- SOURCES SOUGHT NOTICE - DURESS SYSTEM THIS ANNOUNCEMENT IS NOT A REQUEST FOR PROPOSAL The James E. Van Zandt Veterans Affairs Medical Center, Altoona, PA is attempting to procure a duress notification system to be utilized in a number of locations throughout the hospital campus, annex, and community based outpatient clinics. The system should be able to meet the following requirements as identified in the Scope of Work listed below: Scope of Work for a System-wide, Network based, Duress System. The Altoona VAMC (Veterans Affairs Medical Center) has a requirement for a duress system that can meet the following requirements: 1. Keyboard based alarm and alert system which uses keystroke combinations, indoor wall mounted wireless buttons and remote wireless buttons for initiation of alarm through a computer based network system. 2. System shall annunciate on computer screens, existing two-way radio system (Motorola), phones including 911 and strobe with audible alarm. 3. Keyboard shall initiate alarm when PC is logged on, logged off or locked. 4. Keyboards shall be clearly labeled for ease of use. 5. Installation shall include; the server, software, programming, repeaters, interfaces and all devices necessary to provide a complete turnkey installation. 6. Two days of onsite training will be required. Training session will be held at Altoona VAMC Building #1. 7. One year service, including online support and software updates. 8. System shall cover one campus style facility and four smaller remote facilities on the same network. System shall include a minimum of--- "Johnstown, Dubois, and State College CBOC's (Community Based Outpatient Clinics)----total-70 keyboard capable of initiating alarms that annunciate to; local 911 dispatch, 2 local strobes with audio, on screen, Altoona Veterans Affairs Medical Center (VAMC) police office. Total of 18 wireless wall mounted push button initiators in bathrooms to annunciate at local strobe with audio. "Altoona VAMC behavioral health building-----24 keyboards capable of initiating alarms that annunciate at; Altoona VAMC police office, on screen, local strobe with audio, Administrative officer of the day (AOD) desk and VAMC two-way police radio (Motorola). "Altoona VA Medical center building #1----45 keyboards capable of initiating alarms that annunciate at; Altoona VAMC police office, AOD Desk, on screen and VAMC two-way police radio. "Altoona VAMC building #1-----2 remote wireless initiator buttons for use in the Emergency Room only, capable of annunciating at Altoona VAMC police office, AOD Desk, on screen and VAMC two-way police radio. "Altoona VAMC building #7---- Total of 6 wireless wall mounted push button initiators in bathrooms capable of annunciating at a local strobe with audio. "Altoona Annex-----------Total of 25 keyboard capable of annunciating alarms at; the VA police office, 911 and on screen. 9. System shall be expandable to meet future requirements. 10. Some keyboards, all the CBOC's at least will have multiple initiators. These key boards shall have the option of local alarm to strobes with audio or phone to security dispatch. Facility Addresses: Altoona Main campus and outbuildings 2907 Pleasant Valley Boulevard Altoona, PA 16602 Johnstown CBOC 1425 Scalp Avenue, Suite 29 Johnstown, PA 15904 State College CBOC 3048 Enterprise Drive Ferguson Square State College, PA 16801 Dubois CBOC 190 West Park Avenue, Suite 8 Dubois, PA 15801 Altoona Annex 201 Cayuga Ave. Altoona, PA 16602 Security Requirements: Award shall incorporate the requirements of VAAR 852.273-75 Security Requirements for Unclassified Information Technology Resources (OCT 2008) Contractor Personnel Security and Suitability: All Contractor employees are subject to the same level of investigation as VA employees who have access to VA Sensitive Information. The level of background investigation commensurate with the level of access needed to perform the statement of work is: NACI. This requirement is applicable to all subcontractor personnel requiring the same access. The contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM) through the VA, the contractor shall reimburse the VA within 30 days. Performance: The system shall be provided and installed at all facilities within 90 calendar days from receipt of award. _________________ INTERESTED VENDORS WHO PROVIDE A PRODUCT THAT CAN MEET THE REQUIREMENTS LISTED ABOVE SHOULD PROVIDE A CAPABILITY STATEMENT TO INCLUDE THE FOLLOWING INFORMATION: -Vendor Contact Information to include name, phone, address, and e-mail - Socioeconomic Status (i.e. small business, service disabled veteran owned small business, etc.) -DUNS Number of Entity -Detailed Description of the product to be provided and system capabilities (Product Literature should suffice) -Do you provide installation of the product? -If you provide a product that does not meet all of the items listed in the Scope of Work, identify which items your product does not include.
