Description
Performance Work Statement C.1. GENERAL C.1.1 Scope of Work - The Contractor shall furnish all personnel, equipment and supplies to perform all work required to hand plant 1-year old container grown longleaf pine seedlings on 2,500 acres of the Fort Benning Military Reservation.' C.1.2 Scheduling of Services: Land Management Branch (LMB) personnel will schedule the Contractor's tree planting operations with Range Division, DPTM, in order to prevent interference with military training. C.1.3 Work Area: All tree planting work described below shall be accomplished within areas located on the Fort Benning Military Reservation. Exact areas will be determined by LMB personnel at time of planting. Areas have recently been harvested and will have had an application of herbicide prior to planting. Areas may also be burned prior to planting. Planting areas range in size from 1 acre to 300 acres. Average size planting area is estimated at 26 acres. The work area shall be cleared of any cans, containers or lunch debris deposited by Contractor personnel on a daily basis. C.1.4 Contractor Qualification: The Contractor shall possess any required Federal, State or Local licenses or certificates necessary to provide contract services as described in the contract. In addition, the Contractor shall be able to locate the contract work sites using a 1:50,000 scale contour map. C.1.5 Quality Control/Quality Assurance: C.1.5.1 Quality Control: The Contractor shall maintain an inspection system, which shall identify deficiencies in the quality of services performed before the level of performance becomes unacceptable. C.1.5.2 Quality Assurance: The Government will monitor the Contractor's performance and inspection will be performed by LMB personnel to ensure compliance with the performance work statement. The Government will monitor and evaluate Contractor's performance under this contract by 100% inspection of services pursuant to AR 420-76, Chapters 3-12. C.1.6 Hours of Work : Work under this contract shall be performed during the hours of 7:45 am to 4:15 pm, Monday through Friday, excluding legal federal holidays. Exceptions to this must have prior approval from Range Control, the Contracting Officer or the LMB. C.1.7 Contract Work Period: Performance will be between 15 Sep and 28 Feb within each year of performance. C.1.8 Damage Caused by Contractor: The Contractor shall be held accountable and liable for any damage to Government facilities, equipment or grounds, caused by the Contractor or his employees. The purpose for this contract is to reforest pine trees on nonstocked areas of the Fort Benning Military Reservation. C.1.9 Project Manager: The Contractor shall provide an on-site project manager or a designated representative to be physically present during the normal hours of operation during the tree planting operations to act with full authority for the Contractor. This individual shall be responsible for the supervision, overall administration, and coordination of the required services. The project manager shall be the point of contact with the Government and have the authority to act or make decisions for the Contractor. C.1.10 Alternate: An alternate shall be designated to act for the project manager during absences such as illness. C.1.11 Emergency Medical Service: Medical services for the Contractor personnel are the responsibility of the Contractor. However, the Government will provide, on an emergency basis, medical services for on-the-job injuries while an employee is performing under this contract. Contractor shall reimburse the Government for emergency medical services provided upon receipt of invoice from the medical facility. Medical facilities are located at Building 9200, Martin Army Community Hospital, Fort Benning, GA. C.1.12 Safety Requirements: In order to provide safety controls for protection to the life and health of employees and other persons and for avoidance of work interruptions in the performance of this contract, the Contractor shall comply with all pertinent provisions of the publication "General Safety Requirements" EM 385-1-1, dated 84 Oct 01, prepared by the Department of the Army, Corps of Engineers, U.S. Army and published by the U.S. Government Printing Office. Contractor shall take or cause to be taken such additional measures as the Contracting Officer may determine to be reasonable and necessary for the purpose. C.1.12.1 The Contractor and his subcontractors shall maintain an accurate record of and shall report to the Contracting Officer in the manner and on the form prescribed by the Contracting Officer, exposure data and all accidents resulting in death, traumatic injury, occupational disease, or damage to property, materials, supplies, and equipment incident to work performed under this contract, all in accordance with MCOE Reg 385-2. C.1.12.2 The Contracting Officer shall give written notification to the Contractor of any noncompliance with the foregoing safety provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions. Such notices, when delivered to the Contractor or his representative at the site of the work, shall be deemed sufficient for the purpose of notification. C.1.12.3 A copy of the Department of the Army, Corps of Engineers, "Safety Requirements, referenced above, is available for use in the Directorate of Public Works, Construction Inspection Branch, Building 6, Third Floor. Prior to commencement of the work, the Contractor shall meet in conference with the Contracting Officer to discuss and develop mutual understanding relative to administration of the safety programs. C.2 DEFINITIONS: C.2.1 Quality Control: Those actions taken by a Contractor to control the production of goods or services so that they will meet the requirements of the Performance Work Statement. C.2.2 Quality Assurance: Those actions taken by the Government to check goods or services to determine if they meet the requirements of the contract. C.3 GOVERNMENT FURNISHED PROPERTY AND SERVICES: C.3.1 Maps shall be provided indicating the tree planting areas. All pine seedlings to be planted as described in this contract will be provided by the Government. Seedlings to be provided will be 1-year old container grown longleaf pine. Seedlings provided will be extracted from containers and be packed in boxes of 250 seedlings per box. Seedlings will be stored in a cold storage facility or refrigerated trailer on Ft Benning and will be provided at the cold storage facility or trailer to the Contractor on a daily basis. C.4 CONTRACTOR FURNISHED SUPPLIES AND SERVICES: C.4.1 Contractor shall supply all materials, labor and equipment necessary to perform the services described herein. C.5 SPECIFIC TASKS TO BE PERFORMED: C.5.1 Tree Planting: Tree planting shall be conducted by hand using a plug type planting bar. Seedlings shall be planted at a spacing of 6' x 12' (605 seedlings per acre). Only one seedling shall be planted in each debris free planting hole. Seedling shall be planted straight in the hole at a depth the leaves terminal bud above the soil surface. Soil shall be packed firmly around seedling roots. C.6 APPLICABLE DOCUMENTS: C.6.1. Directives applicable to the Performance Work Statement are listed below. These directives are coded as advisory or mandatory and shall be provided to the Contractor at the pre-work conference. TM 385-1-1, General Safety Requirements, 1 Oct 84, Mandatory USAIC Reg 385-2, Accident Prevention Program, 27 Feb 91, Mandatory AR 200-3, Natural Resources, Land, Forest and Wildlife Management, 28 Feb 95, Advisory Contractor Manpower Reporting FFP Contractor Manpower Data A. The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address: https://cmra.army.mil/default.aspx The required information includes: (1) Contract Number (2) Delivery Order Number (if applicable) (3) Task Order Number (if applicable) (4) Requiring Activity Unit Identification Code (UIC) (5) Command (6) Contractor Contact Information (7) Federal Service Code (FSC) (8) Direct Labor Hours (9) Direct Labor Dollars (10) Location Information (where contractor and subcontractors (if applicable) performed the services Reporting period will be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 15 October of each calendar year. If your particular contract crosses fiscal years, 2 entries must be made to capture the data for the contract period; for example if the contract start date is 1 January 2007 and ends 31 December 2007, the data for the period from 1 January 2007 through 30 September 2007 shall be entered not later than 15 October 2007 and the period 1 October 2007 through 31 December 2007 shall be entered not later than 15 January 2008. Contractors shall fill in the fields on the website. B. The contractor shall submit cost and pricing data in sufficient enough detail to substantiate any costs proposed.
