onsite surveys should be completed no later than May 20, 2011. After all surveys are completed, the surveyor shall schedule an exit conference with Safety, Engineering, and Leadership staff to discuss findings and trends. 4. INSTRUCTIONS TO RESPONDENTS: Interested parties should provide capabilities, qualification statements, and examples that demonstrate their ability to satisfy the requirements detailed in paragraph 3 of this announcement. Companies or parties are to use a minimum number of pages to describe in detail the information requested and submit responses in Microsoft Office compatible electronic format. The page limit for responses is 5, 8.5" x 11" double-sided pages, including attachments, and no smaller than size 12 font. Responses must include: A) Industry Point of Contact (POC) for technical and contractual issues to include name, position, telephone number, and e-mail address. B) Overall capabilities as compared against Paragraph 3 Requirements. Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), or small business concerns should indicate interest in this acquisition by providing to the Contracting Officer, not later than 5 calendar days of this notice, evidence of its interest to provide this requirement and a positive statement of eligibility as a SDVOSB, VOSB, or small business concern. If adequate responses are not received from SDVOSB, VOSB, or small business concerns, the acquisition may be solicited on an unrestricted basis. Responses to this notice must be in writing; facsimile responses will not be accepted. Award will be made on the basis of technical acceptability of proposals meeting or exceeding the acceptability standards for non-cost factors, resulting in the best value for the Government. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). All prospective offerors interested in submitting an offer must have a commercial and government entity (CAGE) code. All responsive offerors will be considered by the agency. The entire solicitation will be made available on the Federal Business Opportunities (FBO) Web Site at http://www.fbo.gov. Solicitation release date is on or about 6 May 2011 with responses due back 12 May 2011. The expected date of contract award is 13 May 2011. You may download the solicitation and submit your quote electronically via E-mail. Faxed responses will not be accepted. Once the solicitation is posted it is incumbent upon the interested parties to review this site frequently for any updates/amendments to any and all documents. DISCUSSIONS: One-on-one information gathering sessions with respondents are currently not contemplated. The Government may elect to conduct one-on-one discussions with industries based on its analysis of the notice responses, the quality of submission, and the specifications in the information provided. The Government is under no obligation to conduct these sessions. The Government does not commit to providing a response to any comment or questions. Any revision to this notice based on comments received remains solely at the Government's discretion.
