- Laser Safety Officer Sources Sought Synopsis: The Department of Veterans Affairs is conducting this Market Survey for qualified firms capable of providing a requirement to provide monitoring and enforcing control on laser hazards, and to effect the knowledgeable evaluation and control of laser hazards at the VA North Texas Health Care System (VANTHCS) campus, 4500 S. Lancaster Road, Dallas, TX 75216. Please note that this is NOT a request for quotes or proposals. The Government is seeking information for market research purposes only. The purpose of this sources sought is to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business) relative to NAICS 541620. The Government may or may not issue solicitation documents. Responses to this Sources Sought will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company name, a service capability statement, examples of similar facilities to which you provided similar services to, DUNS number, address, point of contact and the size of your business pursuant to the following questions: (1) is your business large or small? (2) Is your firm a woman-owned or operated business? (3) Is your firm a certified Service-Disabled Veteran-Owned Small or Veteran-Owned Small Business? E-MAIL: brian.delaney@va.gov SCOPE OF WORK Service provider must have been engaged in evaluating laser hazards and installing and establishing laser control to ensure compliance with requirements of medical surveillance of the laser found in American National Standards Institute (ANSI) Z136.3-2005, American National Standard for Safe Use of Lasers in Health Care Facilities. The service provider will oversee the VA North Texas Health Care Center Laser Safety Program. Conduct comprehensive safety review for every Class 2 or higher laser system where direct beam viewing cannot be avoided. Establishes a Nominal Hazard Zone (NHZ) in the space around a Class 3a or higher laser operated in open areas within the VA Medical Center hospital in which exposure to laser light can exceed the Maximum Permissible Exposure (MPE) limit. Conduct Annual self-assessment of the Laser Safety Program and report deficiencies. Assists with the VA surgeon privileging process for laser use. Provide annual VA in-service training (provided training should be in accordance with Appendix F, ANSI Z136.3 “ 2005) in laser activity for applicable staff to include VA operating room staff, clinic staff and surgeons. Provide consultation to biomedical staff for maintenance & repair of laser systems. Assists with the evaluation of all new lasers purchased, or brought onto VA property. Provide a detailed summary of all laser training and a list of attendees to the Senior Industrial Hygienist. The North American Industry Classification System (NAICS) code for this requirement is 541620. The size standard for this NAICS code is $7 million.
