Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 6.302. Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive proposals. However, all proposals received prior to 05/10/2011 will be considered by the Government. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. NAICS code is 562910 size standard 500 employees. Scope of Work: Contractor to complete a decommissioning report of space in Building 2 at the Palo Alto Campus. Contractor will conduct tests for residual radioactivity potentially created by previous functions of the space in the building. Specific room locations to be provided by the Contracting Officer Technical Representative (COTR) 2. Site Location VA Palo Alto Health Care System (VAPAHCS) Palo Alto Division, Building 2 3801 Miranda Avenue Palo Alto, CA 94304 3. Specific Areas of Emphasis -COTR will provide site maps and detailed drawings of proposed area of work. Any questions regarding these details should be referred back to the COTR. Locations of work will include rooms within Building 2. 4. General Background - The VA Palo Alto Health Care System consists of three inpatient facilities located at Palo Alto, Menlo Park, and Livermore, plus seven outpatient clinics in San Jose, Capitola, Monterey, Stockton, Sonora, Modesto and Fremont. VAPAHCS operates nearly 900 beds, including 3 nursing homes and a 100-bed homeless domiciliary - all designed to serve more than 85,000 enrolled veterans. The Palo Alto Facility serves as the primary campus for patient care and is undergoing tremendous growth and change to provide specialized facilities and programs for its veteran population. 5. Description of Work - Contractor shall conduct the analysis according to NUREG 1575, Multi-Agency Radiation Safety and Site Investigation Manual (MARSSIM) protocols. Final report shall be suitable for submission to the Nuclear Regulatory Commission (NRC). 5.1Contractor shall use the following guidelines for conducting research, assessments, on- site analytical services, and report of findings: 5.1.1Historical Site Assessment - Interviews with pertinent personnel who can provide background information on the specific use of each room or space to be surveyed. 5.1.2Characterization Survey - Document necessary field measurements, as per guidelines, and provide data analysis as to results. 5.1.3Reporting of Findings - Provide all necessary findings to COTR, and when necessary, any recommended methods of remediation. 5.1.4Final Status Report - Provide final analysis of all space, including results of any remediation efforts. 5.1.5The following rooms in Building 2 are to be included in the survey. Should the historical site assessment reveal the need for additional surveying, contractor will notify COTR immediately: 1.005B 2.A008 3.A009 4.A010 5.A5 6.6 7.B121 8.B122 9.B123 10.B124 11.B126 12.B127 13.B128 14.B131A 15.B131B 16.B131C 5.2Final Report and Findings shall include, but not limited to, the following items: 5.2.1Methods processes, and equipment used to survey each room 5.2.2Room by room breakdown of all findings and associated mediation recommendations 5.2.3Radiation classifications and descriptions should be provided 5.2.4Description and impact of any mediation efforts taken 5.2.5Final survey status and assessment of each room 5.2.6 Final recommendation of the space pertaining to VA's desire to deactivate and demolish the building. 5.3Contractor shall provide a list of all resources or agencies contacted during the preparation, research, and writing of the submittal document. 5.4Tasks and Deliverables - Contractor to provide the following documentation of findings: 5.4.1Contractor shall contact COTR upon receiving Notice to Proceed (NTP) to discuss and set up access to building and space. 5.4.2Estimated scheduled of completion shall be provided to the COTR at time of NTP. 5.4.3Final copy of the report shall be submitted in electronic (PDF) and printed format (1 Copy) due to COTR 21 Days after agreed upon site survey completion date. 6. Site Impact of Services and Response - Normal business hours at the VA Palo Alto Health Care System are 8:00am to 4:30pm. Special time arrangements can be made by contacting the COTR. Should emergency services be required during business hours, contractor shall coordinate with COTR. Contractor shall abide by all station policies when on site. Attachment One: VISN 21 90 CCA, Contractor Personnel Security Requirement, dated March 1, 2011, total 5 pages. Attachment Two: Wage Determination Rev 10, dated 07-16-2010, total 13 pages.
