Description
Scope of Work Checklist Hazard Analysis Accident Report RESEALING/RESTRIPING OF UPPER AND LOWER PARKING AREAS AND ROADWAY The work to be performed consists of providing equipment, safety equipment, tools, materials and supervision to perform the resealing/restriping of the Upper and Lower Parking areas and the roadway for the Army Corps of Engineers, Buffalo District. INTERFERENCE WITH BUSINESS: •A. The contractor shall furnish sufficient personnel to perform the work specified. The employees shall conduct themselves in a proper and productive manner at all times and shall not loiter. Employees of the contractor assigned for performance of this contract shall be capable employees with sufficient experience and training in resealing/restriping of parking areas and roadway. The contractor shall not employ those persons whose presence would violate Standards of Conduct, DOD 5500.7-R (DOD Joint Ethics). The contractor shall not knowingly allow any employee who is under the influence of alcohol, drugs, or other incapacitating agents to perform work under this contract. GENERAL SAFETY REQUIREMENTS: •A. Contractor shall take safety and health measures in performing work under this contract. Contractor is subject to applicable federal, state, and local laws, regulations, ordinances, codes, and orders relating to safety and health. •B. During the performance of work, the contractor shall comply with procedures prescribed for control and safety of persons visiting the site. Contractor is responsible for his personnel and for familiarizing each of his subcontractors with safety requirements. Contractor shall advise the Contracting Officer of any special safety restriction he has established so that Government personnel can be notified of these restrictions. The contractor shall permit safety inspections of all work being performed. C. The contractor shall follow all Safety Requirements detailed in the Unified Facilities Guide Specifications (USACE UFGS 01-35-26, February 2009), and applicable Safety & Health Requirements Manual per EM 385-1-1. EM 385-1-1 is available on-line at: http://www.usace.army.mil/CESO/Pages/EM385-1-1,2008NEW!.aspx D. Contractor will complete an Accident Prevention Plan (APP). The Accident Prevention Plan will be submitted, reviewed and accepted prior (two weeks) to the start of the work. The APP will be submitted to: U.S. Army Corps of Engineers, Buffalo District Safety Office, Mr. Keith Hall, 1776 Niagara Street, Buffalo NY 14207. E. Attached is a Contractor Abbreviated APP checklist. This checklist can be used as an aid to develop and complete the required Accident Prevention Plan. F. Contractor will maintain a complete inventory of all chemicals in use, to include Material Safety Data Sheets (MSDS's) for each product. Per USACE & OSHA requirements, contractor will maintain this documentation on-site where the products are in use. Contractor will provide the USACE Safety Office with a copy of chemical inventory and all MSDS's. Q UALITY ASSURANCE: •A. The Contractor shall provide for the safe, careful and efficient operations of all tools and equipment. All tools and equipment shall be in good operating condition when calibrated, when required. •B. All employees shall possess required licenses/certificates as required by federal, state, and local authorities. PROCUREMENT: Contractor shall be responsible for controlling procurement sources and those of his subcontractors to ensure that each purchase meets quality requirements. GOVERNMENT FURNISHED PROPERTY AND SERVICES: The Government will not provide any facilities, materials, equipment, supplies or vehicles to the contractor. CONTRACTOR FURNISHED ITEMS: •A. The contractor shall supply all equipment, tools, maintenance products, etc. necessary to perform the tasks, as specified. •B. All equipment must be in good working condition. •C. All equipment, tools, maintenance products, etc. used by the contractor shall be in compliance with OSHA, EPA and other DOD directives. SPECIFIC TASKS: Contractor is to provide all labor, materials and services required to reseal and stripe the upper parking lot, lower parking lot and roadway. •A. All areas shall be cleaned before resealing. •B. Sealing shall have a sand mix. The sand shall be mixed in with the sealer prior to being applied. The sand shall not be applied after the sealer has been applied. C. Areas that have grease and oil spots shall be treated and removed prior to resealing. All sink holes will be repaired. •D. All parking areas shall be striped. •E. All handicap spaces shall be marked with handicapped symbol on blacktop. •F. Road shall have a center line applied. •G. All restricted parking areas are to be striped appropriately, in front of Building 5; south side of Building 2; and double door exit at southwest corner of Building 5. •H. Resealing of the upper & lower parking lots shall be performed on Saturday. The actual date that the work shall be completed will be determined after the awarding of the contract. •I. All striping shall be performed on the Sunday after the scheduled Saturday of resealing. The actual date that the work shall be completed will be determined after the awarding of the contract. •J. If the scheduled dates to do the resealing and restriping cannot be performed due to inclement weather, the work shall be accomplished on the following weekend.
